FY26 Hearing Restoration Focused Research Award 6

3. Program Description

The Defense Health Agency Contracting Activity (DHACA) is soliciting applications to this funding opportunity using delegated authority provided by United States Code, Title 10, Section 4001 (10 USC 4001). The CDMRP is the program office managing this FY26 funding opportunity as part of the Hearing Restoration Research Program (HRRP). The CDMRP is located within the Defense Health Agency Research and Development (DHA R&D), which is a part of the Department of Defense, DOD, herein referred to using the secondary title Department of War, DOW. Congress initiated the HRRP in 2017 to provide support for hearing restoration research with the goal of reducing the burden of hearing loss on Service Members.

Appropriations for the HRRP from FY17 through FY24 totaled $70.0 million (M). The FY26 appropriation is $5.0M.

The HRRP aims to deliver groundbreaking solutions for hearing restoration by advancing the understanding, diagnosis, repair and regeneration of the auditory system.

Program priorities: There are no U.S. Food and Drug Administration (FDA)-approved drugs to treat hearing loss associated with sensory, neural, synaptic or central auditory dysfunction.

Although significant progress has been made in the molecular and cellular understanding of hearing loss and regeneration mechanisms in the inner ear, the majority of research is preclinical. The unique anatomical features of the inner ear render it difficult to access and not amenable for biopsy, creating a major hurdle for the clinical validation and translation of preclinical findings. Furthermore, it severely hinders the advancement of diagnostic capabilities beyond behavioral functional tests. As potential therapeutics to treat sensory, neural, synaptic or central auditory dysfunction are discovered, the ability to correctly match patient populations to appropriate interventions and accurately measure outcomes will be crucial for the success of clinical trials. The HRRP challenges the scientific community to design innovative research to overcome these major obstacles to hearing restoration.

3.1. Award History

The HRRP FRA mechanism was first offered in FY19. Since then, 241 FRA applications were received, and 53 were recommended for funding.

3.2. Intent of the FRA

The FRA supports promising basic through translational research that accelerates drug discovery and therapeutic development for hearing restoration after military-relevant auditory system injury.

The FRA may be used to support preclinical studies and/or clinical research. However, the FRA may not be used to conduct clinical trials.

Tinnitus or vestibular-related research is excluded.

3.2.1. Focus Areas

To meet the intent of the funding opportunity, applications to the FY26 HRRP FRA must address research in one or more of the following Focus Areas:

• Improve and accelerate the translation of biological regeneration/repair mechanisms into clinical applications. Research addressing the damage, repair and regeneration of the auditory system after military-relevant injuries is strongly encouraged.

• Develop diagnostic tests that differentiate sensory, neural, synaptic and central processing disorders, that may inform applicability and outcomes for current or future hearing restoration therapeutics.

• Develop reliable in-vitro human models to facilitate the understanding, derivation and characterization of human auditory cells, and/or to facilitate the evaluation of hearing restoration therapies.

3.2.2. Key Elements for the FRA

• Experimental design requirements for studies that aim to develop therapeutic interventions:

○ If the therapeutic intervention aims to restore hearing after established hearing loss, the experimental design must enable or facilitate the eventual connection of the intervention to specific pathology (e.g., sensory, neural, synaptic or central auditory) and the application of the intervention to real-world injuries.

○ If the therapeutic intervention aims to preserve cells/synapses after acute auditory injury, to be administered soon after or even before the injury, the experimental design must demonstrate sufficient consideration of safety criteria that the intervention will have to meet in order for it to be given to people at risk of, but have not yet been diagnosed with, hearing loss.

Refer to Attachment 1, Project Narrative, for additional details on these requirements.

• Funding Levels: The FY26 HRRP FRA offers two Funding Levels to support research at different stages and the exploration/development of ideas of different maturity levels. The applicant will select the funding level during pre-application submission. The funding level should be selected based on the stage and maturity level of the research project, rather than the amount of the budget.

○ Funding Level 1 supports exploratory, high-risk/high-reward research that is in the earliest stages of idea development. Research must have the potential to yield new avenues of investigation, such as new approaches, new research tools or new paradigms.

– Preliminary data are allowed but not required. Applicants must provide solid rationale of the research idea, supported by literature.

– Investigators without prior experience with the auditory system are welcome to submit research ideas to Funding Level 1, provided that the investigating team has sufficient expertise to test the research idea.

○ Funding Level 2 supports the advancement of more mature research toward clinical translation.

– Funding Level 2 applications may focus on any phase of research from basic through translational.

– Preliminary data supporting the readiness and feasibility of the proposed research are required.

– PPIO: The FY26 HRRP FRA Funding Level 2 includes an option for two PIs. One

PI will be identified as the Initiating PI and will be responsible for the majority of the administrative tasks associated with application submission. The other PI will be identified as a Partnering PI. Both PIs should contribute significantly to the

Basic Information | Eligibility | Program Description | Application Contents and Format | Submission Requirements Application Review Information | Federal Award Notices | Post-Award Requirements | Other Information

FY26 Hearing Restoration Focused Research Award 8 development and execution of the proposed research project. If recommended for funding, each PI will be named on separate awards to the recipient organization(s).

Each award will be subject to separate reporting, regulatory and administrative requirements. For individual submission requirements for the Initiating PI and Partnering PI, refer to Section 5.3, Submission Instructions.

3.2.3. Other Important Considerations for the FRA

In accordance with the National Defense Authorization Act for Fiscal Year 2026, Section 732, CDMRP does not support the conduct of painful research (U.S. Department of Agriculture pain category D or E) involving domestic cats or dogs, except for studies relating to military or service animals.

Applications from investigators within the DOW and applications involving multidisciplinary collaborations among academia, industry, the DOW, the U.S. Department of Veterans Affairs (VA) and other federal government agencies are highly encouraged. These relationships can leverage knowledge, infrastructure and access to unique clinical populations that the collaborators bring to the research effort, ultimately advancing research that is of significance to Service Members, Veterans, their Families and the American Public. A list of websites that may be useful in identifying additional information about ongoing DOW and VA areas of research interest or potential opportunities for collaboration can be found in Appendix 10 of the GAI.

3.3. Funding Instrument

The funding instrument for awards made under the program announcement will be grants (31

USC 6304).

3.4. Funding Details

• Funding Level 1:

Period of Performance: The maximum period of performance is 2 years.

Cost Cap: The application’s total costs budgeted for the entire period of performance should not exceed $400,000. If indirect cost rates have been negotiated, indirect costs are to be budgeted in accordance with the organization’s negotiated rate. Collaborating organizations should budget associated indirect costs in accordance with each organization’s negotiated rate.

The applicant may request the entire maximum funding amount for a project that may have a period of performance less than the maximum 2 years.

• Funding Level 2:

Period of Performance: The maximum period of performance is 3 years.
