Summary of Collier County Medical and Pharmacy Reviews for 2022-2024
This document is a consultative analytics report prepared by Cedar Gate for Collier County Government, analyzing medical and pharmacy claims data for county employee health plans covering the periods January-December 2021 and January-December 2022. The report provides comprehensive utilization and cost comparisons against government industry benchmarks, detailing claims paid totaling $30.4 million in 2022 (down 11.9% from $34.5 million in 2021) and covering approximately 2,246 current employees with 4,736 total members. Key findings indicate that medical PMPM (per member per month) decreased 10.1% to $549.58 but remains 49.0% above benchmark rates, with significant variances in specific service categories including emergency room visits (+428.8% above benchmark), office visits (+108.5% above benchmark), and inpatient services (+44.8% above benchmark). The report documents 54 catastrophic claimants with claims exceeding $100,000, accounting for 31.5% of total plan spend at $12.8 million, with the top diagnosis groups being cancer (9 individuals), infections (5 individuals), and gastrointestinal disorders (6 individuals).
The data reveals concerning utilization patterns, including 30-day hospital readmission rates 103.5% above benchmark (7.1 per 1,000 members) with 18 of 30 readmissions related to mental health or substance abuse disorders, substance abuse admissions increasing 168.9% year-over-year to 5.0 per 1,000, and mental health office visits increasing 6.6% to 1,030.1 per 1,000 members. Network utilization shows 67.3% of claims concentrated with CHP Network (51.8% discount), 24.8% with Cigna (58.9% discount), while out-of-network claims increased from 4.9% to 5.8% of total. The member-to-subscriber ratio of 2.11 exceeds the Allegiance benchmark of 1.97, and the average member age decreased slightly to 38.7 years. Preventive office visits increased positively by 33.0%, and imaging utilization (CT scans +56.5%, MRI scans +42.2%) exceeds benchmarks, suggesting potential opportunities for care management interventions and high-cost case management strategies to address catastrophic spending and readmission rates.

Questions?
Data sources and reconciliations:
Three separate sources have been used to develop the reports in this presentation: (1) SQL databases populated by LuminX, (2) iBiz, (3) Cedar Gate. In addition, although the reports have been run for the same timeframes, timing differences on receipt and processing of data may not coincide fully. As a result, the numbers presented may not tie to one another completely. For example, Cedar Gate generally excludes terminated groups whereas they are included in the SQL and iBiz databases and member counts in iBiz also include administrative enrollments for purposes of billing PPO and other fees.
Further, several of the reports provide information on claims experience prior to any stop loss reimbursements.
