Section C - Descriptions and Specifications

STATEMENT OF WORK

STATEMENT OF WORK (SOW) FOR

PREVENTIVE AND REMEDIAL MAINTENANCE ON

DIESEL GENERATORS AT

KITTERY, ME

1.0 SCOPE OF WORK. At present, Portsmouth Naval Shipyard is in possession of multiple diesel generators for which maintenance services are required to ensure operational availability. The maintenance services shall include two types of service: Preventive Maintenance (PM) scheduled and on call Remedial Maintenance (RM) unscheduled. The Portsmouth Naval Shipyard equipment maintenance requirements are subject to change by the Government when equipment addition(s) and/or deletion(s) are required due to equipment disposal or replacement. Appendices 1-3 identify the equipment requiring maintenance services.

2.0 GENERAL. The Contractor shall provide all labor, equipment, tools, materials, replacement parts, and supervision necessary to perform the maintenance services indicated in this SOW for the diesel generators located at Portsmouth Naval Shipyard (PNS) in Kittery, Maine unless otherwise specified by the Government. In some cases, materials and/or equipment may be more readily accessible by the Government. The Contractor must complete all work In Accordance With (IAW) the terms and conditions specified herein. All references in this SOW to “Contractor personnel” include both Contractor employees and subcontractors. The Government and the Contractor understand that the services to be provided under this contract by the Contractor are non-personal services and that no employer-employee relationship exists between the Government and the Contractor. The Government may provide technical direction which will assist the Contractor in accomplishing the maintenance IAW the SOW; however, the Government will not control the methods used by the Contractor to perform the service requirements set forth in the SOW, except where safety of personnel is at risk. The Contractor shall, at no time, perform any maintenance action (PM or RM) without a representative of the Temporary Services Production Shop (Code 990), the Temporary Services Engineering Division (Code 230), Control Engineering Division (2330), or Nuclear Refueling Division (2370) of Portsmouth Naval Shipyard present.

2.1 The following appendices identify the shipyard technical code that is cognizant of the diesel generator(s) specified in each Appendix, the periodicity of preventative maintenance, and service documentation required on each asset.

2.2 The Shipyard’s primary point of contact (POC) is the Temporary Services Electrical General Foreman***.

The alternate POC is the Temporary Services Shop Operations Manager***.

2.3 Additional security requirements (e.g. laptop registration form, etc.) may be imposed by the Government prior to granting access to secured spaces. All computer equipment shall be made available for inspection upon request, and in general should not contain camera equipment or recording capabilities of any kind.

Contractors are responsible to disclose any computer equipment being brought onto Government property prior to being granted access.

3.0 PERIOD OF PERFORMANCE. Maintenance services shall be performed during the base year period as shown in the contract.

4.0 PREVENTIVE MAINTENANCE (PM) (Scheduled). This maintenance service includes the care and servicing by Contractor personnel for the purpose of maintaining equipment in satisfactory operating condition in accordance with manufacturer’s recommended preventive maintenance tasks. The Contractor shall perform, at a minimum, all services as identified in Appendices 1-3.

4.1 All repairs required as a result of preventive maintenance inspection shall be accomplished under Remedial

Maintenance, with the approval of the Government.

5.0 REMEDIAL MAINTENANCE (RM) (Unscheduled). This maintenance service includes all actions to diagnose equipment malfunctions and restore the equipment to normal operating condition. The Government will advise the Contractor of the suspected problem and request an evaluation either via telephone or by an onsite visit. The Contractor will inspect the equipment and advise the Government of the problem and provide a cost estimate as soon as feasible. In many cases, remedial maintenance services will take place on active systems installed in production work environments governed under more strict government requirements. The Contractor will begin repairs only after approval by the Temporary Services Electrical General Foreman (or a designated representative) AND the Contract Specialist. If repairs are more extensive, or cost more than originally estimated, the Contractor will obtain the approval of the Government before continuing work.

5.1 The Contractor shall provide the Government with a point of contact and telephone number to receive and respond to remedial maintenance service requests. The Contractor shall ensure that this telephone number is monitored to guarantee at least a telephone response, not to exceed 1 hour, to RM service requests.

5.2 Remedial maintenance requests may be deemed “emergent” by the Government based on the circumstances of the deficiency. The Contractor shall be prepared to respond to all remedial maintenance requests in an urgent manner unless directed otherwise by the Government.

6.0 MAINTENANCE PERFORMANCE SCHEDULE.

6.1 Preventive maintenance services shall be completed within 5 business days of scheduled service, unless otherwise approved by the Government.

6.2 The Contractor shall ensure adequate personnel are available to report to Portsmouth Naval Shipyard in response to verbal notification of remedial maintenance service requests. The required timeframe for ON- SITE RESPONSE shall be within 2 HOURS of PNS contacting the Contractor. The Contractor shall be available 24/7/365 for remedial maintenance requests to Portsmouth Naval Shipyard (Kittery, Maine). The response time may be relaxed at the discretion of the Government based on individual circumstances.

6.3 An emergency call number will be provided to the Shipyard POC. Ten (10) weather resistant stickers with company name and contact information will be provided to the shipyard to post as deemed appropriate.

6.4 Upon arrival on site, Contractor personnel shall report to the Temporary Services Electrical Shop or designated Government representative. The Contractor shall continue remedial maintenance services until such time as the diesel generator is returned to full operational capability or as otherwise instructed by the designated Government representative.

6.5 In many cases, maintenance actions will be performed on power generation equipment that is being relied on as a source of standby power for critical equipment or evolutions. During periods of maintenance, the Contractor shall be responsible to restore operational integrity to all affected equipment in 30 minutes or less. If, based on the maintenance actions to be performed, this time constraint cannot be met the Contractor shall notify the Government immediately and request a determination on how to proceed.

Maintenance actions requiring diesel generators to be off line for extended periods of time will require specific permission from applicable technical organizations including Code 2330, Code 2340, Code 2370, and Code 230 (PNS).

7.0 REPLACEMENT PARTS/PRODUCTS. The Contractor shall use only new parts, or parts which have been refurbished to “like new” condition which enable the part to function or operate at a level equal to the specification of the original equipment. The Government may have an inventory of some replacement parts for the diesel generator equipment. The Contractor shall, prior to purchasing a replacement part for the diesel generator equipment, contact the Temporary Services Electrical Shop to see if the part is available from the Government-owned parts inventory. If the part is available, the Government will provide the part to the Contractor who shall use that part to perform remedial maintenance. An invoice must include:

A. Name and address of the Contractor;

B. Invoice date and number;

C. Contract number, contract line item number and, if applicable, the order number;

D. If a “like new” part has been installed or used in the repair of the equipment.

1. If replacement part(s) were government furnished, these shall be included with description, quantity and pricing it at $0.00.

2. Description, quantity, unit of measure, unit price and extended price of the part(s) replaced by the Contractor.

7.1 Parts used for preventive maintenance shall not be invoiced separately but shall be included as part of the

PM cost.

7.2 Replacement parts required in the performance of RM shall be invoiced to the Government in accordance with the terms and conditions of this contract.

7.3 Replacement parts used in the performance of this contract shall be the same or equal to those recommended by the manufacturer’s technical manuals or specifications.

7.4 The Contractor is responsible for obtaining Original Equipment Maintenance (OEM) Service Manual(s) for the specific diesel generator assets identified in Appendices 1 -3 that are not available from the Government.

7.5 The contractor shall be required to turn over to the Government all parts and equipment removed from the equipment covered in the contract if requested. Exceptions may be made by the Government for just causes (e.g. to obtain a core charge reimbursement, etc.). Exceptions shall be in writing and signed by the cognizant technical organization as defined in paragraph 2.0 of this SOW.

8.0 MAINTENANCE SERVICE DOCUMENTATION. The Contractor shall complete an accurate maintenance service document for all PM and RM services provided. Each maintenance service document shall contain the information applicable to PM or RM services and shall be submitted within 5 working days after completion of the maintenance services. Either electronic or hard copy delivery is acceptable.

Appendices 1-3 identify maintenance service documentation required for each diesel generator asset.

9.0 MANDATORY REPORT. The Contractor shall report all Contractor labor hours (including subcontractor labor hours) required for performance of services provided under this contract for the Portsmouth Naval Shipyard via an invoice presented to the Government. The Contractor is required to provide all required data information requested by the Government. Inputs will be for the labor executed during the period of performance during each Government fiscal year (FY), which runs October 1 through September 30. While inputs may be reported any time during the FY, all data shall be reported no later than October 31 of each fiscal year, beginning with 2016. Contractors may direct questions to the Temporary Services Electrical Shop General Foreman.

10.0 CONTRACTOR PERSONNEL. Contractor personnel shall wear required Government furnished badge, and be easily recognized. This may be accomplished by Contractor personnel wearing distinctive clothing bearing the name of the company and/or by wearing appropriate badges which contain the company name and employee’s name.

10.1 Contractor personnel shall comply with directives pertaining to operation of privately owned vehicles on the Portsmouth Naval Shipyard.

10.2 The Contractor shall assure that Contractor personnel completely understand the requirements of this Statement of Work. All Contractor service technicians must have sound mechanical aptitude and ability;

must have basic understanding of machinery, both mechanical and electrical; must have the ability to understand and comprehend technical manuals, drawings, diagrams and any other technical data supplied by various manufacturers. All Contractor service technicians must have working knowledge of diagnosis and repair for the types of equipment included in this SOW and be capable of diagnosis and repair.

11.0 QUALITY CONTROL PLAN. The Contractor shall establish and maintain a complete Commercial

Quality Control Plan to ensure the requirements of the contract are provided as specified.

11.1 The plan shall include an inspection system covering both preventive and remedial maintenance repairs required by this contract. The plan must ensure that quality service levels are maintained throughout contract performance.

11.2 The Contractor shall maintain records of all inspections conducted by the Contractor and any necessary corrective action taken. This documentation and all records and schedules, as described in the SOW, that are the responsibility of the Contractor, are the property of the Government and shall be made available to the Government during the term of the contract.

11.3 Immediately following any maintenance action(s), either remedial or preventive, the equipment undergoing maintenance actions shall be operationally tested for satisfactory performance. The Contractor shall remain on site and available during operational testing. Operational test procedures will be determined on a case-by-case basis by the Government. Technical recommendations may be solicited from the Contractor prior to performance. Operational testing may include, but is not limited to, load bank testing, start-up and shut down procedures, extended run tests, etc.

11.4 Contractor personnel may be required to support the Government in various forums aimed at failure analysis and quality assurance. When equipment requires technical evaluation and investigation following an identified deficient condition, the Contractor shall be required to provide the same response times as indicated in paragraph 5.0 and 6.0. Assistance from Contractor personnel may include, but is not limited to, written statements, meeting attendance, technical evaluation, on site demonstrations, and failure analysis.

12.0 QUALITY ASSURANCE. The Government is responsible for performing Government quality assurance functions for all services and supplies furnished under this contract. Contractor performance is subject to review by the Government and Contract Official. Upon contract award, the identity of the Government POC will be promptly furnished to the Contractor.

12.1 The Government will advise the Contractor in writing when discrepancies occur, and will request corrective action. The Government will make annotation of the discrepancy; the date and time the deficiency was noted, and will request the Contractor (or authorized representative) to initial the entry.

13.0 MAINTENANCE SUPPORT HOURS. The Contractor shall provide preventive maintenance (PM) support services Monday through Friday between the hours of 0700 and 1530, excluding federal holidays.

The Government observes the following federal holidays:

If the holiday falls on a Saturday, it will be observed on the preceding Friday. If the holiday falls on a Sunday, it will be observed on the following Monday. The Contractor shall not be required to perform PM services on these days. Also, in the event an Executive Order issued by the President of the United States declares Agencies of the Federal Government closed on a Contractor-scheduled PM, service date, the Government will advise the Contractor on whether PM services should be performed for that day or services should be rescheduled.

13.1 The Contractor shall provide remedial maintenance support services Sunday through Saturday, including federal holidays, between the hours of 00:00 and 23:59.

14.0 ENVIRONMENTAL COMPLIANCE. The Contractor shall comply with all Federal, State and Local regulations as well as NAVSEA installation rules and policies regarding the use, storage, transport and disposal of hazardous materials and wastes. The contractor is responsible for all liquid spill protection to ground surfaces. The contractor shall be responsible to maintain a spill cleanup kit (in a ready to deploy condition) on site during maintenance operations. Any liquid or solid matter shall be cleaned prior to the contractor leaving the site. If for any reason, the site is left in an unsatisfactory condition, the contractor shall be responsible for clean-up at no additional charge to the Government. In the event of a spill to the ground or surface waters, it shall be the Contractor’s responsibility to immediately notify a representative of the Temporary Services Production Shop (Code 990), the Temporary Services Engineering Division (Code 230), Control Engineering Division (2330), or Nuclear Refueling Division (2370) of Portsmouth Naval Shipyard to contact the Federal Fire Department.

14.1 The Contractor shall provide a Safety Data Sheet (SDS) to the Contracting Officer or Government for any chemicals brought on site to perform the requirements in the contract and shall provide the SDS to the Contracting Officer or Government prior to bringing the chemicals to the worksite.

15.0 SECURITY REQUIREMENTS. The Contractor or authorized representative shall submit to the

Government a roster, on company letterhead, identifying all Contractor personnel who will be performing work under this contract and require access to the equipment for which maintenance services are required.

The roster shall contain the following information about each individual. An updated roster and individual information shall be provided to the Government when any Contractor personnel changes occur.

(1) Name (last, first, middle initial), aliases

(2) Status (type of employment)

(3) Name of company

(4) Office Phone

(5) Home address

(6) Date of birth

(7) Weight, height, color hair and eyes

(8) Length of anticipated employment

(9) Social Security Number

15.1 The roster shall be furnished to the Government within 5 business days after the date of contract award.

When any Contractor personnel changes occur, the Contractor shall provide an updated roster and individual information to the Government within 5 business days. The prompt submission of this information will keep to a minimum any delay and inconvenience to the Contractor and Contractor personnel in performance of the contract. The information will be used for accomplishment of the Form PTSMH 5521/11486, Local Visitor Request, completed by the Government for NAVSEA Portsmouth Site Security Officer.

15.2 Contractor personnel must bring personal identification which indicates proof of United States citizenship, such as an original birth certificate or current Passport, and all completed forms to the Site Security Officer for badge photograph and identification when notified by the Government.

15.3 The Contractor and Contractor personnel shall comply with all security requirements imposed by the installation commander at all times while on the installation. Contractor personnel are prohibited from carrying items such as alcohol, illegal drugs, and firearms on Government property. Entry into some Government spaces may constitute consent to search persons and all property within their control. Personal Electronic Devices (PEDs) are restricted inside the Controlled Industrialized Area (CIA). PEDs include camera equipped mobile phones, photographic equipment, audio recording devices, etc. Contractor personnel involved in civil crimes and/or other incidents of misconduct may be restricted entry to the base.

The Contractor is responsible for insuring badges are returned within three calendar days of employee termination or contract completion. Contractors may be provided a more complete list of security requirements upon request.

15.4 The Contractor shall prohibit the use of Government issued keys or Electronic Access Cards by any persons other than Contractor personnel. The Contractor shall prohibit the opening of locked areas by Contractor personnel to permit entrance of persons other than Contractor personnel engaged in the performance of assigned work in those areas.

16.0. SAFETY REQUIREMENTS. Work Site Operational Safety Environment – Contractors and non-PNS

Government Agencies (NPGAs) shall be responsible to read and understand the requirements of OSHECM Chapter 250 and can see an electronic copy of OSHECM Chapter 250 (latest revision) by using the following web site:

The Contractor shall provide and require their personnel to wear personal protection equipment (PPE) (e.g., hearing protection, eye protection, hard hat, gloves, steel-toed shoes or boots, aprons, masks, face shields, reflective vests, safety harnesses for fall protection in accordance with OSHA CFR 1926 (construction) and CFR 1915 (shipyard) standards. The contractor is required to follow all appropriate lockout/tag out requirements as specified by OSHECM Chapter 250 and CFR 1915.89. Contractor shall conform to all applicable safety and occupational health standards published in the Federal Register or any other Occupational Safety and Health Administration (OSHA) Public Laws and ensure employees observe safety, traffic control, and fire prevention requirements in effect at the site.

16.1 Hazardous Energy Control Contractor Compliance Certification Document on page 12 in Appendix 4 must be signed and returned to the Government prior to working on the shipyard.

16.2 The Contractor shall notify the Contracting Officer or Government immediately (within one (1) hour) of the occurrence of any accidents and incidents resulting in personal injury, loss of life, property damage to a government facility or equipment.

16.3 The contractor shall be aware that the shipyard is recognized as a Voluntary Protection Program (VPP) Star

Site under the Occupational Safety and Health Administration. The Contractor is not required to “certify” to VPP, but Contractor employees must be aware of the VPP and the Shipyard’s safety efforts in maintaining the recognition.

17.0 GOVERNMENT-FURNISHED FACILITIES AND SUPPORT SERVICES.

17.1 Work area: The Government will make available during performance of this contract work spaces close to equipment location. The Contractor shall be responsible for the orderliness and cleanliness of all areas utilized in the performance of this contract. These areas will be neat and clean, free from fire and safety hazards and unsanitary conditions.

17.2 Utilities: When possible, the Government will make available at no cost to the Contractor water and electricity for Contractor connection at Government provided work areas. Given the scope of this work (e.g. power outages, inclement weather, mechanical failures, etc.), government provided utilities may not be available for use by the Contractor therefore requiring the Contractor to be self-sufficient with respect to air, water, and electrical utilities. The Contractor shall instruct employees in utilities conservation practices.

The Contractor shall be responsible for operating under conditions which preclude the waste of utilities and shall include the following:

A. Lights shall be used only in areas where and when work is actually being performed.

B. Mechanical equipment controls for heating, ventilation, and air conditioning systems shall not be adjusted by the Contractor or by Contractor personnel in Government areas unless authorized.

C. Water faucets or valves shall be turned off after use.

D. Government telephones shall not be used for personal reasons. Telephone access will be made available for the Contractor’s use for approved calls only. All calls require prior authorization by the Government. Unauthorized calls by the Contractor will be reported to the Contracting Officer for resolution.

18.0 Emergency Medical Services. The Government will provide emergency medical treatment and emergency patient transportation service for Contractor personnel. The Contractor shall reimburse the Government the cost of medical treatment and patient transportation service at the current impatient or outpatient treatment rate as appropriate. Telephone number for Urgent Care is: 2333 from a Portsmouth Naval Shipyard phone or 911.

19.0 GOVERNMENT OBSERVATIONS. Government personnel other than representatives from the

Temporary Services Electrical Shop may, with Contractor/Government coordination, observe or inspect Contractor operations. These personnel may not interfere with Contractor performance.

20.0 DOCUMENTATION AND RECORDS. All documentation, records, and schedules, as described in this

SOW, which are the responsibility of the Contractor, are the property of the Government and shall remain so upon termination or completion of the contract. The Contractor shall keep these items current.

Documentation, records, and schedules shall be turned over to the Government upon termination or completion of the contract. All documentation, records, and schedules are subject to the Freedom of Information Act and privacy Act.

END OF STATEMENT OF WORK

VENDOR ACCESS TO PORTSMOUTH NAVAL SHIPYARD

Vendor access to Portsmouth Naval Shipyard: No person not known to be an American citizen of good standing and repute shall be eligible for access to Portsmouth Naval Shipyard and adjacent areas. Proof of citizenship may be required. Entrance of foreign nationals requires approval. (Ref: SECNAVINST 5510.34)

SAFETY/SECURITY

SAFETY: All representatives who have a need to enter the Controlled Industrial Area, or other areas specifically designated as safety hazardous, should provide themselves with safety head wear to be worn while in the area.

Vendors and/or subcontractors performing work on vendor-owned or government-owned plant facilities and equipment shall ensure that all OSHA safety regulations are followed. Prior to working on the Portsmouth Naval Shipyard, all vendors shall view the Code 106 safety video.

SECURITY: Commercial vehicles (automobiles) must have conspicuous contractor (company) identification (such as magnetic door-type signs) affixed to the outside of the vehicle if it is to enter the Controlled Industrial Area.

RESTRICTIONS ON USE OF YELLOW MATERIAL

Yellow colored items such as those described below are of special significance within the Shipyard and are subject to strict controls. Accordingly, contractors shall not use yellow or orange-yellow colored materials for the following purposes: protective clothing, hoods, sheeting, tarpaulins, polyethylene bottles or other containers, tapes, bags, banding, identification marks on tools, boundary markers, ribbons, vent ducts, etc. Contractor generated yellow colored waste shall be disposed of by the Contractor off-yard. Shipyard refuse containers shall not be used for disposal of yellow colored waste materials. Yellow colored contract generated debris shall be bagged in non-translucent containers, and promptly removed from the Portsmouth Naval Shipyard.

RADIOLOGICAL INDOCTRINATION

All Contractor employees performing work within the Portsmouth Naval Shipyard must view a radiological video.

Contractor employees who are expected to be on the Shipyard for greater than thirty (30) calendar days must attend a one-half hour indoctrination briefing.
