About this file
This is a Prison Rape Elimination Act (PREA) training and acknowledgement form issued by the Massachusetts Department of Correction (DOC) for volunteers and contractors. The form serves as a prerequisite document for individuals seeking site access to DOC facilities in connection with contract opportunities, including the DRM 2026_37 New Dialysis Room project at MCI Shirley. Pre-site visit documents, including completed PREA forms and background clearance documentation, must be submitted to DRMProjectRequest@doc.state.ma.us no later than 1:00 PM on Wednesday, August 12, 2026.
The form requires contractors and volunteers to acknowledge understanding of federal and state laws prohibiting all forms of sexual contact, abuse, and harassment within DOC facilities. Signatories must confirm awareness that sexual misconduct is strictly forbidden and may result in discipline up to and including termination and criminal prosecution. The form explicitly defines sexual misconduct to include conversations, gestures, correspondence of a sexual nature, demeaning references, derogatory comments, obscene language, and voyeurism. Critically, all employees, contractors, and volunteers have a mandatory duty to report observed or suspected sexual misconduct to the Shift Commander, with failure to report subject to disciplinary action up to and including termination and potential criminal charges. The form is provided in both English and Spanish and requires printed name, company affiliation, signature, and date.
