Attachment 4: GENERAL QUALITY CONTROL REQUIREMENTS
When the contract laboratory is performing sample analyses using the methods specified in Attachment 1, the contract laboratory shall implement ALL quality control requirements specified in that particular method. For all methods, the contract laboratory shall perform the method-prescribed QC operations for ALL/EACH sample fractions. Any modifications to reference method QC requirements must be documented in the laboratory’s analytical SOPs and identified as method deviations/differences (in the SOPs) from the reference methods.
In the event the contract laboratory does not meet accuracy and/or precision criteria for any QA/QC sample, the contract laboratory shall implement the corrective actions specified in project specific documents (which, if they exist, will be provided to the contract lab for discussion prior to sample submission), or in the contract laboratory’s Quality Manual or method SOP, as applicable.
For acceptance of all QA/QC sample analysis results, the following criteria will be used: 1) any applicable project specific QC acceptance criteria; 2) internal (to the lab) acceptance criteria (based on historical QA/QC sample analyses), if the criteria is at least as stringent as the method criteria; 3) if internal criteria does not exist, method acceptance criteria must be used; 4) if neither internal criteria nor method criteria exist, then reasonable criteria agreed to, per method, between the contract lab and PHC.
APHC requires site specific, method required quality control (spikes, duplicates, etc.) for work under this contract.
The Contractor must provide the QA/QC acceptance criteria in the report, along with the QA/QC result and whether or not the QA/QC result meets the acceptance criteria.
All re-analyses required because of breakdown of internal quality control system will be performed by contract laboratory at its own expense.
