About this file
This is a Lobbying Certification Form required for federal contracts, grants, loans, and cooperative agreements issued by the Commonwealth of Pennsylvania, Department of Labor and Industry, Office of Vocational Rehabilitation (OVR), in connection with a supplement solicitation for Enhanced Driving and Ancillary Services in Johnstown, Cambria County. The certification applies to the procurement of Non-Emergency Transport Services for OVR employees and passengers with disabilities who are medically unable to safely operate motor vehicles. Services are required exclusively within Cambria County, with transportation generally originating from the assigned OVR District office to locations where Commonwealth business is performed. The contract encompasses both ambulatory transports for personnel with disabilities who can walk and non-ambulatory transports for those requiring wheelchair or motorized scooter accommodation.
The form serves as a material representation of fact and certification that no federal appropriated funds have been or will be paid to influence federal, congressional, or agency officials in connection with the contract award. Bidders must disclose any non-federal funds used for lobbying activities by submitting Standard Form-LLL. The certification must be included in all sub-award documents at all tiers, including subcontracts and sub-grants. This procurement is reserved exclusively for Pennsylvania Department of General Services (DGS) certified Small Businesses, requiring bidders to attach their Small Business Certificate for eligibility consideration. Failure to file the required certification subjects violators to civil penalties ranging from $10,000 to $100,000.
