Section 00 00 00 - Procurement and Contracting Requirements

STATEMENT OF WORK

Statement of Work

Communications Cables – Supply & Installation

John H. Kerr Powerhouse, Boydton, VA 23917

Summary of Work:

The Contractor shall furnish all labor, equipment, materials, parts, and services necessary to provide the work shown on the attached drawing. This work includes supplying and installing copper and fiber optic cables on the John H. Kerr Project, via directional bored conduits and over-head services. The over-head cables will be installed on existing poles on the project. No additional poles are required.

The Contractor will participate in the Corps’ lock out tag out program but will provide its own lock out devices.

Contractor Qualifications:

Refer to the requirements for the Lowest Priced, Technically Acceptable offer on the last page of this Statement of Work.

Specifications:

1. Supply and install the 12 strand single mode fiber optic cable as shown on the drawing.

The cable shall be manufactured by AFL, Inc., type Mini-Span 424, model number

AE0129C520AA4. The exact location of the installation of the new fiber optic cable is as follows:

a. Install fiber optic cable from the Powerhouse phone room to the

Nature Center.

b. Install fiber optic cable from the Visitor Assistance Center to the Office in the maintenance area.

2. Supply and install the 50 pair, #20 AWG, copper shielded telephone cable as shown on the drawing. The cable shall be suited for direct burial and overhead use.

3. Bore and install schedule 40, 3” HDPE conduits as shown on the drawing.

4. Supply and install the fiber optic junction boxes as shown on the drawings. The boxes must be suitable for environment that they are being installed in and must be submitted for approval before purchased. The fiber must connect on ST type connectors.

5. Supply and install the copper telephone cable junction boxes as shown on the drawings. The boxes must be suitable for environment that they are being installed in and must be submitted for approval before purchased. They shall be pole mounted type boxes, mounted at the locations shown on the drawing. They shall be designed to terminate the entire incoming 50 pair, the outgoing 50 pair, and the tap cables as shown on the drawings with type 66 connections for the conductors.

All work must be completed by the Contractor within 120 of the Notice to Proceed. If necessary, the existing electric distribution can be de-energized for short periods of time, such as a few hours. However, these outages cannot occur every day and must be approved at least two days in advance. All work shall be done in accordance to Federal, State and distribution power regulations, including but not limited to: IEEE C2 and National Electric Code.

Submittal Requirements:

1. Accident Prevention Plan (refer to the Safety section below for details).

2. Fiber Optic Cable - 12 strand single mode OS2.

3. Copper Telephone Cable – 50 Pair, #20 AWG, Shielded.

4. Fiber Optic Junction Boxes

5. Copper Telephone Cable Junction Boxes

6. Schedule 40, HDPE 3” Conduit that will be installed in the bores.

7. Boring procedures and plan for areas shown on the drawing.

Safety Requirements:

The Contractor shall follow the most recent U.S. Army Corps of Engineer’s EM 385-1-1 Safety

Manual including changes, OSHA regulations, state DOT regulations, and any regulations and rules set by the Contractor. All employees must wear safety boots with toe protection, hard hat and high visibility vest when working in a partial closed or opened roadway and around moving equipment. An Accident Prevention Plan must be submitted before work can begin on site.

Correction of Unsatisfactory Work:

If the work is found to be unsatisfactory, the Contractor shall be liable for all costs in connection with correcting such errors where the errors are deemed by the Contractor or COR to be the result of careless or incompetent performance by the Contractor’s employees.

Schedule:

All work shall be completed no later than 120 days after the official Notice to Proceed is given.

Security:

Access and General Protection/Security Policy and Procedures:

1. All contractor and all associated sub-contractors employees shall comply with applicable installation, facility and area commander installation/facility access and local security policies and procedures (provided by government representative). The contractor shall also provide all information required for background checks to meet installation/facility access requirements to be accomplished by installation Provost Marshal Office, Director of Emergency Services or Security Office. Contractor workforce must comply with all personal identity verification requirements (FAR clause 52.204-9, Personal Identity

Verification of Contractor Personnel) as directed by DOD, HQDA and/or local policy. In addition to the changes otherwise authorized by the changes clause of this contract, should the Force Protection Condition (FPCON) at any installation or facility change, the

Government may require changes in contractor security matters or processes.

2. The contractor and all sub-contractors employees will be issued a CAC only if duties involve one of the following: (1) Both physical access to a DoD facility and access, via logon, to DoD networks on-site or remotely; (2) Remote access, via logon, to a DoD network using DoD-approved remote access procedures; or (3) Physical access to multiple DoD facilities or multiple non-DoD federally controlled facilities on behalf of the DoD on a recurring basis for a period of 6 months or more. At the discretion of the sponsoring activity, an interim CAC may be issued based on a favorable review of the

FBI fingerprint check and a successfully scheduled NACI at the Office of Personnel

Management.

3. All contractor employees with access to a government info system must be registered in the Army Training Certification Tracking System (ATCTS) at commencement of services, and must successfully complete the DOD Information Assurance Awareness prior to access to the information systems and then annually thereafter in accordance with personnel security standards listed in AR 25-2 (Information Assurance), an appropriate background investigation will be conducted prior to accessing the government information systems.

4. All contract employees, including subcontractor employees who are not in possession of the appropriate security clearance or access privileges, will be escorted in areas where they may be exposed to classified and/or sensitive materials and/or sensitive or restricted areas.

5. The Contractor must pre-screen Candidates using the E-verify Program

(http://www.uscis.gov/e-verify) website to meet the established employment eligibility requirements. The Vendor must ensure that the Candidate has two valid forms of

Government issued identification prior to enrollment to ensure the correct information is entered into the E-verify system. An initial list of verified/eligible Candidates must be provided to the COR no later than 3 business days after the initial contract award."

*When contracts are with individuals, the individuals will be required to complete a

Form I-9, Employment Eligibility Verification, with the designated Government representative. This Form will be provided to the Contracting Officer and shall become part of the official contract file.

PAST EXPERIENCE AND VIRGINIA CLASS A:

1. a) Offeror’s are required to submit prior experience of recent and relevant contracts/projects.

Contracts/Projects can be performed with Federal, State, local or private companies. Government contracts are preferred but not required.

b) Contracts/Projects that are relevant are defined as those that are similar to the requirements of the Statement of Work (SOW). Contracts/Projects that are recent are defined as those that have been completed within the last five (5) years.

c) A minimum of two (2) contracts/projects shall be provided to include: contract number, title of project, detailed description of work performed, and Point of Contact. Please do not submit more than five (5) contracts/projects.

d) In order for an Offeror to be determined technically acceptable, the offeror must show previous experience with the type of work described in the SOW. Offeror’s must show previous experience on a minimum of two (2) contracts/projects.

2. The Contractor must have a Virginia Class A contractor’s license. Submit a copy of license with your quote.

52.222-23 NOTICE OF REQUIREMENT FOR AFFIRMATIVE ACTION TO ENSURE EQUAL

EMPLOYMENT OPPORTUNITY FOR CONSTRUCTION (FEB 1999)

(a) The offeror's attention is called to the Equal Opportunity clause and the Affirmative Action Compliance

Requirements for Construction clause of this solicitation.

(b) The goals for minority and female participation, expressed in percentage terms for the Contractor's aggregate workforce in each trade on all construction work in the covered area, are as follows:

(c) The Contractor's compliance with Executive Order 11246, as amended, and the regulations in 41 CFR 60-4 shall be based on (1) its implementation of the Equal Opportunity clause, (2) specific affirmative action obligations required by the clause entitled "Affirmative Action Compliance Requirements for Construction,'' and (3) its efforts to meet the goals. The hours of minority and female employment and training must be substantially uniform throughout the length of the contract, and in each trade. The Contractor shall make a good faith effort to employ minorities and women evenly on each of its projects. The transfer of minority or female employees or trainees from

Contractor to Contractor, or from project to project, for the sole purpose of meeting the Contractor's goals shall be a violation of the contract, Executive Order 11246, as amended, and the regulations in 41 CFR 60-4. (d) The Contractor shall provide written notification to the Deputy Assistant Secretary for Federal Contract

52.228-13 ALTERNATIVE PAYMENT PROTECTIONS (JULY 2000)

(a) The Contractor shall submit one of the following payment protections:

Payment bond or An irrevocable letter of credit (ILC)

(b) The amount of the payment protection shall be 100 percent of the contract price.

(c) The submission of the payment protection is required within 10 days of contract award.

(d) The payment protection shall provide protection for the full contract performance period plus a one-year period.

(e) Except for escrow agreements and payment bonds, which provide their own protection procedures, the

Contracting Officer is authorized to access funds under the payment protection when it has been alleged in writing by a supplier of labor or material that a nonpayment has occurred, and to withhold such funds pending resolution by administrative or judicial proceedings or mutual agreement of the parties.

DAVIS-BACON ACT CERTIFIED LABOR PAYROLLS (APRIL 2011)

The contractor is encouraged to use a commercially-available electronic system to process and submit certified payrolls electronically to the

Government. The requirements for preparing, processing and providing certified labor payrolls are established by the Davis-Bacon Act as stated in

FAR 52.222-8, PAYROLLS AND BASIC RECORDS and FAR 52.222-13, COMPLIANCE WITH

DAVIS-BACON AND RELATED ACT REGULATIONS.

If the contractor elects to use an electronic Davis-Bacon payroll processing system, then the contractor shall be responsible for obtaining and providing for all access, licenses, and other services required to provide for receipt, processing, certifying, electronically transmitting to the
