I. INTRODUCTION AND SOLICITATION SCHEDULE
A. Introduction
1. General
This Request for Proposals (hereinafter “RFP” or “Solicitation”) is being issued by San Francisco Sheriff’s Office (hereinafter, “SFSO” or “City”). SFSO, is seeking qualified suppliers (“Proposers”) to provide proposals (“Proposal”) for Discharge Planning Services, which provides a range of reentry services to a diverse population with the specific goal of reducing recidivism.
Comprehensive jail discharge planning, diligent case management, and strong partnerships with community-based programs are critical to connecting vulnerable reentry populations to relevant, effective services. Incorporating housing support as a component of the spectrum of services can often be the determining factor between success and recidivism.
The City has approximately 65 departments, ranging from small to very large. Some departments maintain facilities located outside of the geographic limits of the City. SFSO shall order goods and/or services covered by the awarded contract(s) through the issuance of individual Purchase Orders and/or Task Orders which shall be released against the awarded contract(s) during the contract term.
2. Selection Overview The City shall award a contract to the Proposer that meets the Minimum Qualifications of this Solicitation whose Proposal receives the highest-ranking score. Responsive Proposals will be evaluated by a panel (“Evaluation Panel”) consisting of one or more parties with expertise related to goods and/or services being procured through this Solicitation. The Evaluation Panel may include staff from various City departments. Proposals will be evaluated based on the criteria outlined herein. If applicable, a Contract Monitoring Division (CMD) Contract Compliance Officer will assess Proposal compliance with Local Business Enterprise (LBE) requirements and assign a rating bonus to Proposal scores. The CMD-adjusted scores (if applicable) will then be tabulated, and Proposers will be ranked starting with the Proposer receiving the highest score, then continuing with the Proposer receiving the second highest score, and so on.
B. Anticipated Contract Term A contract awarded pursuant to this Solicitation shall be non-exclusive with an original term of up to three years. The City at its sole, absolute discretion, shall have two options to extend the term for a period of two years or for a period of one year each, by mutual agreement in writing.
In the event such extension rights are exercised, all terms and conditions, requirements and specifications of the Agreement shall remain the same and apply during the renewal terms. The maximum contract period shall not exceed total of five years.
C. Anticipated Contract Not to Exceed Amount A contract awarded pursuant to this Solicitation shall have a not to exceed (“NTE”) amount of $511,077 for the initial term. All funding is contingent upon the availability of funds for every fiscal year covered under this RFP, satisfactory program performance, and demonstrated need for the proposed services. This amount is based on City’s estimated spend over the advertised initial contract term. Should City’s actual spend exceed its estimated spend for the initial term, City may in its sole discretion increase the contract NTE for the initial term. Should City exercise its options to extend the contract beyond the initial term, City may also elect to increase the NTE proportionally.

II. GOODS AND SERVICES REQUESTED
A. Goods and/or Services Requested This Solicitation is being issued by SFSO. SFSO is seeking qualified Proposers to provide Proposals for Discharge Planning Services.
Background Information
SFSO seeks to partner with an agency that has expertise in delivering a range of reentry services to a diverse population with the specific goal of reducing recidivism. County jails serve a complex subset of the population exhibiting one or more compounding risk factors, including homelessness, mental illness, substance use, and chronic medical issues. As a result, this subset of the population with co-occurring issues cycles in and out of not only jails but also hospitals and shelters with much greater frequency than most jurisdictions are able to support.
Many individuals with complex needs are not able to get the stable, comprehensive, and long-term services they require to break the cycle of incarceration. Frequent arrests are costly to the public. Comprehensive jail discharge planning, diligent case management, and strong partnerships with community-based programs are critical to connecting vulnerable reentry populations to relevant, effective services. Incorporating housing support as a component of the spectrum of services can often be the determining factor between success and recidivism.
In February 2018, SFSO initiated a Discharge Planning office to attempt linkage to services for high risk, high needs and vulnerable people who cycle in and out of the jail system.
To that end, SFSO conducts an annual analysis of all bookings and identifies the top 2% of persons booked into the San Francisco County Jail. The current cohort is comprised of 214 individuals with between five and seventeen bookings during fiscal year 25-26.
Proposers are encouraged to understand the demographics of individuals transitioning from the San Francisco County Jail to the community. A snapshot of the jail population on July 21, 2026 shows:
P-690 (01-26) 4 Sourcing Event 0000012066
The Demographic composition of the 217 individuals who comprised the top 2% of bookings in FY 25- 26 closely mirror the larger jail population:
Gender:
• 6% are women and 94% are men
Age Range:
• 6% were 18 - 25
• 88% were 26 – 54
• 6% were 55+
Ethnicity:
• 52% were Black
• 22% were White
• 22% were Hispanic
• 4% Asian/Pacific Islander
The San Francisco County Jail system includes an intake and release facility, housing facilities and the Community Programs Unit, which provides alternatives to incarceration as well as post-release services. The successful proposer will be expected to conduct assessments in custody, and will be provided with an office near the County Jail 1 property window.
Facility Location Use
County Jail #1 425-7th Street Intake and Release – No Housing
County Jail #2 425-7th Street The only jail in the system that houses women, but it also houses men. This jail also houses a sub-acute psychiatric housing unit and a medical step down unit.

I. Scope of Work
The Scope of Work is to be used as a general guide and is not intended to be a complete list of all work necessary to complete the project.
The purpose of the Discharge Planning office is to attempt linkage to services for high risk, high needs, and vulnerable people who cycle in and out of the jail system. The Sheriff’s Office is interested in partnering with a community-based agency that employs re-entry practices that support the following principles:
• Clients respond well to culturally competent, non-judgmental services, delivered in a manner that demonstrates respect for individual dignity, personal strength, and self-determination.
• Because those engaged in unsafe health practices are often difficult to reach through traditional service venues, reentry services providers will seek creative opportunities and develop new strategies to engage, motivate and intervene with potential clients.
P-690 (01-26) 6 Sourcing Event 0000012066
• Relapses or periods of return to unsafe health practices are not equated with or conceptualized as “failure of treatment”.
The San Francisco Sheriff’s Office seeks to select an experienced and highly qualified organization, that possesses a strong knowledge of and access to a wide variety of community-based resources.
The Discharge Planning Office maintains a focus on multiple subpopulations:
• Those most frequently booked into the jail (top 2%)
• Those with a known release date. SFSO provides a weekly list of persons who have been sentenced to County Jail. Typically no more than 4% of the jail population has been sentenced and on July 21, 2026, there were 32 individuals with Jail sentences.
• Those who have pled guilty and will be sentenced at the next court date. Our experience is that these clients are often released at sentencing and so it is critical to offer services during the post-plea period. SFSO will provide a weekly list which typically contains between 6 and 12 names.
• Special focus as directed by the Sheriff. Recent enforcement measures to combat open air drug markets have resulted in an increase in bookings of persons with drug charges and Discharge Planning staff are asked to make every effort to provide linkage to substance abuse treatment services to interested clients.
• On a daily basis, the provider will review booking alerts and respond to referrals from Jail Health Services, contractors, Five Keys staff and SFSO Rehabilitation Services Coordinators.
The provider shall:
• Conduct interviews, and assessments, and develop reentry plans for individuals who comprise the top 2% of bookings, are sentenced or post-plea, or who have been referred from other partners;
• Monitor criminal justice status of clients as needed;
• Refer clients interested in substance use disorder treatment to the Behavioral Health
Access Center or to SOMARise and communicate with Department of Public Health staff about these referrals.
• Provide referrals and linkages to in-custody and community-based organizations;
• Convene case conferences with in-custody and community-based providers;
• Collaborate effectively with the medical and legal system and with community providers;
• Communicate with staff from the Department of Homelessness and Supportive Housing’s
Jail Access Center for Coordinated Entry;
• Provide monthly and quarterly service measure data including number of case management clients who have been released to the community and their linkage status at release;
• Assist with “coordinated release” of newly released clients to community based case managers.
P-690 (01-26) 7 Sourcing Event 0000012066
• Assist newly released clients with transportation needs utilizing SFSO provided cab vouchers (Safe Passage Program).
• Utilize and maintain records with a single room occupancy facility that can be utilized for an emergency housing voucher program (Safe Harbor Program) that will provide a one night hotel stay for vulnerable, homeless, and / or high risk persons who are released from jail late at night.
• Monitor older adult jail population and create a weekly priority list for SFSO-contracted case manager focused on this population and attend monthly meeting focused on reentry plans for older adults.
• Monitor SFSO-created priority list for young adults, ensuring linkage and attend monthly meeting focused on reentry planning for transitional aged young adults (18-24).
P-690 (01-26) 8 Sourcing Event 0000012066
Staffing
The Sheriff’s Office will contract with a qualified community-based agency with demonstrated experience providing services to individuals with a history of criminal justice system involvement. The agency will provide on-site discharge planning services, with a Discharge Planner available between the hours of 2:00 p.m. and 10:00 p.m. SFSO will provide designated office space equipped with a desk, telephone, and internet access. Case management services shall include, at a minimum, one qualified Discharge Planner, an identified qualified relief staff person, and dedicated time from an identified qualified supervisor to provide oversight and support. Agency representation is required at regularly scheduled program meetings and applicable trainings.
Discharge planners must be able to meet SFSO guidelines for jail clearance and California Office of Justice standards for access to CORI information. Staff must also attain access to the JUSTIS Hub and the Jail Management System and biennially pass the California Law Enforcement Telecommunications System Less Than Full Access User examination.
Proposers are encouraged to submit staffing patterns that work for their agency, while clearly delineating how coverage for staff absences will be provided. Proposer is required to provide all necessary equipment and supplies (i.e., computer, etc.), training and other staff support as needed.
Data Management The successful proposer shall maintain case notes for every client interaction in its information management system to capture process outcomes such as the number of persons declining assistance and how many times they were approached.
On a monthly basis, program staff shall provide SFSO with the following service measures:
• Number of clients referred to Safe Passage
• Number of clients accessed Safe Passage hotel rooms
On a quarterly basis program staff shall provide SFSO with the following service measures:
• Number of clients from the Top 2% list who were approached in custody
