Pennsylvania law establishes significant penalties for submitting false certifications, including contract termination and three-year ineligibility to bid on future state contracts under 62 Pa.C.S. § 531. Additionally, all certifications are subject to provisions and penalties under 18 Pa.C.S. § 4904 regarding unsworn falsification to authorities. The form, published February 4, 2022, serves as a prerequisite compliance document that bidders must complete and submit as part of their proposal package for this graffiti removal services procurement.
