Bids must be submitted via email to adrian.stallone@la.gov with the RFx number in the subject line and include complete unit pricing on F.O.B. destination basis with all applicable taxes and prepaid transportation. The contract may be extended for two additional 12-month periods at the same prices, terms, and conditions, with a maximum total term of 36 months. Payment will be made within 30 days of properly executed invoice or delivery, whichever is later. Vendor registration in LaPac is required prior to bid submission and award.
