GENERAL REQUIREMENTS
PART 1 -GENERAL
1.0 SAFETY AND HEALTH PROGRAM: To ensure the Defense Distribution Depot San Joaquin (DDJC) maintains a viable Safety and Occupational Health Program, (as applicable) Contractors shall maintain on site a written Safety and Occupational Health plan which meets all applicable Federal, State and local laws and regulations. Contractor shall designate/identify a person as the Safety and Occupational Health representative. The contractor shall:
1. 1 The contractor shall submit a safety plan no later than one week prior to the pre-construction meeting. The safety plan will address/explain, at a minimum, the following:
The contractor shall provide safety meetings once a month minimum and shall maintain and provide documentation to the Safety Office.
The contractor shall maintain records for daily equipment checks.
The contractor shall conduct safety tailgate meetings prior to the beginning of daily work which pertain to the operations for that day.
A thorough discussion on all required Personal Protective Equipment (PPE) and the proper use/application
All local emergency contact numbers and the phone number/location of the nearest hospital.
How to protect government employees from job hazards (i.e. noise, welding, MHE hazards, overhead hazards, etc.).
Where/how to get fire permits for all welding/grinding operations.
License and certification requirements for every employee involved with any confined space or abatement (i.e., Asbestos, Lead Based Paint removal, etc.).
(Note: if using own company’s confined space documentation, the contractor shall submit the document to the facility’s Safety Office for review prior to work commencing.)
1.2 Notify the Contracting Officer Representative (COR) if any adverse safety and occupational health conditions. Mishaps involving contractor employees shall be reported to the Safety Office (SO), and COR immediately. Report will be given to COR within five (5) working days after accident occurrence.
1.3 In coordination with the COR, the Depot Safety, and Occupational Health staff inspections, surveys, assessments and audits will be conducted of the Contractor’s Safety and Occupational Health programs and processes on an unannounced basis. The frequency shall be at the option of the Safety Office.
2.0 HOURS OF WORK: Contractor's working hours, which do not require a power outage, will coincide with working hours of Defense Logistics Agency, DDJC, a normal five (5) day, forty (40) hour week, Monday through Friday, from 7:00 A.M. to 3:30 P.M., except Federal holidays. Federal holidays are those days officially observed by the Federal Government.

Saturday holidays are observed on the preceding Friday and Sunday holidays the following Monday. New Year's Day, Martin Luther King Day, President's Day, Memorial Day, July Fourth, Labor Day, Columbus Day, Veterans Day, Thanksgiving Day, Christmas Day, and any other day as may be declared by the President to be a Federal holiday.
3.0 TRAFFIC AND SECURITY REQUIREMENTS: Persons employed in performance of this contract, shall be subject to Security Regulations of Defense Distribution Depot San Joaquin. These regulations pertain to the following subjects:
a. Rules of the Road
b. Vehicle Registration requirements
c. Personnel Entrance requirements
d. Observation of "For Official Use Only" operations and associated restriction of discussing and writing about these operations.
Each Prime Contractor, Supervisor, Employee, must have in their possession, at time of registering, the following:
a. Current State Driver's License
b. Current Vehicle Registration
c. Certificate of Insurance for "Company Vehicle" or a current insurance policy giving an "expiration date" for those privately owned vehicles.
Without this information Contractor’s vehicle and or employees will not be allowed entrance to DDJC. Additional information may be obtained from Command Security, Bldg. 1A, Tracy Site, Defense Logistics Agency, DDJC, Tel: (209)839-4302.
4.0 CONTRACTOR - VISITOR BADGES:
4.1 A minimum of one week prior to entry and initiation of work on San Joaquin Facilities, the prime Contractor is to submit a list of all prime and subcontractor employees including any other personnel required to perform work on San Joaquin Facilities during the contract performance period. The list is to be submitted to the Contracting Officer's Representative (COR) and shall include Name, DL # and DL State. The same requirements will apply prior to any new employees' arrival for work at San Joaquin Facilities. Badges will be issued for the contract performance period. A check will be made periodically by Security on the status of these badges.
4.2 Suppliers, vendors, or other one-time or occasional visitors do not need to be escorted to the project site; however, a Contractor employee with a badge must notify the Security Officer at the gate. Notification will be done on the day of the visit, and include the name of the company and the nature of the visit. Security may refuse entry without this notification.
4.3 All expired or unnecessary badges will be returned to the issuing office. The Contractor agrees to pay a replacement fee of $22.00 for each badge not returned.
5.0 SUBCONTRACTORS: Within 14 days from date of award, successful Contractor will furnish to Contracting Officer a listing of all subcontractors to be utilized under resulting contract.
6.0 DISCOVERY OF UNIDENTIFIED MATERIALS OR OBJECTS: The potential exists due to the historical nature of the Tracy site active military support, for the discovery of
01000-3 hazardous materials such as, explosives, chemical (gas) weapons and toxic contaminants, during excavation and demolition operations. The following procedure is applicable:
6.1 Any suspect material or unidentified objects found during construction activities shall be assumed hazardous. Excavated soils with unusual appearance or odor shall be considered suspect.
6.2 Site workers shall immediately secure in a safe condition any equipment being operated and leave the area of the material or object. Do not approach, examine or move the material or object.
6.3 The discovery shall be reported immediately to the Depot Fire Department by calling *911 for both Sharpe and Tracy sites. (Outside phone line emergency numbers to the Depot Dispatch Center is 209-839-4302.)
7.0 PLANNED UTILITY OUTAGES: All utility outages shall be as short a duration as possible. Outages shall be scheduled and approved, with COR, as far in advance as possible, but no later than 72 hours before outage. Contractor shall obtain in writing, from COR, a statement or schedule giving permissible times of outages for particular installations and maximum time allowed for each.
8.0 FIRE PROTECTION:
a. Contractor shall comply with Life Safety Code, NFPA 101, NFPA 30 and NFPA 70, and follow procedures below:
1. Require employees to familiarize themselves with nearest alarm box, telephone and fire extinguisher.
2. Assure employees do not smoke in unauthorized areas.
3. Report all fires to Fire Department, even if already extinguished.
4. To report a fire, activate nearest fire pull box, call Fire
Department on fire phone or business phone by dialing *911 at DDJC, Sharpe or Tracy Site.
5. Depot fire extinguishers or hoses shall not be removed for any purpose other than for emergency use.
6. Open fires are not permitted on DDJC sites.
7. Post Indicator (PI) valves in water distribution systems will not be operated by any contractor or employee without approval from Fire Department.
8. No person shall knowingly maintain a fire hazard.
9. Hot Work permits shall be issued, on a daily basis only by the Fire Department to qualified welders only. Provide a minimum of 24 hours advance notice to ensure availability of inspector. At least 15 minutes prior to any welding or burning operation, contractor personnel shall apply to Fire

Department for an inspection of work site and have an approved fire extinguisher provided by the Contractor and an employee who will be assigned for fire watch duty at work site. An inspection of completion of welding, cutting, brazing, tar melting, or blow torch operation will be done by fire watch at end of each shift inside and outside of building or work site. Welding site shall be at least 35 feet radius of any combustible materials.
10. A fire extinguisher must be within ten (10) feet of fire watch at all times.
11. Materials required for work will be stored in a manner such as not to impede access to building or structure, at least twenty (20) feet from all buildings, fifteen (15) feet from fire hydrants, and clear of marked fire lanes. Flammable liquids containers shall not exceed sixty (60) gallons at anytime.
Open flames or smoking will not be permitted in flammable or combustible liquid storage areas at any time.
12. Only safety gas cans, with spring cover which functions as 5 psi relief valve, flame arrester capable of withstanding heat without failing and listed in Underwriters' Laboratories or Factory Mutual (5 gallon maximum capacity) will be utilized.
b. The following procedures shall be followed for refueling operations:
1. Smoking is prohibited anywhere flammable liquids are stored, handled, or used within 100 feet.
2. Proper fire extinguishers will be on hand at all times.
3. Combustibles shall be kept at least 25 feet away.
4. All containers of flammable liquids shall be plainly marked.
5. When refueling, all other equipment shall be turned off.
6. Refueling shall be done when engines are cooled.
7. Refueling will be done before starting work in the mornings.
8. Fueling operation will be attended by qualified Contractor personnel at all times.
9. Only equipment in a safe operating condition, will be used for any fuel handling operation.
10. Gasoline or diesel fuel will not be siphoned from vehicle for use in other vehicles, or for any other purpose.
11. NO smoking in buildings. Smoking allowed ONLY in designated smoking areas. Smoking on roofs is prohibited.

12. Funnel will be used during fueling operations to avoid spilling of fuel.
13. Vehicles or equipment will not be refueled inside buildings or on roofs of buildings.
9.0 CONTRACTOR SUBMITTALS: All required submittals shall be submitted to the Contracting Officer within 14 days after Notice to Proceed is issued, unless otherwise indicated.
9.1 The Contractor shall submit Project Work Schedule, Health and Safety Plan, and
Quality Control Plan within fourteen calendar days following contract award for Government approval and a copy of Contractor’s Company OSHA 300 LOG and OSHA 300A form for calendar year 2012, 2013, and 2014 (these forms are required for all companies with at least 10 employees). The Contractor shall allow a minimum of 14 calendar days in the schedule for the review and approval.
10.0 APPLICABLE SPECIFICATIONS, STANDARDS AND CODES
10.1 The following list of specifications, standards and codes shall apply to the work covered within this document. The Contractor shall be responsible to ensure that all such applicable regulations are satisfied. Special attention shall be given to Federal, State and Local requirements. The issue in effect for the following documents on the date of the solicitation forms a part of the specification to the extent specified herein:
