About this file

This is a Certification Regarding Debarment, Suspension, and Other Responsibility Matters form issued by the Incorporated County of Los Alamos Procurement Division in New Mexico for the solicitation titled "Redaction Services for Video and Audio Recordings." The form is a mandatory certification document that prospective offerors and bidders must complete and submit as part of their proposal or bid package. The certification requires that offerors confirm, to the best of their knowledge and belief, that neither they nor any of their principals are currently debarred, suspended, proposed for debarment, or otherwise ineligible to conduct business with the County. The form applies to primary covered transactions and serves as a pre-award eligibility verification mechanism.

The certification statement covers a three-year lookback period and requires offerors to attest that they have not been convicted of fraud, criminal offenses related to public contracts, antitrust violations, embezzlement, theft, forgery, bribery, or other offenses indicating lack of business integrity. Offerors must also confirm they have not violated contract provisions, failed to perform satisfactorily on prior contracts, violated ethical standards under the County Procurement Code, been indicted for relevant offenses, or had prior public contracts terminated for cause or default. Additionally, offerors must certify they are not current or former County employees and are not immediate family members of County employees or public officials. Any offeror unable to certify these statements must provide written explanation. The form specifies that falsification of any statement constitutes grounds for rejection or contract rescission, though certification issues do not necessarily preclude consideration for award.
