Correction of Amendment Numbers:

The amendment numbers of the solicitation are in error. The amendment numbers and the date posted are as follows:

Amendment Posting date – 17 August 2015 – this amendment was not given an amendment number. Consider this Amendment 1

Amendment Posting Date – 29 August 2015 – this amendment was given the number of Amendment 1. Consider this amendment as Amendment 2.

Amendment Posting Date – 31 August 2015 – this amendment was given the number of Amendment 2. Consider this amendment as Amendment 3.

REQUEST FOR INFORMATION 2

1. Question: Has the solicitation for HT0014-15-R-0017 been issued yet?

Response: Yes

2. Question: Please confirm that this is a Firm Fixed Price Labor Hour Contract?

Response: Refer to Far Clause 52.216-1 for type contract and the CLINS for “UNIT.” Both items are within the solicitation.

3. Question: Who is the incumbent contractor?

Response: This information will be given to the awardee if requested

4. Question: What is the file naming convention for sanitized and unsanitized copies for each volume?

Response: See table in Other Instructions to Offerors (Addendum 52.212-1) paragraph (L). Use the factor names in both the sanitized and un-sanitized versions.

5. Question: If the staff are working currently at the hospital (LEAD PHYSICIST, Medical PHYSICIST, Dosimetrist, Lead Radiation Therapist, Oncology Registered Nurse), What is the length of service of each?

Response: The requested information is only known to the incumbent contractor. The name of the incumbent will be given to the awardee if requested.

6. Question: Can we have a different rate for unscheduled hours?

Response: No

7. Question: Is 12313 - Registered Nurse II, Specialist the SCA labor category mapped to this requirement? If not, what is the SCA labor category?

Response: Yes

8. Question: Why is Wage Determination information missing from the RFQ?

Response: The solicitation was amended and a revised wage determination guide (WD 05-2103 (rev.-16)) was included – See amendment 2.

9. Question: Can the government please confirm file size constraints for email submissions?

Response: The size of a large file can be affected for a number of reasons, so the size constraints cannot be determined. However, once you have sent your file the solicitation states that you can send an email to confirm receipt of your proposal. Please note – an affirmative answer does not mean that your proposal has been reviewed; only received.

10. Question: Since this is an LPTA procurement, we kindly request that the Offeror may use their CPARS or PPIRS ratings in lieu of submitting Past Performance Questionnaires to our customers for completion. If so, we will confirm in our submission that we have CPARS readily available for that particular reference.

Response: Please refer to the solicitation for mandatory requirements concerning submissions.

11. Question: On page 118 in paragraph (a), the RFP states, “The document shall be submitted in Microsoft Word compatible format and shall not be more than one page for the prime contractor and one additional page for each major subcontractor.” In the Submission Table on page 121, it states that the Past performance volume can be no more than 10 pages. Is each past performance reference citation restricted to one page? Can the government please clarify the requirement?

Response: Yes each past performance is restricted to one page for the prime and one page for each subcontractor. No more than 10 pages are to be submitted.

12. Question: Can the government please clarify the three year recency requirement? Is it three years from the release of the RFP date or three years from the proposal due date?

Response: The three year requirement is from the date of the solicitation.

13. Question: On page 120 in paragraph (e), the RFP states that ‘Each volume shall be submitted in an electronic format either in Microsoft Word or PDF Format.’ On page 121 in the Submission Table, it states that the Compensation Matrix shall be submitted as an Excel file. Should Contractors submit the Compensation Matrix as an attachment to the Technical Volume in an Excel file? If so, can the Government confirm that the Compensation Matrix does not count against the 20 page limitation?

Response: Please refer to the amended submission table in Amendment 2
