About this file

This is a submissions checklist for RFR 2026 DOI 006, issued by the Massachusetts Division of Insurance, soliciting qualified third referee appointees to provide two distinct service categories. Bidders may respond to one or both categories: third referee services for review of fire and casualty insurance agent contract cancellations, modifications, or expirations under M.G.L. c. 175, § 163, and third referee services for review of disputes regarding the amount of loss in fire insurance claims under M.G.L. c. 175, §§ 99 through 100H. The checklist establishes the required sequential order and content for all bid submissions, ensuring standardized evaluation across respondents.

The submission requirements encompass a comprehensive business response section that includes an introduction and cover letter, detailed qualifications description, consultant contractor mandatory submission forms with personnel resumes, three business references from organizations where similar services were previously provided (excluding current Division of Insurance employees), authorized signature verification, environmental practices form, Commonwealth terms and conditions, W-9 taxation reporting verification, standard contract form, and project-specific memoranda of understanding for each service category. Additionally, bidders must submit a cost proposal cover letter stating the proposal's ninety-day validity period from the response due date, along with the completed cost proposal form. The checklist must be submitted as the first item in the business response package to ensure compliance with all procurement requirements.
