Amendment 0002

1. THIS AMENDMENT IS BEING ISSUED FOR THE FOLLOWING REASONS:

A. To revise Attachment 1 ‐ Price Schedule. Offerors shall use and submit the attached ‘Attachment 1 – REVISED per

Amendment 0002’ with their proposal. Changes were made as described below.

i. CLIN descriptions on 6312F6 and 6312GA

ii. Estimated quantities on 6312GA and BA01C7

iii. U/M (from LB to EA) on NR93C6 and NR94C6

B. To revise Attachment 2, see the attached Attachment 2 – REVISED per Amendment 0002. Changes were made as described below.

i. Updated the tables under 1.2.6 LOADING and 1.2.10 BULKING & CONSOLIDATION

ii. Removed NR05B5 from 3.28

iii. Changed the word filter to fillings in 3.29

C. To answer the submitted questions on pages 3‐4 of this Amendment.

2. NO OTHER CHANGES ARE AUTHORIZED.

BIDDER Q&As SP4500‐20‐R‐0003 Amendment 0002 PAGE 3 of 4 PAGES

In accordance with 1. C. of this Amendment, please see the below answers to submitted questions.

1. Q – Please confirm the Price Schedule intentionally does not require pricing for CLIN’s NR90,93,94 for the Base Period.

A – Correct, do not enter pricing for CLINs NR90,93,94 for the base period.

2. Q – Are delivery orders for supplies‐only subject to task order minimums?

A – Yes, the task order minimum will apply to an order for supplies only.

3. Q – Is it the contractor’s decision to send Universal Waste to a state which does not have a Universal Waste program on a hazardous waste manifest? One advantage for waste generators of shipping as Universal Waste is to reduce their hazardous waste tonnage and state waste related taxes by utilizing a non‐hazardous manifest.

A – Please see section 1.1.3d.

4. Q – Clause 2.6.1 states the waste should be capable of being spread on a surface. However, it also references dry solids. What % of the material is the maximum dried solids that can be present and still qualify as fuel blendable?

A – It can be a complete solid or any percentage that will adhere to the surface without leaching.

5. Q – Clause 3.6 (h) lists potential disposal CLIN suffixes as B1,2,3, or 4. However, the waste in a Vacuum Box could contain high percentages of liquid waste which are not consistent with the B1‐4 solid waste CLIN suffixes. Please explain.

A – Please refer to the section on bulk liquids.

6. Q – Clause 3.28 references CLIN NR05B5 as the alternative disposal of bulk liquids if they are not eligible for NR03B5 disposal. However, there is not a NR05B5 CLIN on the bid matrix. Please explain or correct.

A – NR05B5 was removed from 3.28.

7. Q – Clause 3.29 “Dental Amalgam” has a RE prefix. The first sentence says fillings, capsules, and teeth. The second sentence says filters, capsules, and teeth. Please clarify if fillings and/or filters are present.

A – The word filters should read as fillings. Change has been made to 3.29, and yes they are present.

8. Q – Clause 3.30 references points “in and around” pick up points listed in this contract, however there is no specific number of miles these pick up locations are limited to. Please provide a maximum distance from the pickup locations.

A – In accordance with section 0.0 a., work will be located within a 50 mile radius of any pick up point listed on this contract.

9. Q – CLIN’s RE03,04,05,06 are listed per pound but are listed as ME (or EA) on other DLA contracts. Is this intentional?

If so, please provide an estimated weight per bulb.

A – Yes, all of the DLA contracts are now using pounds instead of each on light bulbs. Please research the bulbs you are questioning to find out estimated weights.

10. Q – Clause 3.31/CLIN6502 Please provide additional information. Does the service include waste pumping? (CLIN description lists pumping/3.31 does not.) If the CLIN includes pumping, what equipment is required to do the pumping?

What is CSF? What is operating a 90‐day? What could etc., include? What are the potential “waste storage area tasks?”

BIDDER Q&As SP4500‐20‐R‐0003 Amendment 0002 PAGE 4 of 4 PAGES

A – The service may include pumping; however, the contractor will not be required to provide equipment for pumping, if pumping services are ordered under this CLIN. The words CSF will be removed from CLIN description. Please research operating a HW 90‐day site. Etc. may include anything that was not listed or spelled out on operating a 90‐day site.

Please research operating a HW 90‐day site.

11. Q – Please identify the largest generators, load size (number of drums) and frequency of shipments.

A – Please see the attached Generator Summary Report with information from 01DEC2018 to 01DEC2019.

12. Q – Please provide the actual spend total for all task orders on the current contract for the past twelve months?

A – From 01DEC2018 to 01DEC2019 the total amount spent was $1,164,067.84.

13. Q – CLIN BA01C7, will they carry the D002 code? Please verify the estimated quantity.

A – They ‘may’ carry the D002 code. The estimated quantity has been changed on the Price Schedule from 390,000 to

300,000 LBS.

14. Q – Please verify the estimated quantity for CLIN TX06B6.

A – The estimated quantity on the Price Schedule is accurate.

ATTACHMENT 1 ‐ PRICE SCHEDULE

Facility CLIN Assignment Sheet

IMPORTANT EXCEL INFORMATION ‐ please note that Excel doesn’t always use the visible price when executing formulas.

If the cell is formatted for currency in USD, the visible price will always show the amount to the nearest cent. However, when an additional formula is used to calculate a unit price, for example an escalation from a base period unit price to an option period unit price, Excel uses the actual amount, not the price rounded to the nearest cent that is displayed in the cell on the spreadsheet. The amount shown in the formula bar, not the cell, is the figure Excel uses for calculations.

This can cause inconsistencies between the extended price shown for a CLIN and the offered unit price and subsequent inconsistencies in the total price for the schedule. Unit prices rounded to the nearest cent must be inputted into the spreadsheet. The unit price shown in the cell is the figure we will use to evaluate the price proposal. Offerors are advised to make sure there are not inconsistencies between the unit price shown and the figure in the formula bar for that cell.
