About this file

This Addendum 1 to Request for Proposal Log #2027-0905 serves as a supplement to the Connecticut Department of Public Health's Ryan White Part B (RWPB) Program RFP for "Connecticut Ryan White Part B Core and Support Services 2027-2030." The addendum clarifies and corrects information in the original RFP regarding the scope of services, funding distribution, and budget expectations. Proposers must specify whether they intend to serve as the Eastern RPA Hub (Capitol, Lower CT River, Northeastern, and Southeastern COG Regions), Western RPA Hub (Greater Bridgeport-Metro, Naugatuck Valley, Northwest Hills, South Central, and Western COG Regions), or as the Statewide TLC Coordinator. The Eastern and Western RPA Hubs must provide all eight Core services and six of the seven Support services (excluding Non-Medical Case Management Services), while the Statewide TLC Coordinator must provide short-term transitional case management services to HIV-positive inmates upon release from incarceration. Proposers must detail how services will be delivered directly or through qualified subcontractors and justify their service proposals based on surveillance data, priority population needs, community mapping, needs assessments, and prior experience.

The total RFP funding is $19,500,000.00 over three fiscal years ($6,500,000.00 per fiscal year for 2027-2030). The Eastern Hub receives $3,269,229.00 annually (Core Services: $2,076,922.00; Support Services: $692,307.00; Expanded Services: $500,000.00), the Western Hub receives $2,930,771.00 annually (Core Services: $1,823,078.00; Support Services: $607,693.00; Expanded Services: $500,000.00), and the Statewide TLC Coordinator receives $300,000.00 annually (Core Services: $75,000.00; Support Services: $225,000.00). Eastern and Western Hub proposers must allocate no less than seventy-five percent (75%) of the award to Core services and no more than twenty-five percent (25%) to Support services, while the Statewide TLC Coordinator is exempt from the 75% Core service requirement and may allocate up to 100% of service dollars to Support services. Administrative and general expenses and Clinical Quality Management charges may not exceed fifteen percent (15%) of combined Core and Support Service allocations. Expanded Service funding is available only for service sites not previously funded by RWPB within the last five years and is not available for the Statewide TLC Coordinator. Proposers must submit detailed line-item budgets with justifications, and RWPB functions as the payer of last resort.

A reason for change will be included aŌer each secƟon to help summarize the changes for prospecƟve proposers.

“2.1. Defined Scope of Services”, Page 15: This section is deleted in its entirety, and the following is substituted in lieu thereof:

2.1 Defined Scope of Services

The RWPB program provides funding for two Regional Planning Areas and one statewide service under this RFP. This includes:

Eastern RPA Hub: Capitol, Lower CT River, Northeastern, Southeastern COG Regions.

Western RPA Hub: Greater Bridgeport-Metro, Naugatuck Valley, Northwest Hills, South

Central, Western COG Regions.

Statewide: TLC Coordination services statewide.

Proposers must specify whether they intend to serve as the Eastern or Western RPA Hub to ensure the availability of all eight (8) Core services and six (6) of the seven

(7) Support services (all but Non-Medical Case Management Services) for People with HIV or whether they will serve as the Statewide TLC Coordinator.

The Proposer is responsible for ensuring that all required services are delivered— either directly or through qualified subcontractors—within the selected RPA (Eastern, Western or Statewide, as applicable). The Proposer must also indicate how it will provide services to the areas within the RPA if it does not have a physical presence there by providing proof of service contract agreements with potential subcontractors.

The justification for providing the proposed services within the RPA should be based on surveillance data, needs of the Priority Populations, community mapping, needs assessments, and prior experience serving the population(s). Detailed surveillance data can be found at:

Detailed surveillance data can be found at: https://portal.ct.gov/dph/aids--chronic-diseases/surveillance/hiv-surveillance-program/ct-hiv-statistics?language=en_US

Funding Distribution by RPA for 2027-2030 | Each FY is $6,500,000.00; Total RFP is $19,500,000.00

Eastern HUB Western HUB Statewide TLC Coordinator

Core Services $2,076,922.00 $1,823,078.00 $75,000.00

Support Services $692,307.00 $607,693.00 $225,000.00

Expanded Services* $500,000.00 $500,000.00 $0.00

Total $3,269,229.00 $2,930,771.00 $300,000.00

No specific amount of funds will be earmarked to fund Core and Support services prior to proposal review. HRSA RWPB funding distribution requires allocation of no less than seventy-five percent (75%) of an award to Core services and no more than twenty-five percent (25%) of the award for Support Services. No more than ten percent (10%) of the Core and Support allocations may be charged towards administrative expenses and general costs. Proposers may charge five percent (5%) of the Core and Support allocations towards Clinical Quality Management (CQM) costs, excluding the ten percent (10%) cap on administrative expenses.

Administrative and general expenses and CQM charges may not exceed fifteen percent (15%) of the combined Core and Support Service allocations. The allocations listed in the chart above are preliminary and will be finalized based on the availability of funds and resources within each RPA.

RWPB RFP 2027-0905 Addendum #1

Note: Statewide Services Non-Medical Case Management/ TLC: A proposal for the Statewide TLC Coordinator must include short-term transitional case management services to HIV-positive inmates upon release from incarceration. The Statewide TLC Coordinator is exempt from the 75% Core Medical Service requirement, as its primary purpose is to provide non-medical case management, which is a support service.

*Expanded Service: The funding for this line item is only available if the proposal contains service sites that have not previously been funded by RWPB in the last five (5) years. A list of service providers and their locations funded by DPH in the last five (5) years is available in Appendix G. If the Proposer chooses to apply for this additional funding, the proposal must detail who the service provider will be (either the Proposer or a subcontractor), their experience in working with PWH, what services they will provide, how the Proposer will train the individual(s) responsible for the services, and how the Proposer will monitor the individual(s) providing the services. Funding in this line item that is allocated to service sites that have received RWPB funding in the last five (5) years will be considered non-responsive to this section. Failure to adhere to the requirements outlined in this section will disqualify the proposal from funding consideration for this line item only. The Expanded Service option is not available for the Statewide TLC Coordinator Proposer.

The Core and Support Services description is directly below each of the listed Core and Support Services in Tables 2.1 and 2.2, respectively. Proposers for the Eastern or Western Regional HUBs must describe in detail how they will implement the eight (8) Core and six (6) Support Services in their chosen RPA. Proposers for the Statewide TLC Coordinator must describe the Core and Support Services they propose to provide. The descriptions must include, for each service, the demographics of the Priority Population, the projected number of units of service (each time a client receives a service) to be delivered, and outcome measures. If the Proposer is not directly providing all services in the specified RPA, they must list at least one subcontractor per service and the designated area within the RPA to be served.

Reason for change: The funding for Core and Support services for the Statewide TLC Coordinator was incorrect. The TLC Coordinator is exempt from the 75% Core Medical Service requirement.

Emergency Financial Assistance can be used for housing, associated utilities to maintain housing, and internet services.

“6. Budget Expectations”, page 22: This section is deleted in its entirety, and the following is substituted in lieu thereof:

6. Budget Expectations Proposers must submit a detailed line-item budget with justifications for each line item. HRSA RWPB funding distribution requires allocation of no less than seventy-five percent (75%) of an award to Core services and no more than twenty-five percent (25%) of the award for Support Services for the Eastern and Western Hub proposers. The Statewide TLC Coordinator is exempt from the 75% Core services requirement and may allocate up to 100% of the service dollars to Support services. No more than ten percent (10%) of the Core and Support allocations may be charged towards administrative expenses and general costs.

Proposers may charge five percent (5%) of the Core and Support allocations towards CQM costs, excluding the ten percent (10%) cap on administrative expenses. Administrative and general expenses and CQM charges may not exceed fifteen percent (15%) of the combined Core and Support Service allocations. The Proposer must describe budget expectations in detail, which could include, but are not limited to, cost standards, program funding sources, total available funding for the period of award, proration, third party reimbursement, flat fees, fee-for-service revenues, and subcontractor cost schedules. RWPB is the payer of last resort, and the response to this proposal must demonstrate that the Proposer’s services will be provided in accordance with that requirement.

Reason for change: The Statewide TLC Coordinator primarily provides non-medical case management, which is a support service and is therefore exempt from the 75% Core service requirement.
