About this file
This is a Certificate of Insurance Requirements checklist for the City of West Sacramento, California, applicable to all standard city contracts. The document establishes mandatory insurance coverage thresholds and endorsement requirements that all contractors and vendors must maintain throughout the contract term. The City of West Sacramento requires contractors to name the municipality and its officers, officials, employees, agents, and volunteers as additional insureds on applicable policies, with certificates of liability insurance submitted to 1110 West Capitol Avenue, West Sacramento, CA 95691. The checklist applies to the Law Enforcement Records Management System (LERMS) and Automated Field Reporting (AFR) contract opportunity, which seeks qualified firms to provide an integrated records management and field reporting solution with associated hardware, software, interfaces, and U.S.-based technical support.
Contractors must obtain Commercial General Liability insurance with minimum limits of $1,000,000 per occurrence and $2,000,000 aggregate, Commercial Automobile Liability coverage of $1,000,000 per accident, Workers' Compensation and Employers' Liability coverage per statutory requirements, and Professional Liability (Errors & Omissions) insurance of $1,000,000 per claim if applicable. All policies except Workers' Compensation and Professional Liability must include Additional Insured Endorsements equivalent to CG 2010 0714, Primary and Noncontributory Endorsements, and Separation of Insureds provisions. Certificates must provide thirty days' written notice prior to cancellation or material changes, and Workers' Compensation policies must include a Waiver of Subrogation Endorsement in favor of the City, its officers, agents, and employees, or a Workers' Compensation Waiver if the contractor is self-employed with no employees. A copy of the Additional Insured Endorsement must be attached to all certificates to be considered complete.
