DESIGN NARRATIVE

1.0 EXECUTIVE SUMMARY

1.1 PROJECT BACKGROUND

This project is titled “3rd Floor Addition for Surgery Center, PACU, SPD & ICU, Grand Junction, CO”.

HDR has been contracted by USACE - Sacramento District to provide design services with the ultimate objective of submitting a Request for Proposal (RFP) bid package for a Design/Build (DB) contract. The RFP document will be advertised by USACE – Sacramento District to all potential bidders on a “full and open” basis.

The new addition will be constructed on top of the existing 2-story surgery wing which opened for use in 1986. The VA reports that the initial structure was designed to accommodate not only this 3rd floor addition but a 4th level as well.

The approximately 30,537 SF 3rd floor addition will accommodate a Surgery Center, a Post Anesthesia Care Unit (PACU), a Supply Processing and Distribution unit (SPD), and an Intensive Care Unit (ICU). The project also includes floor plan development to provide a connection between the new addition and Building 1.

HDR’s scope of work includes the following:

• Prepare two alternative floor plans for the new 3rd floor Addition and two alternative (10%) floor plans for the renovated 3rd Floor “T” section of the Building 1 that adjoins the future Addition.

The “T” section renovation for in-patient rooms will be developed for a future VA project.

• Following selection of an acceptable alternative prepare and submit the DB-RFP document including an intermediate 35% submittal, a 100% submittal and a 100% Corrected Final/RFP document.

• Prepare and submit a Design Analysis (DA). The DA will consist of narratives from each discipline and include limited design calculations, only what is necessary to develop the10% level DB documents and an accurate design-construction cost estimate, which will be used to make value engineering analysis and decisions.

• Prepare and provide a project design-construction cost estimate (CE).

• Participate in DB-RFP review meetings and Customer interviews.

Concurrent to this project HDR is providing design services for a seismic corrections project that will include the 3rd floor Building 1. HDR will make sure that the potential building impacts and reconfigurations related to the seismic design are appropriately coordinated with the design of the surgery project. Like this project, the initial submittal for the seismic project encompasses development of alternative seismic solutions for discussion and cost estimating purposes. More precise information and drawings showing the coordinated efforts of the two projects will be provided after alternatives have been selected.

Project Numbers: 575-202; Correct Deficiencies in Surgery / PACU and 575-203 Correct Deficiencies in SPD / ICU, P2 Project #320069

1.3 DESIGN TASKS AND FINDINGS TO DATE

1.3.1 Task 1

Task 1 included a site investigation and survey of the building and interviews of the medical and building operators that will be involved in the new surgery addition. During this survey Michael Lambert, HDR’s Project Architect, interviewed VA staff and gathered information necessary to develop an architectural program and begin preparing design of the plans. Mr. Lambert and other HDR designers also surveyed the 3rd floor of Building 1 since the surgery addition will require in-patient rooms in Building 1 adjacent and connected to the new surgery addition.

1.3.2 Task 2

Task 2 included preparation of a concept level (approximately 10%) design submittal consisting of a minimum of two floor plan solutions. The submittal consisted of drawings, specification outline, design analysis report, and a preliminary construction cost estimate. The primary purpose of this submittal was to provide alternative solutions and costs in order to facilitate selection of one alternative that will be advanced to full design and construction. HDR presented this submittal to the Government during a review conference held at the Grand Junction, CO VA Medical Center on March 12, 2009.

1.3.3 Task 3

Task 3 included preparation of two Design-Build / Request for Proposal (RFP) submittals taken to 35% and 100% status. The submittals consisted of drawings, specifications, design analysis report, and corresponding construction cost estimates. The final RFP set will be the bid document that the Government will utilize to solicit for bids leading to the development of the construction document that the successful bidder will utilize to construct the facility.

FIRE RESISTANCE BASED ON PROPERTY LINE

Table 602 Assumption is that the distance from the building to any real property line is greater than 30 feet.

The new portion of project has no area limit, but the existing portion of the third floor is of unknown construction type. An assumed property line between new and existing portions of the building would require 3 hour fire separation. This would be in effect between floors as well.

SPECIAL PROVISIONS FOR I-2 OCCUPANCY.

407.3.2 At patient rooms, locks on doors shall be operable from the corridor side by authorized staff only, but not restrict egress from the patient room side shall be used.

407.4 Smoke barriers shall divide the third floor into refuge areas of not more than 22,500 square feet. Egress to smoke compartments shall be less than 200 feet.

This will necessitate the creation of 3 refuge areas as defined by 407.4.1 over the entire third floor. A single refuge area must be able to maintain the existing occupant load while absorbing a neighboring occupant load.

A refuge area occupant load must be able to be evacuated into a neighboring smoke compartment area. The following list provides the vital statistics and is based on the assumption that existing elevators can be equipped with automatically activated fire resistive rated magnetic hold open doors without the benefit of an elevator lobby.

Smoke Compartment Area 1.

Total Floor Area = 19,684sf Total Refuge Area* = 17,500sf Total Refuge Capacity = 583 occupants.

Smoke Compartment Area 2 containing Med/Surge Ward and PACU Total Floor Area = 21,061sf Total Refuge Area* = 18,353sf Total Refuge Capacity = 611 occupants

Smoke Compartment Area 3 containing existing portion of third floor.

Total Floor Area = 9,432sf Total Refuge Area = 9,432sf Total Refuge Capacity = 314 occupants.

*Mechanical, electrical, telephone and elevator related rooms not included in refuge area.

Table 1004.1.1 Occupancy Rates Accessory Storage and Mechanical Rooms = 300sf/person Inpatient treatment areas = 240sf/person

407.4.1 Refuge area occupancy rate for floors with patient beds = 30sf/person.

INCIDENTAL USE AREAS

Table 508.2 calls out Group I-2 waste and linen collection rooms as being 1 hour fire resistant. Since the whole floor will be equipped with automatic fire sprinklers, storage rooms do not need fire rated walls.

EGRESS

1005.1 The loss of any one exit will not reduce the available capacity of any one means of egress by 50%.

Table 1005.1 Egress Width for I-2 Occupancy.

Stairways: 0.3 inches per occupant Egress components: 0.2 inches per occupant Assuming 196 occupants on the third floor:

Required minimum stairway width based on occupancy = 58.8 inches Distributed over 8 stair enclosures, this equals 7.35 inches per stair. Minimum stair width governs.

Required minimum egress component width based on occupancy = 39.2 inches. Minimum corridor width governs.

1005.2 Door swings may encroach up to half way through the minimum egress path width. Doors fully open may encroach up to 7 inches into minimum egress path.

1006.3 Emergency power shall provide 90 minutes of means of egress illumination after power supply failure. This shall be provided for rooms needing more than one exit and along paths of egress.

1007.6.1 A 30” x 48” area of refuge shall be within each stair enclosure at the stair landing and free of the door swing. Per 1007.6.3, two way communication shall be available at the area of refuge with directions on use and contact information (1007.6.4). A sign fitting the requirements of ICC A117.1 with the text “Area of Refuge” shall be posted at the door at each area of refuge location (1007.7).

1008.1.1 Doors sizes

Doors accommodating the movement of beds shall have a minimum of 41.5 inches clear width. Patient room doors shall have a 44”x 84” size unless otherwise noted. OR, Endoscopy, Cystoscopy, and Orthoscopy, shall abide by VA standards. SPD doors shall be 48” wide standard. Double door ways shall be 72”x84” typical unless otherwise noted.

Minimum doorway size per code = 32”x80”. Standard door leaf size shall be 36”x 84” unless otherwise noted. ICU patient room doors shall be sliding with tempered glass with break away egress component.

1008.1.1.1 4” projections into the required clear width may occur between 34” and 80” above finish floor.

Below 34”, no projections are allowed.

1008.1.2 Doors shall swing in the direction of egress.

1008.1.3.4 Access controlled egress door shall be regulated by this provision of the code. Access to SPD from semi restricted corridor, egress through SPD to exit stair, access to ICU from public corridor, access to semi restricted corridor from ICU, and access to PACU from public corridor, shall all have access control.

1008.1.8 contains door hardware requirements.

1009. Standard stairway dimensions apply except as noted for area of refuge above.

1011. Standard exit signage applies. Both tactile and illuminated signs shall be provided as prescribed. No point along egress path or within a room with two or more exits shall have a sign more than 100 feet or greater than the listed distance, whichever is less.

1013 contains provisions for guard railing for generic use, fall protection at roofs and mechanical equipment.

U.S. Department of Veterans Affairs

1014.2 Egress through adjoining room guidelines are met.

1014.3 Common path of travel maximum is 75 feet. All spaces conform.

Table 1016.1 With automatic fire sprinklers, the maximum travel distance to an exit enclosure is 200 feet.

Design complies.

1017.2 Corridor widths minimums.

24” for access to electrical, mechanical, or plumbing systems or equipment.

44” minimum standard 72” for surgical areas – no bed movement 96” for bed movement areas.

Table 1017.1 Corridors may be non-rated with automatic fire sprinkler system.

1017.3 No dead end corridors occur.

1017.4 Details mechanical concerns related to rated and unrated corridors.

ACCESSIBILITY

1101 The entire 3rd floor shall be compliant with this chapter in coordination with ICC/ANSI A117.1-2003 or Federal Standard 595 Uniform Federal Accessibility Standards.

INTERIOR FINISHES for I-2

Table 803.5 Class B materials required.

803.1 Class B materials have a flame spread of 26-75 with smoke developed 0-450.

805 and 806 cover use of combustible finishes in Type I construction.

EXISTING BUILDING ALTERATIONS

3403.1 and 3403.2 These provisions in the code justify that both existing and new portions of the Building 2 shall conform to the 2006 IBC requirements for structure. Portions of the project not effecting existing structure nor adding more than 5% to existing forces shall not be required to bring existing structural elements up to current code.

3403.3 Nonstructural alterations to an existing building are permitted to be made of the same materials of which the building or structure is constructed, provided that they do not adversely affect any structural member or fire resistance rating of any part of the building or structure. This shall apply to the Building 1 portion that includes Med/Surge Ward.

It is inferred by these three provisions that the existing two floors in Building 2 will not need to be improved for nonstructural elements.

U.S. Department of Veterans Affairs

3rd Floor Addition for Surgery Center, PACU, SPD & ICU 01 10 10 VA Medical Center; Grand Junction, CO APPENDIX T AM-02

3403.4 Stairways in the existing portion of the building need not be brought up to current code where existing space and construction will not allow a reduction in pitch or slope. An area of refuge within existing stairway enclosures is prohibitive and therefore will not be accommodated.

2.4 ARCHITECTURAL PROGRAMMING

Due to the short turnaround time between notice to proceed and the first submittal due date HDR performed an accelerated and somewhat truncated programming effort that consists of the following steps:

• Building Survey

• Interviewed VA Medical Staff including Medical and Engineering to identify and record planned functions in the new Surgery Addition

• Prepared alternate floor plans for review and further discussion

3.0 STRUCTURAL

3.1 GENERAL DESCRIPTION OF DESIGN CONCEPT/APPROACH

The structure is currently a two story concrete building enclosed in pre-cast wall panels. Floor construction is two-way concrete slabs with drop panels supported by concrete columns, resting on foundation system which consists of concrete pile caps supported by steel piles. The new 3rd story is constructed with steel beam and column system and shall be enclosed by exterior light gage steel panels. New roof structural diaphragm consists of metal decking welded to steel framing.

3.2 PERTINENT CODES AND DESIGN REFERENCE DOCUMENTS

The following items listed are references used for design of the building:

3.3 SEISMIC, LIVE, DEAD AND WIND LOAD CONSIDERATIONS

According to the drawings of the existing structure by MCB Architects dated July 16, 1986, a 3rd story addition has been anticipated, and the structural elements are proportioned to resists the loads from the future added level so, the current roof is built flat, and according to the existing drawings, designed for 100 psf live load plus 25 psf partition loading.

The following loads are considered applicable in analysis of the structural:

Dead Loads – Self weight of the structure.

Live Loads – Floor live loads are as shown below which are in accordance to requirements of the IBC 2006, and additional partition loading of 25 psf is assumed. Reduced live load will be used to check the strength of the gravity load resisting system such as, girders, columns, and foundations as code permits

• Roof 20 psf uniform

• Operating Rooms 60 psf uniform & 1000 Lbs Concentrated

• Patient Rooms 40 psf uniform & 1000 Lbs Concentrated

• Corridors 1st floor 100 psf uniform & 1000 Lbs Concentrated

• Corridors above 1st floor 80 psf uniform & 1000 Lbs Concentrated

Super-Imposed Dead Loads – Super-Imposed loads are estimated to be about 10 psf, and consist of mechanical / electrical / plumbing loads, suspended ceiling, etc.

U.S. Department of Veterans Affairs

3rd Floor Addition for Surgery Center, PACU, SPD & ICU 01 10 10 VA Medical Center; Grand Junction, CO APPENDIX T AM-02

Wind Loads – Wind design load parameters are based on ASCE 7-05.

Snow Loads – 30 psf.

Seismic Loads – Seismic design of the existing structure is based on UBC 1985 and corresponding seismic response parameters are:

Seismic Zone = 1 Z = 3/16

Occupancy Importance Factor = 1.5 K = 1.00 CS = 0.14 (maximum)

Base Shear V (Service) = (Z I K C S) W = 0.0394 W Base Shear V (Ultimate) = 1.4 x (Z I K C S) W = 0.055 W

In other words, the structure is currently designed for base acceleration of only: 0.055 g

Seismic design load parameters based on IBC 2006 are as follows:

Site Class E Site Class D

Resp. Modification Factor R = 4 4 Seismic Importance Factor I = 1.50 1.50

Site Class = E D Ss = 0.287 g 0.287 g S1 = 0.067 g 0.067 g Fa = 2.382 1.57 Fv = 3.500 2.40 Sds = 0.456 g 0.300 g Sd1 = 0.156 g 0.107 g Ta = 0.318 sec 0.318 sec

Response Coefficient Cs = 0.137 g 0.090 g Seismic Design Category = D C ρ = 1.0 1.0

As can be seen above, the Site Class is assumed to be type “D,” which is a reasonable assumption in absence of more credible geotechnical information. The most recent geotechnical studies available on this site were performed by “Lambert & Associates” and the results can be found in their report dated January 2, 2008. The said report includes a section on seismicity, in which they make an unsubstantiated reference to presence of Site Class type “E.” As shown above, the difference between Site Classes “D” and “E” amounts to a substantial jump (52%) in the calculated base shear, and an entirely different Seismic Design Category. The structural calculations or the geotechnical report used to design the existing 2 story structure are not available at this time to investigate the effect of increased new base acceleration, however we will use the seismic parameters based on Site Class “D,” unless type “E” is substantiated.

4.0 MECHANICAL

4.1 GENERAL DESCRIPTION OF DESIGN CONCEPT/APPROACH

The mechanical systems for the 3rd floor addition to the clinic and renovation to the existing 3rd floor will be designed in accordance with the VA design guides and standards listed below. The systems include Heating, Ventilating and Air Conditioning (HVAC), plumbing (including medical gases and vacuum), sanitation waste and storm drainage.

The clinic was originally designed to accommodate two additional floors. The utilities (low, medium steam, domestic water, and sanitary waste) were design to accommodate these floors. Connection interface points for these utilities are located in the second floor mechanical rooms. The health services to be provided as part of the 3rd floor addition (surgery center, ICU, Recovery and SPD) tend to be very energy intensive. At this time it appears the capacities provided in the original design are adequate to support the addition

The design concept for each of the required systems is discussed below. Should the system descriptions be in conflict with any of the codes, standards and design guides discussed below, the codes, standards and design guides shall govern.

4.2 PERTINENT CODES AND DESIGN REFERENCE DOCUMENTS

The HVAC and Plumbing system design for the 3rd floor addition will comply with the following codes and standards, and Design Guides and those codes and standards referenced therein:

• International Mechanical Code – 2006

If the requirements of the documents referenced above conflict with the requirements of the codes and standards, the more stringent code, standard or design guide shall govern.

4.3 MECHANICAL SYSTEMS OUTLINE

4.3.1 Direct Digital Control System

The direct digital control system (DDC) shall be a Johnson Controls Inc (JCI). BacNet DDC system and shall integrate in to the existing JCI DDC system.

The system shall be configured such that each mechanical room has a local controller with an interface where maintenance can connect a laptop to troubleshoot system problems. The primary control point is the central plant control room. Control shall be through graphical displays that are identical in form and function as the existing displays. See the VA HVAC Design Manual for additional DDC requirements including minimum point schedule requirements.

4.3.2 Heating, Ventilation, Air Conditioning and Exhaust

The VA HVAC Design Manual for Hospitals along with the Surgical Service and Supply, Process and Distribution Design Guides provide the basis for the HVAC design for the health care services provided in the addition. The system concept described below was developed in accordance with these design guides and is designed to fit within the confines of the mechanical spaces shown on the VA’s preferred floor plan (see Appendix N). The design build contractor can propose an alternative design so long as it complies with the codes, standards and the design manuals described above, and conforms to the VA’s preferred floor plan.
