About this file

This document is a medical and pharmacy claims review report for Collier County Government, covering claims data from January through December 2022, prepared by Cedar Gate. The report provides a comprehensive analysis of medical utilization, costs, and key healthcare metrics for the county's employee health plan, which covers approximately 2,246 employees and 4,736 total members. The review compares current period data to the previous year and benchmarks the plan's performance against government industry standards, highlighting variations in medical spending, utilization rates, and high-cost claimants.

Key findings include a 11.9% decrease in total medical plan paid costs, with 39 claimants (0.8% of members) accounting for 31.5% of total plan spend, averaging $236,000 per high-cost individual. The plan experienced decreased utilization in several areas, including emergency room visits (-25% below benchmark) and inpatient admissions (-28.5% below benchmark), while maintaining higher-than-average costs in categories like office visits (+108.5% above benchmark) and emergency room expenses (+428.8% above benchmark). The top diagnostic categories for high-cost claims were cancer, infections, and cardiac disorders, with 98.1% of high-cost claimants having chronic conditions and 87% having two or more concurrent health conditions.

Questions?

Data sources and reconciliations:

Three separate sources have been used to develop the reports in this presentation: (1) SQL databases populated by LuminX, (2) iBiz, (3) Cedar Gate. In addition, although the reports have been run for the same timeframes, timing differences on receipt and processing of data may not coincide fully. As a result, the numbers presented may not tie to one another completely. For example, Cedar Gate generally excludes terminated groups whereas they are included in the SQL and iBiz databases and member counts in iBiz also include administrative enrollments for purposes of billing PPO and other fees.

Further, several of the reports provide information on claims experience prior to any stop loss reimbursements.
