About this file

This document is Exhibit E, a Statement of Qualifications Instructions for a Human Services Client and Staff Goods and Services Request issued by the Douglas County Department of Human Services in Colorado. The solicitation invites vendors to submit detailed qualifications for various goods and services categories, with the county seeking to establish a pool of short-listed vendors. The county aims to have at least three qualified responses per category and will evaluate vendors based on specific criteria, though designation as a short-listed vendor does not guarantee a subsequent service contract.

Vendors are required to follow specific submission guidelines, including using a minimum 11-point font, limiting responses to two single-sided pages, and avoiding generic phrases like "See attached". The instructions emphasize providing clear information about service rates, evidence-based services, insurance acceptance, and specific qualifications for each category of goods or services requested. Vendors can apply as new or existing providers, and must reference the categories and details outlined in Exhibit C when preparing their submissions. The solicitation appears designed to create a flexible vendor pool that can meet the diverse human services needs of Douglas County.
