1. The SOW includes “room modifications” - please define scope for each affected area. Is the goal for the renovation portions of this work to provide minimal renovation required for the equipment (typical VA /Military renovation standard)? Or, are upgrades to finishes, millwork, lighting, etc desired?
The scope of work for the room modifications solely includes work required to install the new equipment.
2. Stated under “price schedule” is the following: “this herein is considered an all-inclusive rate”. Will change orders be entertained/allowed?
Yes, change orders will be considered for new, unknown items; however, all items outlined in this RFQ should be priced prior to award. Price changes will not be made for taxes.
3. A “phased delivery schedule” is listed as a requirement in the “Combined Synopsis”. Please elaborate on this and provide timing for the phases and expectation of the overall schedule.
A phased delivery schedule will demonstrate project management capabilities by the vendor. It should be a critical path or similar style schedule. It will be used to inform the facility of your intended plan for the project implementation and will be further discussed and approved after award at a post-award kick-off meeting.
4. Is there room for equipment/tools/materials or will storage PODs be required?
No, there is not room in the existing facility. Vendors will be permitted to bring on a POD to store equipment/tools/materials.
5. Is there a room that can be used as a “construction office” during the course of work?
No, there is no room to have a construction office. There are RV spots that can be rented, if needed.
6. What are the capabilities of the facility’s staff? Will they be taking on any of the construction responsibilities?
None. No.
7. Is the expectation to have the PM on site whenever work is being done?
No. The expectation is to have one main POC that coordinates all communication between the vendor and the facility, providing bi-weekly (every other week) schedule updates reflecting completed and adjusted, pending scheduled items. A conference call will be held with the PM and facility bi-weekly to review the schedule and provide updates on outstanding items and progress. The PM will be responsible to provide meeting notes to the group within 72 hours after each meeting.
8. Are as-built drawings available for the building?
See Attachment A.
9. Do asbestos surveys exist for the renovated areas; if so, is it available for review? The R&F room has a double layer of flooring. This often indicates the potential for the presence of Asbestos confining flooring or mastic for the base layer. Is asbestos abatement work to be considered as part to the SOW or if required, addressed via a change order?
See Attachment B. Asbestos abatement, if needed, is included in the contract.
10. Does the facility have preferred sub-contractors that may already be familiar with the facility infrastructure and standards? Of special interest would be Electrical, Finishes, Fire Sprinkler, Fire Alarm, HVAC, and HVAC Controls (if BMS controls are present in the building).
No preferred sub-contractors. Please note: The facility HVAC controls are Automated Logic. The local (Phoenix) for Automated Logic is “Integrated Control Systems Inc.”

document. Some modalities have superscript numbers which refer to the notes at the end of the document.
If the software used on a modality is revised by the manufacturer, the modality will need to be re-validated.
The VA retains the right to change modality testing requirements at any time as needed for VA medical care. These changes could require re-testing of the modalities.
VA facilities are required to complete a quality review for the images produced by the device before it may be placed into clinical use. The results of the quality review must be filed with the National VistA Imaging Project Office.
This list of approved DICOM modalities does not in any way constitute a product endorsement by the VA. It only states whether or not the listed product has passed the VA’s internet-based validation testing. Upgrades to the vendor devices can affect interoperability and may require re-validation. The validation process does not indicate how well the actual product will work operationally, either within the VA or elsewhere.
