Project Information

Gw2 Engineering, Inc. is pleased to present this HAZMAT limited sampling forAsbestos

& lead-based paint, at a two-story office building 193 and Hangar 518 located at Altus AFB

, Altus, Oklahoma. The survey included interior building materials, and is provided in support of future demolition or remodeling of the buildings.

Client and Authorization

This project was authorized by Mr. Chandresh Gajera, PE of Merrick & Company on

October 25, 2013 via email and acceptance of our proposal number M091-13 dated

September 24, 2013. The scope of services for this subsurface exploration were performed in accordance with our proposal.

Gw2 Engineering, Inc.

Project No. G035-13

August 30, 2013

Summary of Findings

The scope of work included the identification of suspect asbestos-containing materials

(ACM) and interior building components painted with lead-based paint (LBP). Gw2’s scope of work was limited to asbestos and lead. However, we did a visual inspection of the fluorescent tubes, potential mercury-containing materials. The inspection was conducted on October 28, 2013.

Visual Inspection

Initially, we walkthrough the building to determine the presence and existing conditions of the potential suspected materials that were exposed or accessible. Both floors 1st and 2nd had very similar appearance and we were able to group into homogeneous (color texture and appearance) sampling areas. During the walkthrough, we asked information on the history of the building on maintenance and installation of the light system. Outside pictures were not taken due to the restricted areas at Altus AFB. A few pictures were taken inside the building 193 (lights and ceiling covering).

Asbestos-Containing Materials

A total of five (5) samples of suspect ACM representing ceiling and walls of homogeneous materials were collected from the building. Materials sampled included wall paint and ceiling panels, acoustic ceiling panels, wall plaster, and drywall system.

Laboratory results indicated no detectable levels of asbestos in ceiling tile, acoustic panels and walls.

All samples were taken as randomly as possible, without creating any damaged to the sampled areas, since the building is still occupied. Three samples were collected for each test.

Gw2 Engineering, Inc.

Lead-Base Paint

A total of nine (3) samples were collected from the building 193 for lead analysis during the survey.

The survey for the existing Hangar 518 included the sampling of the exterior and interior lead-base paint on the structural steel components. Once we determined the random samples, paint-chips sampling was performed. The paint-chip was approximately 2"x1" and 2"x2" chips that represented all potential contaminated layers of paint. Three (3) of the paint chip samples had detectable levels of lead at the Hangar steel structures.

EPA guidelines were used to determine the sampling protocol. Sampling locations were chosen to be representative of homogeneous material. All samples were taken as randomly as possible, and minimizing disturbance of the materials.

EPA Inspection Protocol

a. Surfacing Materials (Spray or trowel to member of the building)

b. Thermal Insulation (Steel Structures)

c. Miscellaneous Material (Any other out of the above classification)

d. Friable Materials: Any material containing more than 1% Asbestos that when dry can be crumble, pulverized or crushed by hand pressure.

e. Category I Non-Friable: materials as packing, gaskets, resilian floor coverings, and asfalt roofing.

f. Category II Non-Friable: Any material that contains more than 1% asbestos and cannot be reduced to powder by hand.

Fluorescent Tubes Visual Inspection

Several fluorescent tubes were inspected for potential mercury-containing material. The inspection was limited to a visual inspection and verifying the brand name and the

Gw2 Engineering, Inc.

description of content from the manufacturer. The fluorescent tubes manufactory description indicates that there is no mercury containing material inside the light tubes.

Reliance and Limitations

This report was prepared expressly for the use of Merrick & Company for the building 193 and Hangar 518, ocated inside the AFB in Altus, Oklahoma. The information and recommendations contained in this report may not be adequate and should not be used by other parties or for other projects.

Information contained in this report is intended for design or information purposes. We recommend that contractors perform their own investigation to determine construction means and methods if there is unforseen conditions on suspected materials.

Please note that mold or any other environmental issues were not considered for this project and are beyond the scope of this report. Since we are not involved in all the phases of the construction process, Gw2 is not and cannot be responsible for mold or any other environmental issues.

This project, engineering evaluation, and report were conducted in accordance with the standard of care for visual inspection, laboratory test results and geotechnical engineering in the state of Oklahoma at the time of the investigation.
