About this file
This is an Iran Free Procurement Certification Form required by the Commonwealth of Pennsylvania under Sections 3501-3506 of the Procurement Code for vendors seeking contracts worth $1,000,000 or more with Commonwealth entities. The form applies to the supplemental solicitation for Bagged De-Icing Materials (50 lb. bags), which serves as a supplement to existing IFBs 6100060797, 6100061430, and 6100066274. These materials will be distributed to various state agencies for use as de-icing agents during winter weather events. Vendors must complete the certification form as a condition of contract eligibility and award.
The form provides two compliance options: Option #1 requires vendors to certify they are not on the current list of persons engaged in investment activities in Iran maintained by the Pennsylvania Department of General Services, or Option #2 permits vendors to submit a written exemption previously obtained from DGS under Section 3503(e) on a case-by-case basis. Vendors must identify their organization name, provide authorized signature, and include printed name, title, and date. Pennsylvania law establishes significant penalties for false certifications, including civil penalties of the greater of $250,000 or twice the contract amount, contract termination, and three-year ineligibility to bid on future Commonwealth contracts.
