About this file

This is an Insurance Checklist document issued by the City of West Sacramento establishing mandatory Certificate of Insurance requirements for all standard city contracts. The document outlines comprehensive insurance coverage standards that all contractors and vendors must maintain and submit prior to contract execution. The checklist requires contractors to provide evidence of Commercial General Liability coverage at $1,000,000 per occurrence/$2,000,000 aggregate, Commercial Automobile Liability at $1,000,000 per accident, Workers' Compensation coverage per statutory requirements, and Professional Liability (Errors & Omissions) coverage at $1,000,000 per claim if applicable. All policies except Workers' Compensation and Professional Liability must name the City of West Sacramento as an additional insured, with certificates directed to 1110 West Capitol Avenue, West Sacramento, CA 95691.

The document specifies that all Certificate of Insurance submissions must include additional insured endorsements equivalent to CG 2010 0714, primary and noncontributory endorsements, and separation of insureds clauses for both general liability and automobile liability coverage. Workers' Compensation policies must include a Waiver of Subrogation Endorsement in favor of the City and its representatives, or a Workers' Compensation Waiver for self-employed vendors with no employees. All certificates must provide thirty days' written notice to the Certificate Holder prior to any cancellation or material change. Contractors and vendors are advised to forward this checklist to their insurance agents or brokers, and incomplete certificates lacking required endorsements will not be accepted.
