About this file

This document is a Workers Compensation and Employers Liability Insurance Policy endorsement issued by Federal Insurance Company for PG Environmental, LLC, effective January 1, 2022. The endorsement specifically involves the Arizona Department of Environmental Quality located in Phoenix, Arizona, and includes a waiver of subrogation rights. The policy number is 71754106, and it contains special provisions for Missouri-based work.

The endorsement focuses on a waiver of the insurance company's right to recover payments from parties named in the Schedule, which in this case is the Arizona Department of Environmental Quality. The waiver applies only to work performed under a written contract requiring such an agreement. A specific note for Missouri-based policies indicates that the waiver does not apply to construction group classifications as designated in the insurance company's manual. The premium is listed as "Incl." (included), suggesting it is part of the overall policy cost.
