A virtual bidders' conference is scheduled for July 29, 2026 at 10:00 a.m. EST.
Eligible proposers are Connecticut-registered community-based organizations, HIV service organizations, Federally Qualified Health Centers, community health centers, local health departments, and other eligible healthcare providers that are in good standing with DPH, have sound fiscal health with clean audited financial statements for the last three years, and are registered with the System of Award Management by contract execution. Expanded Service funding of $500,000 annually per Hub is available only to entities not previously funded by RWPB within the last five years.
Services must be delivered minimum four days per week between 8:00 a.m. and 5:00 p.m. with on-call emergency availability. Medical Case Managers must hold a bachelor's degree in a healthcare-related field. Proposals must follow strict formatting: 10-point Arial or Calibri font, 0.7-inch minimum margins, 30-page maximum (excluding cover sheet, table of contents, appendices, financial statements, and budgets).
