About this file
This document is a Solicitation Addendum (Addendum Number 2) issued by the Pennsylvania Department of Human Services (DHS) for the HEZ Community Health Workers grant opportunity (Solicitation Number: 25-RFA-13139). The addendum extends the response due date to July 18, 2025, at 12:00 PM and includes a comprehensive Questions & Answers Log that provides clarification on various aspects of the Request for Applications (RFA). Key details include confirmation that current HEZ grantees are eligible to apply, the potential grant agreement award timeframe is anticipated for April 2026, and there is flexibility in the implementation period for proposed projects.
The document provides critical guidance for applicants, including that there is no page limit for technical responses, no additional budget guidelines beyond those in the RFA, and no predetermined list of evidence-based interventions. The Department considers evidence-based projects as those relying on data and proven methods, and applicants will be evaluated based on the criteria outlined in the RFA. Nonprofit organizations seeking funding can request up to $500,000 per year, with administrative fees capped at 10% of the total proposed cost. The program aims to deploy Community Health Workers in North Philadelphia to address social determinants of health, improve healthcare access, and develop a culturally competent workforce to reduce health disparities in vulnerable communities.
