ATTACHMENT C

SYSTEM FLIGHTS/READINESS STATUS INFORMATION PROGRAM

(Ref. SOW Para. 3.2.30, DI-MGMT-80555)

The Program shall address the following:

a. Summary (submittal and status) of QDR and Reports of Discrepancy (ROD).

b. Quality control information on frequency and type of problems associated with items of system equipment; unresolved problems along with recommendation for remedial action; items of special interest based on quality audits, safety surveys, investigations, and DCMA visits.

c. Target pool and TCS operability information (identify serial number, physical location, current configuration, and operational condition status).

d. Identity of targets that are deadlined for repair/spare parts, timeframe that the end item has been deadlined, listing of missing items, identification of resupply action taken by Purchase Request Number or Purchase Order Number, and quantities of items due in.

e. Corrective repair actions taken for TCSs and associated test equipment.

f. Identify equipment failure circumstances (i.e. bench check, pre-flight, launch, flight), failure reason, and cause analysis.

g. List operations and maintenance related improvement recommendations.

h. Identify targets flown, date of flights, and date targets were returned to flight ready status..

i. Unusual events/activities with focus on flight support efforts, safety, security, weather.

j. Problems that affect the overall aerial target flight services program with recommended solutions.

k. Identify official visitors to the Government furnished contractor facilities indicating duration of visit and purpose.

l. List significant decisions requested by the contractor that are pending Government response.

m. Outstanding Government requests placed on the contractor with status and expected reply date.

n. Identification of stock shortage items with projected zero balance dates.

o. Completion of a post flight analysis for all target flights that end prematurely.

p. Identity and description of outstanding Equipment Improvement Recommendations (EIRs) and Field Fixes.

q. Summary of current subcontracts to include, as a minimum for each: the originator of the request within the ATFS contractor’s organization, the date the request was originated, the date the request was submitted to the TMO, the date the request was approved, Local Purchase request number, the target system/end application, ATFS contractor’s Purchase Order (PO) number (or other standardized reference number), item/service purchased, quantity, unit price, PO line item total, PO grand total, ATFS contractor buyer’s name, vendor’s name and location, PO issue date, requested delivery date, actual delivery date, any consideration requested, and any relevant remarks/status information. In addition, for all current subcontracts experiencing delinquent deliveries, a statement of impact to the ATFS program shall be included. Current subcontracts shall be considered all those for which there are pending deliveries and those for which delivery has occurred within the previous 60 days. Subcontract summary shall be in a COR approved format.

r. Identity of employees requiring access to Government automated systems by name and system.

s. Identification of items bearing IUID marks that have experienced lifecycle events (receipts, losses/expenditures, or transfers) which require DOD IUID registry adjustment. Draft
