About this file
This document is a Work Order Request form from the State of North Dakota (SFN 54383 version 2-2020), designed to facilitate contract work between a state agency and a vendor. The form provides a standardized template for agencies to request specific work under an existing state contract, with space for detailing project requirements, staffing needs, and evaluation criteria.
The work order allows for two agreement types: Time and Materials or Fixed Bid Project, with a structured approach to vendor selection based on a 100-point evaluation system. Key selection criteria include vendor project proposal solution (30 points), qualification and availability of vendor staff (40 points), and cost (30 points). The document requires vendors to submit detailed proposals including staff resumes, reference checks, and potential background checks. The form also establishes parameters for project scope, staffing requirements, project dates, deliverables, and specifies that the total contractual amount cannot be exceeded without a written amendment signed by both parties.
