About this file

This document is a Proposal Cover Sheet for a Mini-RFP issued by the Oregon Department of Transportation (ODOT) for the Long-Range Mobility Plan (TGM File Code: 2D-23). The solicitation, identified by OregonBuys Bid Solicitation Number S-73000-00012644, is seeking consulting services for developing a comprehensive long-range mobility strategy, with prospective consultants required to complete and submit this cover sheet as part of their proposal package.

The cover sheet includes critical certification requirements for proposers, such as affirming compliance with state non-discrimination policies, confirming no undisclosed debt to the state, accepting all Mini-RFP terms, and completing mandatory Conflict of Interest (COI) disclosures. Proposers must also certify they have a written policy preventing sexual harassment and discrimination, particularly for contracts anticipated to exceed $150,000. The document provides a formal mechanism for an authorized representative to bind their firm to the proposal submissions and representations, with provisions for detailing subconsultants, meeting minimum requirements, and attesting to the accuracy of all submitted information.
