About this file

This is an Addendum #4 Questions and Answers document issued by the New York State Office of Mental Health (OMH) for IFB 26-016 regarding NYS 988 promotional items. The addendum, dated September 23, 2026, corrects Question #41 regarding decoration methods for promotional items. The solicitation encompasses 14 promotional item categories including foam stress relievers, fidget toys, pens, beanies, t-shirts, baseball hats, die-cut stickers and magnets, notebooks, canvas bags, hand sanitizer, ceramic mugs, and plastic shopping bags, with quantities ranging from 1,000 to 100,000 units. All items must be customizable to print "NYS 988 Promo" branding. The original bid due date was September 10, 2026 at 3:00 PM ET; however, this addendum indicates a revised bid due date of September 24, 2026 at 3:00 PM ET. The anticipated contract term runs from October 1, 2026 through March 31, 2031 as a Purchase Authorization vehicle allowing OMH Central Office personnel to order items on an as-needed basis.

The addendum clarifies critical decoration specifications: embroidery is required for beanies (item #5), baseball hats (item #7), and t-shirts, while screen printing is preferred for t-shirt designs. Vendors must bid on all 14 item categories to be eligible for award. MWBE contracting goals apply at 16% MBE and 14% WBE (30% combined), plus a 6% SDVOB goal. No DBE goal applies. Vendors must demonstrate at least three years of experience in the required business services and maintain New York State Workers' Compensation and Disability Benefits Insurance plus general liability insurance. Annual escalation adjustments are permitted not to exceed 3% or the Consumer Price Index, whichever is less. Award will be made to the responsible and responsive bidder offering the lowest total bid amount. Electronic payment through the New York State Office of the State Comptroller is required unless OMH expressly authorizes paper check payment.

# Question Answer

Decoration method for headwear. For the beanies (item #5) and baseball hats (item #7), the specification calls for two imprint locations. Could OMH confirm whether embroidery, screen print, or heat transfer is required or preferred, and whether the same requirement applies to the t-shirts? Decoration method is a significant cost driver at these quantities.

Embroidery is required for beanies, hats, and t-shirts. OMH would like designs screen-printed on the T-shirts. The beanies and hats should be embroidery
