About this file
The Supplemental Bidder Attestation Form is a required document for vendors seeking to participate in the MPA 533 Open Enrollment solicitation issued by the State of Rhode Island Division of Purchases for the Rhode Island Department of Corrections. This form serves as an attestation mechanism for qualified vendors interested in providing food products through the Spot/Opportunity Buy Market program. Vendors must complete and submit this attestation form as part of their proposal process to be considered for inclusion on the qualified vendor list. The initial contract term runs from approximately June 2024 through July 2025, with potential renewal options for two additional 12-month periods. Proposals may be submitted at any time between April 29, 2024, and July 11, 2025, and vendors must register in the Ocean State Procures eProcurement system to submit their documentation. Evaluation of vendors occurs on a pass/fail basis, after which accepted vendors will be placed on a qualified vendor list for state agencies to request price quotes for specific food product needs.
Vendors participating in this program must maintain comprehensive insurance coverage including $1,000,000 in general liability, workers compensation, and automobile liability insurance. The solicitation emphasizes Minority Business Enterprise participation, and vendors must submit an MBE Participation Acknowledgment Form with their attestation. Additionally, at least one quote per project must originate from an MBE/WBE/VBE certified vendor if such vendors are available on the qualified list. The state will assess a 1% administrative fee on all purchase orders issued under the Master Price Agreement. There is no guaranteed purchasing volume, and the Division of Purchases reserves the right to make single or multiple awards, remove vendors from the qualified list for non-responsiveness, and cancel the solicitation at its discretion. The contract has statewide applicability, allowing participation from political subdivisions, institutions of higher education, municipal and state schools, quasi-public agencies, and the Legislative and Judicial branches of Rhode Island government.
