QUESTIONS AND ANSWERS

1. In Attachment 2, Article 4 Performance Requirements Summary on pages 34 & 35 the

Acceptable Quality Level (AQL) bonus/penalty program is described. Can you please provide how this has been awarded or penalized over the past 5 years under the existing contract?

We would like to know how much has been either awarded to penalize to the current contractor.

Response:

a) See Article 4 for response and following explanation:

DEP staff constantly monitors the performance of contract personnel to determine if work performance met the quality standards identified in the SOW and score the standards in Attachment

4 Performance Standard every week to compile a monthly summary report that is sent to the contractor every month.

Each performance standard is assigned a point value. If the performance standards are met, the full point value is awarded. And if the performance standards was not met, the full negative point value is awarded.

Total potential scores for each month is (+or-) 88 points.

At the end of the year a final report is prepared based on the average of all the (12) monthly points scored (+or-88) and added to the two other Performance Standards to be scored on an annual basis only (because the items/standards do not occur each month) with a total of (+or-)

12 points and making a grand total of (+or-) 100 points.

The contractor receives a monetary award based on 3% of the total annual value of invoices paid per contract year.

The ranges of annual score for monetary award or penalty are listed on page 35.

b) The amount awarded or penalized to the current contractor over the past 5 years cannot be provided.

2. We are also preparing our proposal and believe we see an error in the pricing pages. In attachment 23, under section 2. Front End Loader

(FEL) Services, Item (iv) Vertec Typ 8 cuyd

(including installation) shows 36 months in the base year, and the option years. This would be for three vertical compactors. In Amendment 5, under the NIH Campus Container Inventory, the only 8 cu yd Vertical compactor we identified was billing item #2.c(iv). We believe this should be corrected to 12 months in the base and option years.

Response: This RFP indicates the contract will be for an indefinite ¿delivery/indefinite-quantity with fixed price contract (see FAR 16.501-2). The appropriate type of indefinite-delivery contract

may be used to acquire supplies and/or services when the exact times and/or exact quantities of future deliveries are not known at the time of contract award. Government stocks are to be maintained at minimum levels. Historically, we have used one to three 8-cubic yard compactors.

While we are currently using one, we may require additional units in the future. The contract shall require the Government to order and the contractor to furnish at least a stated minimum quantity of supplies or services and, if ordered, the contractor to furnish any additional quantities not to exceed a stated maximum. The pricing sheet maximum indicates 36 mos¿, or three compactors, for the base and option years.

3. Can you please provide a manufacturer and model number for the MPW Yellow Collection Carts required in attachment 23, under section 8.

Medical Pathological Waste (MPW) Services, item

(j) MPW Carts.

Response: The MPW Yellow Collection Cart perfectly holds 30 MPW boxes in 2 rows and the dimension is 74 Lx39 Wx69 H. Manufacturer and model number not applicable, as they were custom-made in house. For reference, the carts are 2 rows deep 5 boxes across and 3 rows high, so 6 rows or 5 boxes.

The due date for proposals is 5/8/18 @ 10:00 AM.

There are no other changes as a result of this amendment.

End.
