About this program

The Department of Health and Human Services Centers for Disease Control and Prevention funds technical assistance, monitoring, and evaluation activities that strengthen harm reduction and syringe services programs to prevent infectious disease among people who inject drugs, under CFDA 93.488, with the most recent grant opportunity posted July 24, 2026.

Recent grant activity demonstrates sustained but variable funding momentum. Seven cooperative agreements totaling $45.9 million in federal dollars obligated have been awarded since 2019, with a spike to six awards in 2022 following a single award in 2019. The most recent grant award was made September 30, 2022, more than four years ago; no awards appear to have been disbursed since then, though the portfolio block does not show activity through 2026. All awards take the form of cooperative agreements with periods of performance of one to five years, subject to satisfactory progress and availability of funds. Recipients may continue funding non-competitively after initial awards if progress is documented and federal interest is established.

Awarded grants support concrete operational and surveillance work: strengthening syringe services program capacity and performance nationally, implementing biennial biobehavioral surveys of people who use drugs in high-impact settings, documenting service delivery capacity and gaps, and conducting monitoring and evaluation of infectious disease testing, treatment, and prevention services. Recent representative awards include $8.8 million to strengthen syringe services programs for people who use drugs (September 2022), and $9.1 million and $6.5 million cooperative agreements for National HIV Behavioral Surveillance in undisclosed jurisdictions (both January 2022). Recipients must perform substantive project roles and may not use funds for furniture or equipment unless identified in the budget and approved.

A current funding opportunity, CDC-RFA-PS-27-0239 "Strengthening Services to Prevent the Infectious Disease Consequences of Drug Use," was posted July 24, 2026, and accepts applications from for-profit organizations, nonprofits, state and local governments, tribal entities, and higher education institutions. The opportunity has two components: biennial surveys to assess capacity and document gaps at high-impact settings serving people who use drugs, and support for infectious disease testing, treatment, and prevention services. Applicants may apply for either or both components. No application due date is specified in the source.

Objective and description

Objective

The purpose of this program is to strengthen harm reduction programs in order to prevent infectious disease resulting from injection drug use, and improving health outcomes for people who inject drugs; including strengthening linkage to medication-as

Description

Funds may be used for costs associated with strengthening the capacity and improving performance of harm reduction programs throughout the United States; particularly costs associated with conducting media analysis and developing media content to address implementation barriers, increasing the understanding of benefits of these programs and the implementation of monitoring and evaluation of syringe services programs. Recipients may not generally use funding for the purchase of furniture or equipment. Any such proposed spending must be identified in the budget. The recipient must perform a substantial role in carrying out the project objectives and not merely serve as a conduit for an award in another party or provider who is eligible.

Eligibility

Uses of assistance

The purpose of this program is to increase the capacity of government and non-government organizations to strengthen harm reduction programs, including Syringe Services Programs (SSPs). Activities by the cooperative agreement program include technical assistance, communication, and monitoring and evaluation of harm reduction programs. These activities will facilitate and sustain efforts to effectively and efficiently address the infectious disease consequences of opioid use and injection drug use.

Length and time phasing of assistance

Financial assistance is provided for a 12-month budget period with a period of performance of up to five years subject to the availability of funds and satisfactory progress of the recipient.

How to apply

Criteria for selecting proposals

Applications will be evaluated on the review criteria described in the Notice of Funding Opportunity Announcements (NOFOs). In general, the review and selection process of complete and responsive applications to the NOFO consists of determination of the scientific and technical merit by objective or peer review, availability of funds, and relevance of program priorities and the priorities of CDC.

Award procedure

After review and approval, a Notice of Award (NoA) will be prepared and processed, along with appropriate notification to the public. Initial awards provide funds for the first budget period (usually 12 months) and the NoA will indicate support recommended for the remainder of the project period, allocation of Federal funds by budget categories, award requirements, and special conditions, if any.

Renewals

Cooperative agreement period of performance is 1 to 5 years. After initial awards, and subject to availability of funds, projects may be continued non-competitively contingent upon satisfactory progress by the recipient (as documented in required reports) and the determination that continued funding is in the best interest of the Federal government.
