About this file

This is a Proposer Information and Certification Sheet for RFP S-62900-00017640 issued by the State of Oregon's Department of Administrative Services (DAS) Procurement Services for environmental planning and compliance services with the Umatilla National Forest. The contract seeks to establish an agreement for a Hydrologist and Fisheries Biologist to serve as members of an Umatilla National Forest Interdisciplinary Team (IDT). The RFP requires proposers to submit all required documents by the proposal closing date and time specified in the primary solicitation materials. The firm offer remains valid for 180 days following the closing date. Upon contract award, the selected proposer must perform the scope of work and meet performance standards as outlined in the final negotiated scope of work detailed in Attachment A.

The certification sheet requires authorized representatives to confirm understanding and acceptance of RFP requirements, receipt of all addenda, tax compliance status, and non-discrimination policies in employment and subcontracting practices. For contracts with an anticipated price of $150,000 or more, proposers must certify they have a written policy and practice meeting ORS 279A.112 requirements regarding prevention of sexual harassment and discrimination. Proposers must also certify they are not listed on the Office of Foreign Assets Control's Specially Designated Nationals and Blocked Persons list or the System for Award Management government-wide exclusions lists, possess no actual or potential conflicts of interest with the State, and that all proposal contents are truthful, accurate, and prepared independently without collusion or fraud. False or fraudulent statements may subject proposers to claims under the Oregon False Claims Act.
