ADDENDUM TO FAR PROVISION 52.212-1 ENTITLED “INSTRUCTIONS TO OFFERORS –

COMMERCIAL ITEMS”

I. GENERAL

In addition to FAR 52.212-1, “Instructions to Offerors – Commercial Items” and any other instructions contained elsewhere in this solicitation, the following information is provided. Quoters are required to submit their quotes in two separate volumes as follows:

Volume I: Non-Price Quote

The quoter shall provide a video on CD/ROM in which their proposed speaker will demonstrate task knowledge, speaking ability and style of the instructor. The video shall be 10 to 15 minutes in duration.

The video shall clearly display the speaker, both visually and aurally, in the act of presenting to an audience. The quoter also has the option to provide an electronic link in their technical proposal instead of the CD/ROM. Additionally, the performance approach submission shall provide a demonstration of the quoter’s understanding of and familiarity with, the specific requirements as indicated in the Performance

Work Statement.

If the quoter chooses to submit a CD/ROM, all CD/ROMs should be mailed to the following address:

Quoters simply providing general statements or paraphrasing/parroting the PWS in whole or in any part shall result in a rating of unacceptable.

Factor II: Past Performance

The quoter shall provide information on no more than three (3) previous contracts whose effort was relevant to the effort required by this solicitation; the contracts provided should have been performed within the last five (5) years from the closing date of this solicitation. Evaluation will focus only on work experience already performed. Yet-to-be performed work and experience prior to the last five (5) years will not be considered. Quoters may submit performance data regarding current contract performance as long as a minimum of one year of performance has been completed as of the closing date of this RFQ. The

Government may contact some or all of the references provided, as appropriate, and may collect information through questionnaires, telephone interviews and existing data sources to include but not limited to Contractor Performance Assessment Reporting (CPARS). The Government reserves the right to obtain information for use in the evaluation of past performance from any and all sources including sources outside of the Government.

The quoter shall complete a “Past Performance Information Form” for each reference submitted. The form is an attachment to the solicitation. The forms will count toward the Volume II page limit described above.

For additional information regarding a particular reference beyond that which will fit on the form, the quoter may continue onto another sheet of paper. Such continuation sheet(s) for submitted references will count toward the Volume II page limit. The quoter shall also provide a copy of the corresponding

Performance Work Statement (PWS)/Statement of Work (SOW)/Statement of Objectives (SOO) for each contract referenced in the quoter’s past performance submission. The PWS/SOW/SOO will NOT be included in the past performance page count. A quoter’s past performance submission may be determined to be “Not Relevant” if this substantiating information is not provided.

For purposes of this solicitation, a contract is defined as a single contract or a single task order placed under an Indefinite Delivery/Indefinite Quantity (ID/IQ) contract, a Blanket Purchase Agreement (BPA), or a

Federal Supply Schedule. Additionally, quoters may submit, as part of its (up to) three (3) past performance contracts, a combination of up to three (3) delivery orders (DOs), task orders (TOs), technical direction letters (TDLs), and/or technical area tasks (TATs) placed under an ID/IQ or BPA. Such a combination of

DOs, TOs, TDLs and or TATs must also meet the requirements stated above: each individual example in the combination must demonstrate at least one (1) year of completed performance and have been performed within five (5) years of the solicitation issuance date.

Quoters may provide contracts describing their own experience as a subcontractor. Quoters shall describe the major or critical aspects of the work performed, as well as the dollar value of work performed, under the submitted contract.

Quoters may also submit contracts of subcontractors they propose to use under this contract. Quoters shall describe the major or critical aspects of the work subcontractors are proposed to perform under the contract.

Subcontractor performance (either the quoter’s own subcontractor experience or proposed subcontractors experience) of major or critical aspects of this requirement will be considered as highly as prime contractor past performance information for the quoters.

The quoter shall demonstrate relevant and completed past performance or affirmatively state that it possesses no relevant past performance. Relevant past performance is performance under contracts or efforts within the past five years prior to the solicitation closing date that is the same as or similar to, the scope and magnitude of the work described by this solicitation.

Additionally, the Government requests the quoters provide a Past Performance Report Form (PPRF) to each point of contact (POC) identified and submitted as Past Performance information. The PPRF shall be emailed directly from the quoter’s POC to the NAVSUP FLC Contracting Department POC Shannon Davis at shannon.davis@navy.mil by the due date and time of this request for quote. The Government reserves the right to consider past performance report forms received after the due date of the solicitation and to contact references for verification or additional information.

Volume II Price Quote

This volume shall include the following completed solicitation documents (to be provided in Volume II):

A complete and signed SF1449 (Solicitation/Contract/Order for Commercial items) and executed copy of

Amendments, if applicable, completed by the quoter;

Page 3 of the RFQ “Schedule of Supplies/Services” completed by the quoter;

A completed “Edutainment Contract Pricing” spreadsheet provided as an attachment of this RFQ. The pricing spreadsheet will indicate pricing information for all events and well as the total.

The completion and submission of the above items will constitute a quoter and will indicate the quoter’s unconditional assent to the terms and conditions of this RFQ and any attachments and/or exhibits hereto. All price and price supporting information shall be contained in the price quote. No price or pricing information shall be included in any other volume including cover letters. Quoters are responsible for submitting sufficient information to enable the Government to fully evaluate their price quote.

The quote shall be valid for 180 days from solicitation closing date.

IMPORTANT NOTES:

(1) In order to be eligible for award, the quoter shall be a small business under the NAICS 611430. The quoter shall submit documentation substantiating this qualification as a matter of responsibility. Any quoter determined to be not qualified will be ineligible for award.

(2) Quoters shall respond to all requirements of the solicitation document. Quoters are cautioned not to alter the solicitation. Hand-carried quotes are not authorized and will not be accepted. Each page of each copy shall be affixed with the following legend:

(3) In the event any person who is not a bona fide employee of the quoter participated in the creation, formulation, or writing of any portion of the quote, a certificate to this effect shall be included in the quote which shall be signed by an officer of the quoter. Such certificates shall identify the name of the person who is not a bona fide employee, that person’s employment capacity, the name of the person’s firm, the relationship of that firm to the Quoter, and the portion of the Quote in which the person participated.

(4) In order to ensure that all questions submitted by potential Quoters are answered prior to the solicitation closing date, one consolidated list of questions concerning the solicitation should be submitted via e-mail to the contracting point of contact, Shannon Davis at shannon.davis@navy.mil NO LATER THAN 11:00

AM, Eastern Standard Time (EST), on Wednesday, 11 July 2018. The Government reserves the right not to respond to any questions received concerning this solicitation after the questions receipt date above.

Accordingly, Quoters are encouraged to carefully review all solicitation requirements and submit questions to the Government early in the Quote timeframe.

(5) Quotes are to be submitted via electronic mail submission to Shannon Davis at shannon.davis@navy.mil; in the attachments, format and quantities as identified. All electronic files and versions of quotes shall be compatible with the current release of Adobe Acrobat and Microsoft Office Suite version 2010 (Excel).

II. REQUIREMENTS FOR FORMAT OF QUOTE CONTENT

(1) Introduction and Purpose: This section specifies the format that quoter shall use in this RFP The intent is not to restrict the quoters in the manner in which they will perform their work but rather to ensure a certain degree of uniformity in the format of the responses for evaluation purposes.

(2) Each attachment should contain the following items in addition to the other information required by this solicitation:

(3) Requirements for Style: Each quoter shall submit a quote that clearly and concisely describes and defines the contractor’s response to the requirements contained in the RFQ. Unnecessary elaboration or other presentations beyond that sufficient to present a complete and effective quote are not desired.

Each volume is limited to the maximum number of pages as defined in the table above. These page limitations are inclusive of the executive summary and any charts, diagrams, and/or other graphics. Graphics (including tables) in the quote may use an alternative font with 8 point size type or larger. Each “page” is defined as one sheet, 8 ½ x 11 inches, with at least one inch margins on all sides, using a font with a point size of 10 or greater (e.g., Times New

Roman style with 10 point font). Lines shall, at a minimum, be single-spaced. Pages shall be consecutively numbered. Multiple pages, double pages, two-sided pages, or foldouts will count as an equivalent number of 8 ½ x

11 inch pages. Quoters are permitted to submit no more than one page for each of the following which will not be included in the page count: a cover sheet, list of tables, list of figures, tabs, and dividers. Tables of contents and any compliance matrices may exceed one page, which will not be included in the page count. Pages submitted in excess of the page limitations described above will not be evaluated.

The price quote is not page limited; however, the price quote is to be strictly limited to price information and completed solicitation documents as described in the General Section at the beginning of this text.

PERFORMANCE WORK STATEMENT FOR U.S. FLEET FORCES COMMAND (USFFC)

IN SUPPORT OF USFF WORKSHOPS COUNTERING DESTRUCTIVE BEHAVIORS

Edutainment (All-Hands) Speaker – Workshop

1. INTRODUCTION:

United States Fleet Forces Command (USFFC) supports both the Chief of Naval Operations (CNO) and

Combatant Commanders worldwide by providing responsive, relevant, sustainable Naval forces ready-for-tasking. The command provides operational and planning support to Combatant Commanders and integrated warfighter capability requirements to the CNO. Additionally, USFFC serves as the CNO's designated

Executive Agent for Anti-Terrorism/Force Protection, Individual Augmentees (IA), and Sea Basing.

In collaboration with U.S. Pacific Fleet, USFFC organizes, mans, trains, maintains, and equips Naval forces, develops and submits budgets, and executes readiness and personnel accounts to develop both required and sustainable levels of Fleet readiness. Additionally, the command serves as the unified voice for Fleet training requirements and policies to generate combat-ready Navy forces per the Fleet Response Plan using the Fleet

Training Continuum.

2. BACKGROUND:

Fleet Workshops are designed to focus on prevention efforts to minimize the impact of destructive behaviors on

Sailors and their families which directly impact Fleet readiness. Destructive behaviors include, but are not limited to: alcohol related incidents (including Driving Under the Influence); suicide related behaviors; suicide, domestic violence; sexual assault; sexual harassment; discrimination; hazing; and drug abuse.

Sailors engaging in destructive behaviors impact the amount of time and resources commands have to accomplish their mission. Productivity is lost when a Command leadership team must adjust valuable command time away from mission accomplishment to personally manage an individual Sailor’s readiness or recovery, post-catastrophic event. Additionally, if a Sailor must be away from the command to support recovery from destructive behaviors (legal adjudication, medical care, counseling, etc.) mission readiness is further degraded by the temporary loss of that Sailor. Further, if a Sailor is separated as an unplanned loss to the Navy due to behavioral problems, the long-term gaps for a command can be significant. By focusing proactive prevention efforts at all levels of the chain of command, adverse impacts to unit readiness and to individual Sailor’s lives will be avoided. Reactionary efforts post destructive behavior erodes unit cohesion undermine mission accomplishment, and likely lead to temporary or permanent loss of the Sailor to the Navy.

3. PURPOSE:

The purpose of this effort is to provide Fleet Personnel Programs support services for USFFC. Support services shall include training aimed to bring awareness, reduce risk, prevent, and provide education on the range of destructive behaviors that affect Navy Sailors, inclusive of suicide, drug and alcohol abuse, domestic violence, and sexual assault prevention. It shall address strategies to reduce stress and increase resiliency and strength in our Sailors.

4. SCOPE:

This task shall require part-time Contractor support to perform services on various dates beginning 02 August

2018 through 21 September 2018. Direct labor for this task shall be in accordance with this PWS. Working hours shall vary, as required, to support twenty Fleet Workshops throughout the USFFC Area of Operations

(AOR). Training shall be constructed to provide targeted messaging for “All Hands” (all ranks, E-1 to O-9) sessions with the use of “Edutainment,” which provides education and awareness through the use of an entertainment format, leading into a more serious, and specific, message about the impact of destructive behaviors on an organization. The “Edutainment” should be an innovative presentation that engages Sailors in the understanding that climate change within both the assigned command and the local micro-climates in which a Sailor works every day may require specific shifts in culture by both the individual and the organization to prevent destructive behaviors before they take place. Training and raising awareness are both critical components to prevention, but are complementary to the required shifts to achieve true, primary prevention.

RECOGNIZED FEDERAL HOLIDAYS:

Below are the traditional Federal holidays in which Government facilities will be closed. With the exception of responsibilities, such as 24-hour operations to support training events/exercises and real world events, where prior approval has been granted by the COR to perform on-site at the Government installation, Contractor shall follow their corporate policy for work on recognized Federal holidays where on-site performance cannot be conducted:

6. TASKS:

6.1 US Fleet Forces Command Workshops to counter destructive behaviors focus on encouraging commands to integrate their prevention efforts on Suicide and Suicidal Behaviors, Sexual Assault, Domestic Violence, Alcohol Abuse, and illicit and prescription drug abuse. Further, these workshops also encourage individuals struggling with overcoming these destructive behaviors to reach out for assistance and to understand that their situation is something the Navy can help them with. Individuals presenting at the

Fleet Workshops must be prepared to address these destructive behaviors, demonstrating knowledge of current Navy policy and philosophy on countering destructive behaviors while providing “edutainment” presentation, 60-75 minutes each in length at each site indicated in paragraph 9. The presentations shall provide awareness, education, and insight to; at a minimum, the four (4) destructive behavior topics listed below:

(1) Sexual Assault Prevention Training (SAPR): This task involves providing consistent, comprehensive training on sexual assault prevention. Objectives of the training are to reduce the number of sexual assault incidents in the Fleet Forces areas of operation, provide members of all ranks with tools to enhance decision-making concerning destructive behavior, enhance command climate by encouraging active intervention pre-assault, and provide command level leaders with tools and strategies to increase knowledge and improve command program effectiveness. Training shall be designed to emphasize Navy core values of Honor, Courage, and Commitment, focus on a theme of respect for self and others, and provide a comprehensive discussion on sexual relations and consent. Contractor must demonstrate an understanding of current Navy SAPR Protocols in the wake of a sexual assault and provide best practices to help Sailors prevent sexual assault before they happen, whether intervention for Shipmates observing potentially destructive situations, or prevention for Sailors involved in risky circumstances. Sexual assault understanding must also provide insights into identification of precursor behaviors that could lead to sexual assault for both male and female victim sexual assaults.

(2) Suicide Prevention Training: This task involves providing consistent, comprehensive training on suicide prevention. Objectives of this training are to reduce the number of suicides and suicide related behaviors, increase knowledge and awareness of contributing factors and key indicators that may lead to suicide or suicide related behaviors, and to provide command level leaders with tools and strategies to improve on their command programs. Training should emphasize individual resilience and coping skills and emphasize individual responsibilities to ask for command support.

Contractor must demonstrate an understanding of the difference between an individual asking for help (suicidal behavior) and an individual who is at risk of actual suicide, providing tools and resources available to Sailors in such circumstances and best practices for Shipmates of such Sailors.

(3) Drug and Alcohol Abuse Training: This task involves providing consistent, comprehensive training on drug and alcohol abuse prevention. Objectives of this training are to enhance Fleet readiness by reducing the number of drug and alcohol related incidents and provide effective drug and alcohol prevention and education for all members of the Navy within Fleet Forces. Training shall provide tools and strategies to improve current program prevention efforts, emphasize personal responsibility, emphasize counseling, treatment, and restoration to active service. Drug & Alcohol abuse prevention train shall be outlined to align with the Navy’s “Keep What You Have Earned”

Campaign. Contractor must Demonstrate an understanding of the potential alcohol and drugs have to enable other destructive behaviors and the ways individuals can overcome such influences, along with an understanding of Navy alcohol and drug abuse prevention programs.

(4) Domestic Violence Prevention: This task involves providing consistent, comprehensive training on domestic violence prevention. Objectives of this training are to reduce incidents of domestic violence and minimize the negative impact of violence on Sailors and their families. Training shall highlight key indicators of spouse and child abuse, provide tools and strategies for individuals to deal with everyday stressors, emphasize resiliency, and outline potential resources for support. Contractor must demonstrate an understanding of the various types of domestic abuse to include physical and sexual violence. Additionally, be prepared to explore the various tools that an abuser may use to control their victim: economic; intimidation; emotions; isolation; pets; and children.

6.2 GENERAL TRAINER DUTIES:

Trainer shall clearly articulate the importance of the subject matter and relate it to the duties and responsibilities of military leaders, front line supervisors, and program managers.

Trainer shall present training applicable to each group and be prepared to speak to several thousand personnel at each location with many groups being in excess of 500 personnel.

Trainer shall coordinate with the Government representative and make all travel arrangements to ensure timely arrival, set up and preparations before training at each location.

Trainer shall coordinate with the Government team lead in advance of special requirements such as laptop computer, projection requirements, microphones, etc. well in advance of each event.

“EDUTAINMENT” is defined as Educational Entertainment (also referred to by the

Edutainmenthttps://en.wikipedia.org/wiki/Educational_entertainment - cite_note-2) is media designed to Educate through Entertainment. Most often it includes content intended to teach but has incidental entertainment value. It has been used by academia, corporations, governments, and other entities in various countries to disseminate information in classrooms and/or via television, radio, and other media to influence viewers' opinions and behaviors.

“EDUTAINMENT” shall be implemented by the trainer using an established model with positive references and a minimum of 10 completed sessions with the Navy or other DoD services. The trainer must be able to integrate interactive events to effectively reach the Navy audience.

“EDUTAINMENT” must be presented consistently for All Hands events regardless of the size of the audience.

Trainer must possess general administrative skills with the ability to express self clearly and concisely, both orally and in writing.

Trainer shall coordinate with the Government Representative for training services. He or she monitors all work in progress and coordinates for any necessary or desirable changes, progress, quality, or materials.

Trainer shall provide a mechanism that measures feedback forms each training session and report the findings to the Government Representative. An electronic means of providing feedback is strongly preferred. Paper surveys must not be utilized.

The contractor must be capable of taking general direction and develop curriculum that meets objectives without detailed guidance for assigned training.

7. INSTRUCTOR EXPERIENCE REQUIREMENTS:

The specific instructor who will be utilized at each location, not the bidding company, shall have a minimum of ten (10) years’ experience in the below and five (5) years of training requirements for the Department of

Defense or any of its agencies.

a) Adult education requirements with specific expertise in edutainment/infotainment providing face to face training.

b) Presenting for no less than 60-75 minute durations in large sessions.

The instructor shall also have a minimum of five (5) years’ experience in providing training in the following subject areas:

10. DELIVERABLES:

a. Trip Reports (Travel): The contractor shall submit a post travel trip report using a word document submitted via e-mail to the Government Representative within five (5) business days following the conclusion of the trip. The report shall include a summary of time tables, accomplishments, significant discussions/events, contacts and action items.

b. After Action Report: The contractor shall provide an executive summary using a word document or PDF that contains a brief overview of the trainings conducted as well as course evaluations to the trainees within five (5) business days following the conclusion of the training event.

11. SECURITY AND ACCESS REQUIREMENTS:

All references and deliverable items associated with this task are unclassified. A portion of the work will be performed in overseas locations and a valid U.S. Passport is required. There may also be country clearance requirements that may have to be met prior to departure from the United States. There are no other known requirements for access to restricted areas or information connected with the performance of this task.

IAW DODM 5205.02M, contractor shall protect information listed in the Critical Information List (CIL) and any other sensitive, but unclassified information that may be useful to our adversaries. Contractor shall abide by OPSEC policies and procedures, as detailed in DODM 5205.02M, COMUSFLTFORCOMINST 3432.1A, and/or as directed by the OPSEC Manager/Officer or an OPSEC Coordinator. Contractor shall accomplish

USFF initial OPSEC training within 90 days of in-processing and complete refresher OPSEC training annually.

Contractor shall notify their OPSEC Coordinator or the OPSEC Manager/Officer of recommendations for the

Contractor Unclassified Access to Federally Controlled Facilities, Sensitive Information, Information

Technology (IT) Systems or Protected Health Information (Apr 2017)

Homeland Security Presidential Directive (HSPD)-12, requires government agencies to develop and implement

Federal security standards for Federal employees and contractors. The Deputy Secretary of Defense Directive-Type

Memorandum (DTM) 08-006 – “DoD Implementation of Homeland Security Presidential Directive – 12 (HSPD-

12)” dated November 26, 2008 (or its subsequent DoD instruction) directs implementation of HSPD-12. This clause is in accordance with HSPD-12 and its implementing directives.

APPLICABILITY

This text applies to contractor employees requiring physical access to any area of a federally controlled base, facility or activity and/or requiring access to a DoN or DoD computer/network/system to perform certain unclassified sensitive duties. This clause also applies to contractor employees who access Privacy Act and Protected Health

Information, provide support associated with fiduciary duties, or perform duties that have been identified as National

Security Position, as advised by the command security manager. It is the responsibility of the responsible security officer of the command/facility where the work is performed to ensure compliance.

Each contractor employee providing services at a Navy Command under this contract is required to obtain a

Department of Defense Common Access Card (DoD CAC). Additionally, depending on the level of computer/network access, the contract employee will require a successful investigation as detailed below.

ACCESS TO FEDERAL FACILITIES

Per HSPD-12 and implementing guidance, all contractor employees working at a federally controlled base, facility or activity under this clause will require a DoD CAC. When access to a base, facility or activity is required contractor employees shall in-process with the Command’s Security Manager upon arrival to the Command and shall out-process prior to their departure at the completion of the individual’s performance under the contract.

ACCESS TO DOD IT SYSTEMS

In accordance with SECNAV M-5510.30, contractor employees who require access to DoN or DoD networks are categorized as IT-I, IT-II, or IT-III. The IT-II level, defined in detail in SECNAV M-5510.30, includes positions which require access to information protected under the Privacy Act, to include Protected Health Information (PHI).

All contractor employees under this contract who require access to Privacy Act protected information are therefore categorized no lower than IT-II. IT Levels are determined by the requiring activity’s Command Information

Assurance Manager.

Contractor employees requiring privileged or IT-I level access, (when specified by the terms of the contract) require a Single Scope Background Investigation (SSBI) or T5 or T5R equivalent investigation , which is a higher level investigation than the National Agency Check with Law and Credit (NACLC)/T3/T3R described below. Due to the privileged system access, an investigation suitable for High Risk national security positions is required. Individuals who have access to system control, monitoring, or administration functions (e.g. system administrator, database administrator) require training and certification to Information Assurance Technical Level 1, and must be trained and certified on the Operating System or Computing Environment they are required to maintain.

Access to sensitive IT systems is contingent upon a favorably adjudicated background investigation. When access to IT systems is required for performance of the contractor employee’s duties, such employees shall in-process with the Navy Command’s Security Manager and Information Assurance Manager upon arrival to the Navy command and shall out-process prior to their departure at the completion of the individual’s performance under the contract.

Completion and approval of a System Authorization Access Request Navy (SAAR-N) form is required for all individuals accessing Navy Information Technology resources. The decision to authorize access to a government IT system/network is inherently governmental. The contractor supervisor is not authorized to sign the SAAR-N;

therefore, the government employee with knowledge of the system/network access required or the COR shall sign the SAAR-N as the “supervisor”.

The SAAR-N shall be forwarded to the Command’s Security Manager at least 30 days prior to the individual’s start date. Failure to provide the required documentation at least 30 days prior to the individual’s start date may result in delaying the individual’s start date.

When required to maintain access to required IT systems or networks, the contractor shall ensure that all employees requiring access complete annual Information Assurance (IA) training, and maintain a current requisite background investigation. The Contractor’s Security Representative shall contact the Command Security Manager for guidance when reinvestigations are required.

INTERIM ACCESS

The Command's Security Manager may authorize issuance of a DoD CAC and interim access to a DoN or DoD unclassified computer/network upon a favorable review of the investigative questionnaire and advance favorable fingerprint results. When the results of the investigation are received and a favorable determination is not made, the contractor employee working on the contract under interim access will be denied access to the computer network and this denial will not relieve the contractor of his/her responsibility to perform.

DENIAL OR TERMINATION OF ACCESS

The potential consequences of any requirement under this clause including denial or termination of physical or system access in no way relieves the contractor from the requirement to execute performance under the contract within the timeframes specified in the contract. Contractors shall plan ahead in processing their employees and subcontractor employees. The contractor shall insert this clause in all subcontracts when the subcontractor is permitted to have unclassified access to a federally controlled facility, federally-controlled information system/network and/or to government information, meaning information not authorized for public release.

CONTRACTOR’S SECURITY REPRESENTATIVE

The contractor shall designate an employee to serve as the Contractor’s Security Representative. Within three work days after contract award, the contractor shall provide to the requiring activity’s Security Manager and the

Contracting Officer, in writing, the name, title, address and phone number for the Contractor’s Security

Representative. The Contractor’s Security Representative shall be the primary point of contact on any security matter. The Contractor’s Security Representative shall not be replaced or removed without prior notice to the

Contracting Officer and Command Security Manager.

BACKGROUND INVESTIGATION REQUIREMENTS AND SECURITY APPROVAL PROCESS FOR

CONTRACTORS ASSIGNED TO NATIONAL SECURITY POSITIONS OR PERFORMING SENSITIVE

DUTIES

Navy security policy requires that all positions be given a sensitivity value based on level of risk factors to ensure appropriate protective measures are applied. Contractor employees under this contract are recognized as Non-

Critical Sensitive [ADP/IT-II] positions when the contract scope of work require physical access to a federally controlled base, facility or activity and/or requiring access to a DoD computer/network, to perform unclassified sensitive duties. This designation is also applied to contractor employees who access Privacy Act and Protected

Health Information (PHI), provide support associated with fiduciary duties, or perform duties that have been identified as National Security Positions. At a minimum, each contractor employee must be a US citizen and have a favorably completed NACLC or T3 or T3R equivalent investigation to obtain a favorable determination for assignment to a non-critical sensitive or IT-II position. The investigation consists of a standard NAC and a FBI fingerprint check plus law enforcement checks and credit check. Each contractor employee filling a non-critical sensitive or IT-II position is required to complete:

SF-86 Questionnaire for National Security Positions (or equivalent OPM investigative product)

Two FD-258 Applicant Fingerprint Cards (or an electronic fingerprint submission)

Original Signed Release Statements

Failure to provide the required documentation at least 30 days prior to the individual’s start date shall result in delaying the individual’s start date. Background investigations shall be reinitiated as required to ensure investigations remain current (not older than 10 years) throughout the contract performance period. The

Contractor’s Security Representative shall contact the Command Security Manager for guidance when reinvestigations are required.

Regardless of their duties or IT access requirements ALL contractor employees shall in-process with the

Command’s Security Manager upon arrival to the command and shall out-process prior to their departure at the completion of the individual’s performance under the contract. Employees requiring IT access shall also check-in and check-out with the Navy Command’s Information Assurance Manager. Completion and approval of a System

Authorization Access Request Navy (SAAR-N) form is required for all individuals accessing Navy Information

Technology resources. The SAAR-N shall be forwarded to the Navy Command’s Security Manager at least 30 days prior to the individual’s start date. Failure to provide the required documentation at least 30 days prior to the individual’s start date shall result in delaying the individual’s start date.

The contractor shall ensure that each contract employee requiring access to IT systems or networks complete annual

Information Assurance (IA) training, and maintain a current requisite background investigation. Contractor employees shall accurately complete the required investigative forms prior to submission to the Command Security

Manager. The Command’s Security Manager will review the submitted documentation for completeness prior to submitting it to the Office of Personnel Management (OPM); Potential suitability or security issues identified may render the contractor employee ineligible for the assignment. An unfavorable determination is final (subject to SF-

86 appeal procedures) and such a determination does not relieve the contractor from meeting any contractual obligation under the contract. The Command’s Security Manager will forward the required forms to OPM for processing. Once the investigation is complete, the results will be forwarded by OPM to the DoD Central

Adjudication Facility (CAF) for a determination.

If the contractor employee already possesses a current favorably adjudicated investigation, the contractor shall submit a Visit Authorization Request (VAR) via the Joint Personnel Adjudication System (JPAS) or a hard copy

VAR directly from the contractor’s Security Representative. Although the contractor will take JPAS “Owning” role over the contractor employee, the Command will take JPAS "Servicing" role over the contractor employee during the hiring process and for the duration of assignment under that contract. The contractor shall include the IT

Position Category per SECNAV M-5510.30 for each employee designated on a VAR. The VAR requires annual renewal for the duration of the employee’s performance under the contract.

BACKGROUND INVESTIGATION REQUIREMENTS AND SECURITY APPROVAL PROCESS FOR

CONTRACTORS ASSIGNED TO OR PERFORMING NON-SENSITIVE DUTIES

Contractor employee whose work is unclassified and non-sensitive (e.g., performing certain duties such as lawn maintenance, vendor services, etc. ...) and who require physical access to publicly accessible areas to perform those duties shall meet the following minimum requirements:

Must be either a US citizen or a US permanent resident with a minimum of 3 years legal residency in the United

States (as required by The Deputy Secretary of Defense DTM 08-006 or its subsequent DoD instruction) and

Must have a favorably completed National Agency Check with Written Inquiries (NACI) or T1 investigation equivalent including a FBI fingerprint check prior to installation access.

To be considered for a favorable trustworthiness determination, the Contractor’s Security Representative must submit for all employees each of the following:

SF-85 Questionnaire for Non-Sensitive Positions

Two FD-258 Applicant Fingerprint Cards (or an electronic fingerprint submission)

Original Signed Release Statements

The contractor shall ensure each individual employee has a current favorably completed National Agency Check with Written Inquiries (NACI) or T1 equivalent investigation, or ensure successful FBI fingerprint results have been gained and investigation has been processed with OPM

Failure to provide the required documentation at least 30 days prior to the individual’s start date may result in delaying the individual’s start date.

* Consult with your Command Security Manager and Information Assurance Manager for local policy when IT-III

(non-sensitive) access is required for non-US citizens outside the United States.

12. GOVERNMENT FURNISHED RESOURCES (GFR):

The Government will provide on-site workshop furnishings to include the venue, microphones, and projection equipment as required. The Government will also complete all announcement and advertising for each event location.

13. ENTERPRISE CONTRACTOR MANPOWER REPORTING APPLICATION (ECMRA):

The contractor shall report contractor labor hours (including subcontractor labor hours) required for performance of services provided under this contract for the training services via a secure data collection site.

Contracted services excluded from reporting are based on Product Service Codes (PSCs). The excluded PSCs are:

(1) W, Lease/Rental of Equipment;

(2) X, Lease/Rental of Facilities;

(3) Y, Construction of Structures and Facilities;

(4) D, Automatic Data Processing and Telecommunications, IT and Telecom- Telecommunications

Transmission (D304) and Internet (D322) ONLY;

(5) S, Utilities ONLY;

(6) V, Freight and Shipping ONLY.

The contractor is required to completely fill in all required data fields using the following web address:

Reporting inputs will be for the labor executed during the period of performance during each Government fiscal year

14. STATUS OF FORCES AGREEMENT (SOFA):

52.212-2 EVALUATION--COMMERCIAL ITEMS (OCT 2014)

The Government will award a contract resulting from this solicitation to the responsible vendor whose quote conforming to the solicitation will be most advantageous to the Government, price and other factors considered. The following factors shall be used to evaluate quotes: Technical, Past Performance and Price. A determination of responsibility will be made prior to award.

The Government will conduct a Lowest Price Technically Acceptable (LPTA) source selection process. To determine the LPTA quoter, the Government will place all quotes in order of lowest total price to highest total price.

Starting with the lowest total priced quote, the Government will review Technical and Past Performance for acceptability. If the quote is found to be “Acceptable,” no other quotes will be reviewed and award will be made.

However, if the quote is found to be “Unacceptable,” the next lowest total priced quote will be considered. This process will continue until either a quote is considered “Acceptable” or until all quotes have been considered

“Unacceptable.”

In order to be considered awardable, the quoter must have received an “Acceptable” rating in every non-priced factor. LPTA non-price factors for this requirement will consist of Technical and Past Performance. Past

Performance will be evaluated for acceptability based on the relevance and quality of the submitted…
