About this file

This document is a Workers Compensation and Employers Liability Insurance Policy Endorsement issued by Federal Insurance Company for PG Environmental, LLC, effective January 1, 2021. The endorsement specifically involves the Arizona Department of Environmental Quality, located in Phoenix, Arizona, and includes a special waiver of subrogation provision. The policy includes a notable caveat for Missouri-based work, indicating specific conditions for waiving subrogation rights in that state.

The endorsement focuses on a waiver of the insurance company's right to recover payments from liable parties for work-related injuries, but only to the extent that work is performed under a written contract requiring such an agreement. The policy applies specifically to PG Environmental, LLC's operations, with the premium amount marked as "Incl." (included), suggesting the cost is integrated into the base policy. The document does not detail specific goods or services, but rather provides a legal mechanism for managing potential workers' compensation recovery rights in the context of environmental quality work.
