The original bid receipt date of Monday, November 10, 2014 at 10:00 AM was moved to Thursday, November 13, 2014 at 2:00 PM, as indicated in Addendum No. 1. Sealed hard-copy proposals (two copies required, no electronic submittals) must be submitted to City Hall, 16 Colomba Road, DeBary, Florida 32713. Addendum No. 2 consists of seven pages and includes revised specification sections, plan sheets across architectural, structural, MEP/fire protection, civil, and landscape disciplines, responses to 33 bidder questions, and multiple architectural, structural, HVAC, civil, and landscape plan modifications. Approved substitutions are provided for Stanley hardware, Koroseal wall guards, and Windor 9020 windows.

Each proposal requires a bid bond equal to five percent of the maximum bid price, with a performance bond of 100% of the contract price required upon award. The City will pay all building permit fees and impact fees. The owner will provide the generator, with the contractor responsible for installation and all necessary electrical work. Key contractor responsibilities include permitting and removal of the existing drain field and septic system, conducting the fire flow test in coordination with the Utility Department, and removal of the existing light pole in coordination with Duke Energy. Federal Davis Bacon prevailing wage requirements do not apply. Subcontractors are not required to meet the same insurance requirements as the general contractor. Electronic CAD drawings are available on the City website and will be provided to the winning bidder. The City of DeBary reserves the right to reject any or all proposals and to waive any formality or technicality.

TO: PROSPECTIVE BIDDERS AND OTHER CONCERNED PARTIES

This Addendum No. 2 to the Plans, Specifications and Contract Documents for City of DeBary, “Public Safety Complex” (Bid 06-14) is hereby declared a part of the original Contract Documents dated October 7, 2014 along with Addendum #1 dated November 3, 2014, and in the case of conflict, the following Addendum No. 2 shall govern. Bidders shall state in their proposal that this Addendum No. 2 has been taken into consideration in their bids.

This Addendum consists of a total of 7 pages, plus the following attachments items:

RESPONSE TO QUESTIONS:

1. QUESTION: Would permitting include removing existing Drain field & Septic Tank(s)

RESPONSE: Contractor is responsible for permitting and removal of existing Drain field and Septic System.

2. QUESTION: Would Owner Provide the Generator /ATS and Contractor install the same?

RESPONSE: Owner will provide the generator. Contractor is responsible to install generator and all necessary wiring, switches, grounding, etc. to provide power to building.

3. QUESTION: Will the Fire Flow Test be provided by the City?

RESPONSE: Contractor is responsible for fire flow test and should coordinate with the Utility Department as needed.

4. QUESTION: a) Are Roof Trusses Pre-engineered Wood Trusses?

RESPONSE: Yes

5. QUESTION: Are the roof trusses free span from exterior wall to exterior wall?

RESPONSE: Yes; with the exception of the Mechanical Platform which will have interior bearing walls.

6. QUESTION: Are there any heavy timbers in this project.

RESPONSE: There is no heavy timber in this project. Neither is there any fire-rated wood.

7. QUESTION: Would the plumbing in the Detention Cells require a fully accessible chase?

RESPONSE: No. The combi toilet/lav fixture type has been updated per Sheet P001 (delta #2) to a unit that has a side access panel for maintenance.

8. QUESTION: Are there any Direct Purchase items by the City?

RESPONSE: At this time, the only item for direct purchase will be the generator.

9. QUESTION: Are electronic Drawings available to the bidders?

RESPONSE: Yes, They are available on the City’s website. CAD drawings will be provided to the winning bidder.

10. QUESTION: Is Federal/ Davis Bacon required?

RESPONSE: No.

11. QUESTION: Do Sub-contractors require the Same Insurance as GC?

RESPONSE: No, sub-contractors are not required to meet these requirements. The GC is required to meet the insurance requirements.

12. QUESTION: When will the Site Visit be conducted?

RESPONSE: Site visit was held immediately following the Pre-Bid Meeting.

13. QUESTION: Is Building Permit fee paid by the City?

RESPONSE: Yes.

14. QUESTION: Are Impact Fees paid by the City?

RESPONSE: Yes.

15. QUESTION: Will the Bid Date be moved to a later date?

RESPONSE: Bid was moved per Addendum #1 to Thursday, November 13th, 2014 at 2:00pm.

16. QUESTION: Are 2 copies of Bid Submittal required.

RESPONSE: Yes 2 hard copies, no electronic submittals needed. Refer to Specification Section 00200.

17. QUESTION: Shall existing Driveway be reused?

RESPONSE: No. Please see Civil Plans.

18. QUESTION: Shall Existing Light Pole be removed?

RESPONSE: Yes, Existing Light Pole shall be removed by General Contractor, please coordinate with Duke Energy.

19. QUESTION: What is the Bid Bond amount?

RESPONSE: Bid Bond Amount is 5% of Maximum Bid Price.

20. QUESTION: What is the amount for Performance Bond?

RESPONSE: 100% of the Contract Price

21. QUESTION: When is the Bid Date?

RESPONSE: Bid Date is indicated in Addendum #1.

22. QUESTION: Will the list of attendees be available to all bidders?

RESPONSE: Refer to List of Attendees at Pre-Bid Meeting in Addendum #1.

22. QUESTION: Sheet E001 Note#14 reflects that each wall or floor outlet shall have (1) ¾” conduit stubbed to above ceiling unless otherwise shown on the drawings. Given that MC Cable is acceptable per the specifications for use on branch circuit wiring, this addition of conduit stub ups would result in quite a costly installation to be performed on each wall device. Please verify this is actually desired to be incorporated into the base proposal or whether to provide as an alternate add.

RESPONSE: See revised general note #14/ sheet E001.

23. QUESTION: On Sheet E001 of the plans, Light Fixtures Schedule for fixture type C2 under notes states "PROVIDE EMERGENCY INVERTER” there are no specifications for this and I cannot locate on any drawings. Please advise.

RESPONSE: See Light Fixture Schedule sheet E001. The (3) fixture manufacturer catalog numbers indicate different characteristics for the fixture including the requirement for emergency battery power for fixture type C2.

24. QUESTION: On Sheet E101 of the plans, Rooms # 105 & 114 - There are floor boxes indicated, are they 2-gang or 3-gang ? What is the size of the systems conduit that is to be stubbed up from each box? The power circuits are not identified please advise.

RESPONSE: SEE FLOOR BOX REQUIREMENT IN PLAN NOTE 3 OF SHEET

E101. Floor Box RFB4-CI-1 This box comes equipped with 2 brackets for 2 duplex power receptacles. Communications Bracket- (order 1 for each box) CILT-2-RT Blank- (order 2 for each box) CIH/LT-B Aluminum Cover w/flange for installation for VCT- (order 1 for each box) S36BBTCAL. VERIFY EXACT

REQUIREMENTS WITH VOLUSIA COUNTY. SEE ALSO GENERAL NOTE 2

OF SHEET E101. FLOOR BOXES IN ROOM 114 ARE CIRCUIT RP-1:39.

FLOOR BOXES IN ROOM 105 ARE CIRCUIT RP-1:15.

25. QUESTION: On Sheet E101 of the plans, security / card access / intercom system have no specifications nor is it clear if it is a box and conduit stub-up or a complete raceway to somewhere. Please advise.

RESPONSE: See revised symbol legend regarding systems devices.

VERIFY EXACT REQUIREMENTS WITH VOLUSIA COUNTY. SEE ALSO GENERAL

NOTE 2 OF SHEET E101.

26. QUESTION: On Sheet E101 of the plans, Telephone / Data -There are no specifications and is this a box and 3/4' conduit system stubbed-up into ceiling cavity.

RESPONSE: See revised symbol legend regarding systems devices. SEE

ALSO GENERAL NOTE 2 OF SHEET E101.

27. QUESTION: On Sheet E101 of the plans, TV System - There are no cavity.

RESPONSE: See revised symbol legend regarding systems devices. SEE

ALSO GENERAL NOTE 2 OF SHEET E101.

28. QUESTION: On Sheet E101 of the plans, Fire Alarm System - There are no cavity.

RESPONSE: Fire alarm system, drawings, calculations, cut sheets shall be designed and installed by the installing contractor.

29. QUESTION: On Sheet E101 of the plans, Who is responsible for the installation and support of the "Ceiling mount projector".

RESPONSE: The projector, support and installation of ceiling projector is by others.

30. QUESTION: On Sheet E201 of the plans, In rooms # 111, 112 ,113 , 123 ,124 ,& 121 have switches but there are NO LIGHTS indicated in these rooms. Please advise.

RESPONSE: These switches were originally for the closet lights which were removed from the project. See revised drawings.

31. QUESTION: On Sheet E201 of the plans, There are no electrical control or exhaust fans indicated are any required and where?

RESPONSE: Exhaust fans are indicated on sheet E101 in rooms 115, Jan.

Closet, 126 and 106. Exhaust fans are addressed in the Motorized Equipment Schedule on sheet E401.

32. QUESTION: On Sheet E201 of the plans, there appears to be some three (3) way switches missing in rooms # 101 & 105. Please advise.

RESPONSE: 3-way switching has been eliminated in room 101. Control shall be accomplished via occupancy sensor and override switch. 3-way switching will occur in room 105 as indicated.

33. QUESTION: Water Inlet Size incorrect for Water Heater Specified RESPONSE: The hot and cold lines for the single phase water heater should be

¾”. Refer to P001; Delta #3

ARCHITECTURAL PLAN AND SPECIFICATION MODIFICATIONS:

1. Geotech Report is attached to this Addendum.

2. The upper floor shall be considered a “Mechanical Platform.”

Refer to Sheet A101 and other sheets in set.

3. Shower controls and nozzle shall be installed on the wall opposite the shower seat per FBC – ADA-2012. Refer to Sheet A602, note 16. The shower controls shall be within 15” between the shower nozzle and the shower curtain.

5. Slope at employee entrance and vestibule entrance shall be modified to 1”.

Refer to Sheet A100 and S101.

6. Clarification on Window Specifications:

Refer to attached Specification Section 08570. Laminated glazing in windows shall not be any less than 7/16” thick.

7. Roof sheathing shall be 5/8” thick CD-X plywood and not 3/4”.

Refer to revised sheet A301.

8. Split face block shall be primed and painted with waterproof paint. Refer to revised Specification Section 09900; Part 2.03 Paint Systems Exterior; E.

Refer to revised sheets A200, A201 and A400.

9. Modification to fiberglass column details #1 & #2 Add 5/8” stucco on CMU wall before installing fiberglass column. Refer to revised sheet A701.

10. Provide .024 aluminum drip edge above roof gutter.

11. Refer to Sheet A601: Holding Area Keynotes 7 & 8 are not used.

12. Refer to Sheet A301 for update footing detail at interview rooms. Refer to structural drawings for monolithic footings at interior masonry wall.

13. Refer to Sheet A401, Wall Section E for turndown edge at aluminum storefront entrance.

STRUCTURAL PLAN MODIFICATIONS:

1. Refer to S001 for updated live load and fill material

2. Refer to S101 and S102 for corrected concrete floor thickness. Changed to 4” with the vapor barrier thickness of 10 mils. Reinforcement updated and slab slope at entrances. Updated footing at interview rooms. The stem wall foundation for interior CMU partitions walls shall be substituted by a thickened slab 2’-0” wide, 12” deep with (3) #5 cont. rebars and #5 @ 12” o.c. rebars x 1’-6”.

3. Clarification of beam locations:

CB1 is shown on Sheet S201.

MB1 is located on Sheet S201

3. Refer to Sheets S202 for spacing requirement for flat straps & blocking

HVAC PLAN AND SPECIFICATION CLARIFICATIONS

1. AHU #5 label was added in IT room on revised Sheet M101

2. Refer to revised Sheet P001 for added water heater.

3. Refer to Sheet E401 for clarifications to electrical service and panel name.

Electrical Service is 208Y120V-3 Phase/ 4 wire. Panel name shall be RP-1

4. Clarification on Site Lighting and Surge arrestor(s).

Parking lot lighting design and photometrics will be provided by DUKE Energy.

Surge arrestor(s): SPD’s appear in specification section 16289, updated on E401

5. Clarification of fire alarms, pull stations, strobes & FACP Locations Refer to revised sheets F100 and F101 for location of above items.

APPROVED SUBSTITUTION ITEMS :

The following products have been approved for substitution:

1. “Stanley” as applied to Hardware: Specification Section 08710 (www.stanleysecuritysolutions.com)

2. “Koroseal” Wall Guard: as applied to Wall guards: Specification Section 10260 (www.koroseal.com)

CIVIL PLAN MODIFICATIONS

FDOT: A separate set of plans were prepared to address the comments received from FDOT on the Driveway Connection Application. The following plans include revisions as noted:

1. Domes were deleted on the sidewalk access adjacent to the driveway access.

2. FDOT notes 1-10 were added on Sheet C-8. Driveway concrete noted as 3000 psi.

Shrubs Groundcover; Will you please provide quantities for the shrubs groundcover.

Quantities provided on attached L-1 drawing.

2. QUESTION: Reference drawing L-1 Landscape Plan, see beneath the building and future expansion area, there are down arrows that are not shown on the schedule, what are those down arrows?

3. QUESTION: Could you please provide an estimate of sod required for the project.

Refer to attached L-1 drawing for sod quantity.

4. QUESTION: Can pond bottoms be seeded?

No, this is a wet pond.
