SUBJECT: Past Performance Survey for Solicitation HTC711-11-R-C001
The U.S. Transportation Command is in the process of selecting a contractor to perform international door-to-door express package delivery service to include time-definite, door-to-door pick-up and delivery, in-transit visibility, and customs processing for packages between "Letter" size and 300 pounds. The successful contractor will perform a full range of courier services for the Department of Defense and other Federal Agencies .
One of the significant considerations in proposal evaluation is the verification of the offeror's past performance on contracts which reflect the offeror's ability to perform on the proposed effort. We depend on information received from sources such as yours , which have had first-hand experience with an offeror, for the evaluation of the offeror's performance on those contracts. If information on this particular contractor has been entered into the Contractor Performance Assessment Reporting System (CP ARS), please annotate that in the appropriate block. We will use this information, as well as the specific information obtained from this survey, to complete our analysis of an offeror's past performance.
Our areas of interest in the offeror are summarized in the enclosed survey. We need your written response no later than 15 April 2011. This schedule will allow us sufficient time to analyze the data prior to the start of evaluations .

Introduction
The Department of Defense (DOD) is conducting a competitive procurement to provide international door-to-door small package express service. Express service includes high volume movement of time-definitive, door-to-door pick-up and delivery, Intransit Visibility (ITV), Syncada participation (payment processing), expedited customs processing, of packages (Letter to 300 lbs) for the Department of Defense and authorized Federal Government Agencies.

Please provide your assessment of the level of performance associated with the contract for which you are a reference. Mark the appropriate block, indicating the contractor’s past performance with the ratings provided below.
Rating Definitions
Exceptional (E) - Performance met all contract requirements and exceeded many to your company’s (or agency’s) benefit. Problems, if any, were negligible and were resolved in a timely, highly effective manner.
Very Good (V) - Performance met all contract requirements and exceeded some to your company’s (agency’s) benefit. There were a few minor problems that the contractor resolved in a timely, effective manner.
Satisfactory (S) - Performance met contract requirements. There were some minor problems and corrective actions taken by the contractor were satisfactory.
Marginal (M) - Performance did not meet some contractual requirements. There were problems, some of a serious nature, for which corrective action was only marginally effective or was not fully implemented.
Unsatisfactory (U) - Performance did not meet most contractual requirements. There were serious problems and the contractor’s corrective actions were ineffective.
Unknown (N/A) - No record of past performance or the record is inconclusive.
