About this file

This is an Invitation for Bid (IFB) document for the State of Illinois Department of Mental Health (DMH) solicitation B-54172, concerning asphalt and concrete repair services at the Chicago Read Mental Health Center campus located at 4200 North Oak Park Avenue, Chicago, Illinois 60634. The procurement seeks qualified vendors to provide comprehensive roadwork and pavement repair services across the campus facilities. The document establishes bidding procedures, submission requirements, and compliance standards that all interested vendors must follow to be considered for contract award. Bidders are required to complete and submit all documentation specified in solicitation section A.1 as a mandatory requirement, with failure to meet these requirements resulting in automatic disqualification from consideration.

This procurement is designated as a Small Business Set Aside Program solicitation, restricting participation to small business vendors only. Bidders may claim applicable preferences under the Illinois Procurement Code, including the Illinois Business Preference and Domestic Product Preference, provided they submit the required affidavits and documentation to support their claims. The resulting contract will incorporate the State of Illinois Standard Terms and Conditions, which are available on the Illinois Central Procurement Office website. Bidders who object to specific contract terms must submit a redlined version of the Standard Terms and Conditions with their bid submission, though the State reserves the right to accept or reject any proposed exceptions. All submissions must be authorized by a representative with binding authority on behalf of the bidding entity.
