1.2 PROJECT INFORMATION
A. Project Identification/Project Name: Norway Beach Recreation Area Reconstruction Phase 2
– Sanitary Sewer System Replacement, Project No. CH 10619.
1. Project Location: The project is located approximately 8 miles east of the municipality of Cass Lake, Minnesota in the Chippewa National Forest, Blackduck Ranger District in Cass County.
B. Owner: Government or Owner refers to the USDA Forest Service.
1. Owner's Representative: The Federal Contracting Officer (CO) and his/her designated representative(s), as named in writing. Contracting Officer (CO) refers to the person assigned by the USDA Forest Service to administer the Contract. Soon after Contract award, the Contracting Officer will designate a Contracting Officers Representative
(COR).
1.3 REQUIREMENTS OF THE OFFEROR’S PROPOSAL
A. The Government DRAWINGS and SPECIFICATIONS are the basis for this Contract.
B. Those products listed in the SPECIFICATIONS are preferred products, chosen for compatibility, longevity, durability and quality. Equal or better alternatives submitted by the OFFERER will be considered but must be approved by the CO/COR prior to use in the Project.

Bidders are cautioned to bid the Project as designed, as assumed equal or better alternatives may not be accepted by the Government and the selected Contractor will be required to provide the Work as specified.
1.4 WORK COVERED BY CONTRACT DOCUMENTS
A. The Work of the Project is defined by the Contract Documents and consists of the following:
1. Mobilization to site, construction surveying and staking, quality control including sampling and testing, erosion control, traffic control and protection of the public users and other occupants of the Recreation Area for the duration of the contract.
2. Selective demolition/removals, tree/vegetation clearing and grubbing, excavation, dewatering, construction of six (6) new sanitary septic systems including septic/holding tanks, sewer lines, manholes, lift stations, mound treatment systems and associated primary and secondary electrical improvements and site restoration work.
3. Perform all septic system leak testing, systems start-up/commissioning and training of FS personnel on system operation and furnishing Operation and Maintenance Manuals to the Owner.
4. Other work indicated in the Contract Documents.
B. Type of Contract:
1. Project will be constructed under a single prime contract.
1.5 PHASED CONSTRUCTION
A. The Work shall be conducted in three (3) phases, with each phase substantially complete as indicated.
The primary intent of the Work phasing is to ensure that during the camping season, two out of the three project area campgrounds are open for complete and full use of the restroom/shower facilities at any one given time: This includes Norway Beach Campground, Chippewa Campground and Wanaki Campground. The camping season is typically from Memorial weekend, starting Thursday (5/22/2020) through the end of September (9/30/2020).
1. Phase One (1): All work identified at the Visitor Information Center (VIC) and Norway Beach Campground East Loop and West Loop project areas.
a. Commencement of Construction:
1) Notice to Proceed: Work of this phase shall commence within ten (10) days after the Notice to Proceed (or within ten days of execution of the Contract if a notice to proceed is not issued).
2) Start Date Work Restrictions: Construction site work of this phase shall not commence before July 6, 2020.

b. Sequencing for Phase 1 Work:
1) Prior to decommissioning/demolition of the existing sewer lines at the West Loop project area, both of the Norway Beach CG Restroom/Shower buildings must be locked and secured to prevent untreated sanitary sewer flows to the environment.
c. Substantial Completion:
1) On or before September 8, 2020.
2) Substantial Completion includes commissioning, start-up and acceptance of the Phase 1 sanitary sewer systems.
2. Phase Two (2): All work identified at the Wanaki Campground East Loop and West Loop project areas.
a. Commencement of Construction:
1) Start Date Work Restrictions: Construction site work of this phase shall not commence before September 8, 2020 and shall also not commence before substantial completion of the Phase 1 project.
2) Exception to the Start Date Work Restriction: Construction of the Primary Electrical improvements by (or in cooperation with) Beltrami Electric Cooperative, can commence prior to September 8, 2020.
b. Sequencing for Phase 2 Work:
1) Prior to decommissioning/demolition of the existing sewer lines at the West Loop project area, both of the Wanaki CG Restroom/Shower buildings must be locked and secured to prevent untreated sanitary sewer flows to the environment.
c. Substantial Completion:
1) On or before October 30, 2020.
2) Substantial Completion includes commissioning, start-up and acceptance of the Phase 2 sanitary sewer systems.
3. Phase Three (3): All work identified at the Chippewa Campground East Loop and West Loop project areas.
a. Commencement of Construction:
1) Start Date Work Restrictions: Construction site work of this phase shall not commence before September 28, 2020 after the campground has closed for the season.
b. Sequencing for Phase 3 Work:

1) The Chippewa Campground is the only campground with available electric camping sites at the Norway Beach Recreation Area. Because of this fact, either the East Loop or the West Loop project work must be substantially complete, commissioned and accepted prior to the Memorial holiday weekend in 2021.
2) It is the intent of the Owner to make available one completed Chippewa Campground loop for the entire 2021 camping season. The Contractor must plan accordingly to provide temporary traffic control and accommodate the traveling public.
c. Substantial Completion:
1) East or West Loop on or before May 27, 2021.
2) Remaining Loop on or before June 25, 2021.
3) Substantial Completion includes commissioning, start-up and acceptance of the Phase 3 sanitary sewer systems.
B. Before commencing Work of each phase, submit an updated copy of Contractor's construction schedule showing the sequence, commencement and completion dates for all phases of the Work.
1.6 WORK UNDER SEPARATE CONTRACTS
A. General: Cooperate fully with separate Government contractors, volunteers and employees so that work performed on those contracts to operate the Norway Beach Recreation Area may be carried out smoothly, without interfering with or delaying Work under this Contract or other contracts.
Coordinate the Work of this Contract with work performed under separate contracts.
1.7 CONTRACTOR'S USE OF SITE AND PREMISES
A. The Government will conduct a pre-construction survey with the Contractor to review and document the existing project conditions prior to construction.
B. Restricted Use of Site: Contractor shall have limited use of Project site for construction operations as indicated on Drawings by the Contract limits and as indicated by requirements of this Section.
C. Limits on Use of Site: Limit use of Project site to areas within the Contract limits indicated. Do not disturb portions of Project site beyond areas in which the Work is indicated.
1. Limited space on the premises is available for the Contractor's storage and related activities, as staked by the Government. Schedule deliveries to minimize on-site storage of material and equipment.
2. Existing materials and equipment that are removed as part of the construction operations, and that are not reused or salvaged as Government property, shall become the property of the Contractor and shall be promptly removed from the site.

D. Condition of Existing Grounds: Maintain portions of existing grounds, landscaping, and hardscaping affected by construction operations throughout construction period. Repair damage caused by construction operations to same condition as pre-construction condition.
1.8 COORDINATION WITH OCCUPANTS/GOVERNMENT OCCUPANCY
A. Partial Owner Occupancy: The Owner and the Public will occupy the premises during entire construction period. Cooperate with Owner during construction operations to minimize conflicts and disruption and facilitate usage by the Owner and Public. Perform the Work so as not to interfere with Owner's operations. Maintain existing exits unless otherwise indicated.
B. Owner Limited Occupancy of Completed Areas of Construction: Owner reserves the right to occupy and to place and install equipment in completed portions of the Work, prior to Substantial Completion of the Work, provided such occupancy does not interfere with completion of the Work. Such placement of equipment and limited occupancy shall not constitute acceptance of the total Work.
1.9 WORK RESTRICTIONS
A. Comply with restrictions on construction operations.
1. Comply with limitations on use of public streets, work on public streets, rights of way, and other requirements of authorities having jurisdiction.
B. On-Site Work Hours: Limit work in all project areas to normal business working hours of 8 a.m.
to 6 p.m., Monday through Friday, unless otherwise indicated.
1. No work is permitted without written authorization on Government Holidays or weekends.
C. Existing Utility Interruptions: Do not interrupt utilities serving facilities occupied by Owner or others unless permitted under the following conditions and then only after providing temporary utility services according to requirements indicated:
1. Notify Owner not less than two days in advance of proposed utility interruptions.
2. Obtain Owner's written permission before proceeding with utility interruptions.
D. Noise, Vibration, Dust, and Odors: Coordinate operations that may result in high levels of noise and vibration, dust, odors, or other disruption to Owner occupancy with Owner.
1. Notify Owner not less than two days in advance of proposed disruptive operations.
2. Obtain Owner's written permission before proceeding with disruptive operations.
E. Smoking and Controlled Substance Restrictions: Use of tobacco products, alcoholic beverages, and other controlled substances on Project site is not permitted. Use of any type of marijuana on federal property is illegal.

1.10 SPECIFICATION AND DRAWING CONVENTIONS
A. Division 00 Contracting Requirements: General provisions of the Contract, including General and Supplementary Conditions, apply to all Sections of the Specifications.
B. Division 01 General Requirements: Requirements of Sections in Division 01 apply to the Work of all Sections in the Specifications.

SECTION 012700 - MEASUREMENT AND PAYMENT
PART 1 - GENERAL
1.1 RELATED DOCUMENTS
A. Drawings entitled “Norway Beach Recreation Area Reconstruction Phase 2 - Sanitary Sewer System Replacement” and applicable provisions of the Solicitation/Contract including but not limited to: Section B - Schedule of Items (and associated Schedule of Values), Section C - Statement of Work, Section G - Contract Administration Data/Invoices and Payments and Section I - Contract Clauses.
1.2 SUMMARY
A. This Section includes:
1. Measurement and payment criteria applicable to the work performed.
2. Administrative and procedural requirements necessary to prepare and process Applications for Payment.
3. Defect assessment and non-payment for rejected work.
4. General description of work included under each pay item.
B. Related Requirements:
1. Schedule of Values Form (SOV): The Contractor is required to complete the SOV Form, provided by the Government, as part of their Proposal or Bid together with the completed Section B – Schedule of Items. Totals from the SOV Form must agree exactly with the corresponding item totals on the Schedule of Items.
2. Invoice Processing Platform (IPP) System: All invoices are to be submitted via the electronic IPP System. The contractor must follow the instructions on how to register and submit invoices via IPP as prescribed in communications from USDA and Treasury. This is a mandatory requirement initiated by the U.S. Department of Treasury and you can find more information at this website https://www.ipp.gov/index.htm. Ensure your company has registered at https://www.ipp.gov/vendors/enrollmentvendors.htm to establish your account. Offeror must have an established account prior to commencement of work.
3. Certified Payrolls: Certified payrolls for construction work must be submitted to the CO/COR for review at regular intervals as work progresses. At least one week prior to Contractor requesting payment via IPP, all certified payrolls related to the subject pay request work must have been received by the CO/COR. Incomplete or inaccurate payroll submittal will result in rejection of the IPP invoice. Contractor and all subcontractors, through the contractor, shall furnish all certified payrolls covering payments to laborers and mechanics for on-site work. Payroll hourly rates and fringe benefits must be in accordance with the established Wage Determination for the project.
1.3 AUTHORITY
A. The Contracting Officer’s Representative (COR) will take all measurements and compute quantities accordingly.

A. Linear Measurement: Measured by the linear dimension, (i.e. LF = lineal foot), at the item centerline or mean chord. Measurement will be made to the nearest given linear unit.
B. Each: Measured by the number of individual items of work completed.
C. Lump Sum Quantities (LSQ): These quantities denote one complete unit of work as required by or described in the Contract, including all necessary materials, equipment, and labor to complete the job. They will not be measured.
D. Actual Quantities (AQ): These quantities denote completed units of work, as described in the Contract, including all necessary materials, equipment, and labor, to complete the job. They will be measured in the field.
E. Designed Quantities (DQ) - These quantities denote the final number of units to be paid for under the terms of the contract. They are based upon the original design data available prior to advertising the project. Original design data include the preliminary survey information, design assumptions, calculations, drawings, and the presentation in the contract. Changes in the quantity of units SHOWN in the SCHEDULE OF ITEMS may be authorized under any of the following conditions:
1. As a result of changes in the work authorized by the Contracting Officer.
2. As a result of the Contracting Officer determining that errors exist in the original design data used to determine designed quantities, which cause a pay item to change by 15 percent or more.
3. As a result of the Contractor submitting to the Contracting Officer a written request showing evidence of errors in the original design data used to determine design quantities that cause a pay item total to change by 15 percent or more. The evidence must be verifiable and consist of calculations, drawings, or other data that show how the designed quantity is believed to be in error.
1.5 PAYMENT
A. Pay Request Documentation from Contractor: Contractor must submit the following documentation to the CO/COR for review and approval at least seven (7) days prior to requesting a payment via IPP.
1. All Certified Payroll forms from the general contractor (including payrolls from all sub-contractors) for work covered by the pay request.
2. A completed draft Schedule of Values (SOV) Form for work covered by the pay request, indicating the percent complete for each line item in the SOV.
3. Written Quality Control testing results if applicable to items in the payment request.
B. Payment Includes: Full compensation for all required labor, products, tools, equipment, plant, transportation, services, and incidentals; erection, application, or installation of an item of the Work; overhead and profit.
1. "Incidentals" are to be considered as an additional obligation to the other work required for the item(s). Incidental work is not payable directly, but is to be considered included in the contract price for the item(s) of work specified.
2. All work not specifically identified as a separate bid item is "Incidental".
C. The accepted quantities will be paid for at the contract unit price for each item as shown in the Schedule of Items.

D. Payment will not be made for any of the following:
1. Products or construction placed beyond the lines and levels of the required work.
2. Products or construction determined as unacceptable before or after placement.
3. Products wasted or disposed of in a manner that is not acceptable.
4. Products remaining on hand after completion of the Work.
5. Loading, hauling, and disposing of rejected products.
1.6 DESCRIPTION OF ITEMS
The following information describes generally what items of work are included in each bid item. See individual specification sections and the drawings for more detailed information.
A. DIVISION 1 – GENERAL REQUIREMENTS:

Description of work – Pay Item #3: Salvage light pole and fixture, Removals including sanitary manhole #8, CIP sewer pipe, abandoned septic tank and concrete light base, Remove asphalt paving, Reconstruct siphon manhole #2, Drain field area vegetation clearing and surface preparation, Clearing and Grubbing general construction limits, furnish and Install concrete 3,600 gallon/1,200 gallon septic/pump combination tank, Furnish and Install concrete 6,000 gallon/2,000 gallon septic/pump combination tank, Construct 4" Dia. SECTION 012800 - MOBILIZATION
PART 1 - GENERAL
1.1 SUMMARY
A. This work consists of moving personnel, equipment, material, and incidentals to the project, and performing all activities necessary to accomplish the work at the project site. This work also includes obtaining permits, insurance, and bonds (if required).
1.2 MEASUREMENT AND PAYMENT
A. Method of Measurement will be by Lump Sum Quantity (LSQ).
B. The maximum Mobilization bid price shall not exceed ten percent (10%) of the total base bid contract price or optional bid contract price (not including Mobilization).
C. Progress Payments for Mobilization will be in accordance with the following schedule:
1. The cost of premiums paid for performance and payment bonds are considered incidental to Mobilization and will not be paid for separately. Payment for bond premiums will be in accordance with FAR Clause 52.232-5, Payments under Fixed-Price Construction Contracts, after receipt of the evidence of payment.
2. Progress payments for the Base Bid Mobilization lump sum price will be prorated as follows:
a. Visitor Information Center (VIC) Project Area = 20%
b. Norway Beach Campground Loops Project Area = 40%
c. Chippewa Campground Loops Project Area = 40%
3. 50 percent of the prorated lump sum, not to exceed 5 percent of the original contract amount for each project area will be paid following completion of 5 percent of the original contract amount for each project area (not including mobilization).
4. 100 percent of the prorated lump sum, not to exceed 10 percent of the original contract amount for each project area will be paid following completion of 10 percent of the original contract amount for each project area (not including mobilization).

SECTION 013100 - PROJECT MANAGEMENT AND COORDINATION
PART 1 - GENERAL
1.1 SUMMARY
A. This section includes certain administrative provisions for managing and coordinating construction operations.
1.2 GENERAL PROJECT COORDINATION
A. Coordination of Trades: Coordinate construction operations to provide an efficient and orderly installation of each part of the Work.
1.3 CONSERVATION
A. Use best management practices for the conservation of energy, water and materials during construction operations.
1.4 SPILL AND EROSION CONTROL
A. Spill and Erosion Control Plans: The Contractor shall develop a plan to minimize site erosion from construction activities, to prevent sediment from entering wetlands or waters of the United States and to provide for containment of hazardous materials and unplanned spills in accordance with all applicable federal, state and local laws. Submit a plan in accordance with paragraph 1.7 in this section. Permit will not be required for work on federal land.
1.5 ADMINISTRATIVE AND SUPERVISORY PERSONNEL
A. In addition to meeting the requirements of FAR 52.236-6 for an on-site Project Superintendent, the Contractor shall provide other administrative and supervisory personnel as required for performance of the Work.
1.6 CONFERENCES AND MEETINGS
A. Pre-construction Conference: The Contracting Officer or Contracting Officer’s Representative will schedule a pre-construction conference before starting construction at a time and place agreeable and convenient to both the Contractor and Government and will review responsibilities and personnel assignments in accordance with FAR 52.236-26
B. Progress Meetings: Progress meetings at the Project Site shall be conducted at regular intervals.
Dates of meetings shall be coordinated with preparation of the payment request in accordance with FAR 52.232-5.

1. Agenda: Include topics for discussion as appropriate to the status of the Project. Include review of the Contractor's Construction Schedule and of the present and future needs of each entity present.
2. Schedule Updating: If the progress of the work falls behind the approved Contractor’s Construction Schedule, the Contractor shall submit a supplementary schedule for approval by the Contracting Officer in accordance with FAR 52.236-15. The COR will keep the Contractor informed of any scheduled events that may impact the contractor’s work schedule. It is imperative the Contractor provide an accurate Work Schedule to the COR and shall update it whenever the current Work Schedule is not accurate.
1.7 SUBMITTALS
A. Spill and Erosion Control Plans shall be submitted within 5 calendar days after Notice to Proceed.
B. Staff Names: Within 5 calendar days after Notice to Proceed, submit a list of principal staff assignments, including the Superintendent and other primary personnel. Identify individuals by name, duties and responsibilities, home address, and business/cell and home (in case of emergencies after hours) telephone numbers. Post copies of this list in the temporary field office (if present).
C. Construction Schedules shall be submitted as directed in FAR 52.236-15, Schedules for Construction.

3.1 GENERAL COORDINATION PROVISIONS
A. Inspection of Conditions: Prior to installations, require the installer of each major component to inspect both the substrate and conditions under which work is to be performed.
B. Construction in Progress: Keep construction in progress, adjoining materials in place, and clean during handling and installation. Apply protective enclosures or coverings for protection from damage or deterioration.
C. Completed Construction: Clean completed construction and provide maintenance to prevent damage, soiling or other deterioration through the remainder of the construction period. Adjust and lubricate operable components to assure operability without damage.
D. Limiting Exposures: Supervise construction operations to prevent exposure of any part of construction, completed or in progress, to harmful, dangerous, damaging or otherwise deleterious conditions during the construction period.

SECTION 013300 - SUBMITTAL PROCEDURES
PART 1 - GENERAL
1.1 SUMMARY
A. This Section and FAR 52.236-21 include requirements for Shop Drawings, as defined by
FAR 52.236-21 and Designs. Shop drawings and Designs may include drawings, specifications, diagrams, layouts, schematics, descriptive literature, illustrations, schedules, performance and test data furnished by the Contractor to explain in detail specific portions of the work required by the Contract.
B. This section includes requirements for Samples. Samples may include mailed samples, field samples or mock-ups of full-size physical examples erected on-site or elsewhere.
C. This section includes requirements for Product Data. Product Data includes manufacturer's standard catalogs, manuals, pamphlets and other printed materials, and includes but is not limited to the following: product specifications and installation instructions, color charts and catalog cuts, rough-in diagrams and templates, wiring diagrams, performance curves, operational range diagrams, mill reports, test reports, shop drawings and code approvals.
D. This section includes other quality assurance submittals such as certifications of compliance or conformance, manufacturer's instructions and field reports, and mix designs.
1.2 GENERAL SUBMITTAL REQUIREMENTS
A. Coordination: Coordinate preparation and processing of submittals with performance of construction activities and with the schedule.
1. Allow 15 calendar days each for submittal review, corrections following the initial review and re-submittal review before activities scheduled after the submittal approval.
2. Incomplete submittals will be returned to the Contractor with no further review. Delays due to incomplete or rejected submittals will not be excused and no compensation in the form of monetary or time will be provided to the Contractor.
3. When the Contractor desires to submit for approval a product for an “approved equal”, the Contractor MUST provide documentation of the original specified product and the Contractor proposed product in a side-by-side comparison, such as a table, which clearly demonstrates the Contractor proposed product meets or exceeds the originally specified product. If this product comparison is not done the first time it is submitted, the proposed product will be rejected, and the Contractor will be required to provide the specified product. The Government is not obligated to review a re-submittal for a rejected product.
4. Submittal Transmittal: Transmit each submittal from the Contractor to the Government by use of one transmittal form per submittal. Transmittal format to be approved by CO, and shall include the following information on the transmittal form.
a. Project name and Forest Service contract number.
b. Date.
c. Name, address and telephone number of firm or entity that prepared the submittal
d. Names of subcontractor, manufacturer, supplier, etc. as applicable.
e. Category and description of submittal.
f. Section of Specifications or Drawing sheet number the Submittal is representing.

g. Certification by Contractor stating that submittal complies with the Contract, or statement of deviations from the requirements of the Contract.
h. Signature of transmitter.
i. Space to record Contractor's review and approval markings, and for Government response.
B. Submit 3 hardcopies of each submittal to the COR for review unless otherwise stated. One copy will be returned to the Contractor and two copies will be retained, one for the COR and one for the Inspector.
1.3 SHOP DRAWINGS AND DESIGNS
A. Submit originally prepared information, drawn accurately to scale.
B. Submittals: Submit three blue-line or black-line prints of each drawing submittal. One print will be marked with action taken and returned. Distribution: When submittal is approved, the Contractor shall prepare and submit three final copies.
C. Shop drawings shall be done in AutoCAD format. Upon final approval the drawings shall be sent on a thumb drive or via email to the COR.
D. Specifications shall be done in Microsoft Word format and a copy sent on a thumb drive or via email to the COR.
1.4 PRODUCT DATA
A. Collect Product Data into a single submittal for each system or element of construction.
Mark each copy to show specific product choices and options applicable to the project.
Product Data shall include the following information, where applicable:
1. Manufacturer's printed recommendations.
2. Compliance with recognized trade association standards.
3. Compliance with recognized testing standards.
4. Applicability of testing agency labels and seals.
5. Notation of dimensions verified for fit by field measurements.
6. Notation of coordination requirements.
B. Preliminary Submittal: Prior to submittal of complete Product Data, submit a preliminary single copy of that part of Product Data when selection of options is required, such as for color charts. Preliminary submittal will be returned, with selection noted, for the Contractor's use in subsequent submittals.
C. Submittals: Unless otherwise indicated, submit 3 copies of each Product Data submittal. Two copies will be retained, and the remaining copy will be marked with action taken and returned.
The COR may allow electronic copies to be transmitted in lieu of hard copies.
D. Material and Product substitution shall be in accordance with FAR 52.211-6 and Section 013300-1.2.A.3.
1.5 SAMPLES
A. Submit full-size, fully fabricated samples, cured and finished in the manner specified.
Samples shall be physically identical to the material or product proposed for use.
B. Preliminary Submittal: Where color, pattern, texture or similar characteristics are to be selected from a manufacturer's range of standard choices, submit a preliminary single set sample of available choices when required by the CO.

1.6 OTHER QUALITY ASSURANCE SUBMITTALS
A. Submit other quality assurance submittals in compliance with requirements in the individual specification sections.
1.7 REVIEW ACTION ON SUBMITTALS
A. Except for submittals for the record or for information or for another purpose where no action and return is required, the Government will review submittals and mark returned copies to indicate action taken within a maximum time period of 15 calendar days.
B. Submittals that do not contain an appropriate marking of approval by the Government shall not be used for construction.

SECTION 014000 - QUALITY CONTROL
PART 1 - GENERAL
1.1 SUMMARY
A. This section, FAR 52.246-1 Contractor Inspection Requirements and FAR 52.246-12 Inspection of Construction, include requirements for Contractor quality control services used to verify quality assurance requirements specified elsewhere in the Contract.
B. The Government reserves the right to conduct independent tests.
C. This section includes requirements for quality control services such as soil density compaction, concrete mix and strength testing, sewer line pressure tests and bedding and sand gradation tests per State of Minnesota Agency Requirements – MnDOT, MPCA and MDH – as they apply to the work being performed.
D. The quality control services include tests and related actions, including reports, performed by the Contractor, independent testing agencies or governing authorities.
1.2 QUALIFICATIONS OF INDEPENDENT TESTING AGENCIES
A. A qualified independent testing Agency shall be an accredited entity engaged to perform tests or inspections, and to report on and, if required, to interpret results of those tests or inspections.
1.3 SUBMITTALS
A. Reports: The Contractor shall submit a certified written report, in duplicate (or electronic format), of each test, inspection or other quality control service to the Government.
B. Written reports shall include but not be limited to the following:
1. Date of issue.
2. Project title and number.
3. Name, address, and telephone number of testing agency.
4. Dates and locations of samples and tests or inspections.
5. Names of individuals making the test or inspection.
6. Designation of the work and test method.
7. Identifications of product and specification section.
8. Complete test or inspection data.
9. Test results and an interpretation of test results.
10. Ambient conditions at the time of sample taking and testing.
11. Comments or professional opinion on whether tested or inspected Work complies with
Contract Document requirements.
12. Name and signature of laboratory inspector.
13. Recommendations on retesting.

1.5 RESPONSIBILITIES
A. Contractor Responsibilities: The Contractor shall provide for tests, inspections and other quality control services where specified in the Contract or when required by authorities having jurisdiction. Costs for these services are included in the Contract price. The Contractor shall maintain complete inspection records and make them available to the Government.
B. The Contractor shall employ and pay for a qualified independent testing agency (Agency) to perform the quality control services.
C. Retesting: The Contractor is responsible for retesting, including repeated inspections and other services, where results of the initial tests, inspections or other quality control services indicate noncompliance with the requirements of the Contract.
D. Associated Services: The Contractor shall cooperate with others performing required tests, inspections and other quality control services, shall provide access to the work, and shall furnish incidental labor and facilities necessary to facilitate inspections and tests.
E. Take adequate quantities of representative samples of materials that require testing or assist the Agency in taking samples.
1. Deliver samples to testing laboratories.
2. Provide security and protection of samples and test equipment at the Project site.
F. The Agency shall provide qualified personnel to perform required inspections and tests.
G. The Agency shall notify the Government and the Contractor of irregularities or deficiencies observed in the Work during performance of their services.
H. The Agency is not authorized to change requirements of the Contract or approve or accept any portion of the Work. The Agency shall not perform any duties of the Contractor.

SECTION 014100 – CONSTRUCTION SURVEYING AND STAKING
PART 1 - GENERAL
1.1 SUMMARY
A. This work consists of performing surveying, staking, calculating, and recording data for the control of work. See FAR Clause 52.236-17 Layout of Work.
B. The Contractor shall provide technically qualified survey personnel experienced in construction survey and staking. The survey work must be performed under the direct supervision of a Professional Land Surveyor licensed to practice in the State of Minnesota. Survey personnel shall be capable of performing the work in a timely and accurate manner.
C. The Government reserves the right to conduct independent survey work for quality assurance purposes.
D. Control Point (CP) data. A minimum of two control points and one benchmark point (X,Y and Z coordinates) at each of the six project areas, will be made available by the Government to the Contractor prior to the start of construction. Reformatting and additional calculations may be required by the Contractor for the convenient use of the Government-furnished data. Provide immediate notification of apparent errors in the initial staking or in the Government-furnished data.
1.2 SUBMITTALS
A. Contractor shall provide a price breakdown by individual tasks for construction surveying and staking for use in making progress payments and price adjustments.
B. Include staking activities in the construction schedule. Include the dates and sequence of each staking activity.

3.1 GENERAL REQUIREMENTS
A. The Contractor shall provide a survey crew supervisor on the project whenever surveying and staking is in progress.
B. The Contractor shall furnish survey instruments and supporting equipment capable of achieving the specified tolerances.
C. The Contractor shall furnish acceptable tools and supplies of the type and quality suitable for project survey work. Furnish stakes and hubs of sufficient length to provide a solid set in the ground with sufficient surface area above ground for necessary legible and durable markings.
D. Record survey and measurement field data in an approved format. Submit as-staked data and corrections made to the Government-furnished survey data. Submit survey and measurement data at least weekly. Field data and supporting documentation become the property of the Government upon completion of the work.
