About this file
This document is a Price Increase Percentages form from the Arizona Department of Transportation (ADOT) Procurement division, associated with Solicitation No. BPM004114, which is available online at https://appstate.az.gov/. The form is designed for potential contract offerors to specify maximum price increase percentages for contract renewal periods.
The document outlines a structured approach to potential price increases during contract renewals, with four distinct renewal periods. Notably, the offeror is instructed to specify the maximum percentage increase for each renewal period, with the critical stipulation that increases are calculated against the original contract price. The State of Arizona emphasizes that price increases are not automatic and require documented justification at the time of renewal. An example table demonstrates how percentage increases would be calculated, showing incremental increases from 2% to 8% over four renewal periods, always based on the original contract price. The form provides clear guidance that if no increase percentages are specified, pricing will remain consistent with the original contract term.

The offeror should use the space below to identify the maximum price increase percentage that they may want to request at the time of renewal if ADOT exercises its option per the applicable provisions under any resultant contract(s).

2nd Renewal Period __No Increase will Occur_________________% Maximum Increase
3rd Renewal Period ___No Increase will Occur________________% Maximum Increase

The offeror is cautioned that each renewal period’s increase percentage shall be computed against the ORIGINAL contract price. If information is not provided in the space above, prices shall remain the same as the original contract term. Please see the following example for further clarification.
Example: If the offeror anticipates requesting a 2% increase at each renewal period per Special Terms and Conditions, they should enter 2%, 4%, 6%, and 8% in the attachment.
Each percentage increase is calculated using the original contract price. See example in table below.
