About this file

This is a Supplemental Bidder Attestation form issued by the Rhode Island Division of Purchases on behalf of the State of Rhode Island Department of Elementary and Secondary Education (RIDE) for the Higher Education Partnerships to Develop a School Psychologist Pipeline Program solicitation. The form, revised as of November 29, 2024, requires authorized signatories from bidding entities to certify compliance with Rhode Island General Laws 37-2-13.1(b) regarding conflicts of interest. Bidders must attest that their organization, including parent corporations, subsidiaries, affiliates, and subcontractors, have no conflicts of interest with any official, officer, or agency involved in the Request for Proposal and did not materially participate in or receive consultation regarding the RFP's requirements, technical aspects, or formation. Additionally, bidders must confirm that the solicitation does not relate to any audit, examination, independent verification, review, or evaluation of their work, financials, or operations previously performed for the State of Rhode Island.

The attestation must be completed and signed by an authorized representative of the bidding entity, with the signatory providing their printed name, signature, and the date of attestation. The form specifies that formal responses to prior Requests for Information (RFI) issued by the Division of Purchases do not constitute participation or consultation disqualifying vendors from subsequent RFP participation. The completed attestation is a requirement for bid submission and must include the solicitation number and title. Vendors must reference the conflict of interest definitions contained in Rhode Island Chapter 14 of Title 36 and acknowledge that being a current or past vendor to the State does not automatically create a disqualifying conflict of interest.

1 A “conflict of interest” occurs when private interests or relationships interfere in any way with the interests of the

State. Simply being a current vendor or a past vendor of the State does not in itself cause a conflict requiring preclusion. Additionally, formal responses to a Request for Information (RFI) issued by the Division of Purchases does not constitute “participation” or “consultation” with respect to a future RFP and does not disqualify a vendor from future participation from a subsequent RFP.
