1. Question: page 118 of the solicitation stated, “The technical information shall not exceed 20 pages and must be submitted in Microsoft Word compatible format. However, page 121 of the solicitation stated 10 pages for the Technical Recruitment& Retention Plan.”

Response: See Amendment 2, Changed Technical Volume I Maximum Page Limit to 20 pages .

2. Question: Can you please tell me if this is a new requirement?

Response: This is a follow on contract.

3. If not, who is the incumbent?

Response: The awardee will be given the name of the incumbent.

4. What are the current rates?

Response: This information will not be given.

5. Question: Page 51. STATEMENT OF EQUIVALENT RATES FOR FEDERAL HIRES (MAY 2014) -The current 2015-DCB Salary Table for the DC, Baltimore, Virginia, DC-MD-VA-WVA-PA lists equivalent wage rates that are different that the rates shown in the Solicitation documents. The comparative labor rates could be viewed as small and insignificant. However, given this requirement is being evaluated as Lowest Price Technically Acceptable, it is critical that the offeror use the correct pricing model for their submittal. Please provide guidance with regard to which price model should be used.

12. Question: The FBO release states the Response Date is September 17, 2015 at 10:00 a.m. Eastern. Block 8 in SF1449 states offer due date is 15 September 2015 at 10:00 a.m. Which date is correct?

13. Question: Page 118. PART I. TECHNICAL INFORMATION, Subfactor 2: Recruitment and Retention Plan: States “The technical information shall not exceed 20 pages and must be submitted in Microsoft Word compatible format”. Does the 20 page limit include the Compensation Matrix pricing sheets to be submitted in Microsoft Excel and the Recruitment & Retention Plan?

14. Question: For the solicitation for HT0014-15-R-0017, it indicated that the COMPENSATION MATRIX is included as Attachment 7. I don't see an Attachment 7 in the solicitation of August 17, 2015. Do I need to look somewhere else?

Response: Compensation Matrix is incorporated in the solicitation and is labeled Attachment 7. Recommend viewing solicitation in print preview and search for the attachment.

ADDENDUM to 52.212-1

(b) Submission of Offers. The following supplements this paragraph with respect to the information and documents required for submission in response to this solicitation:

(1). ELECTRONIC OFFERS. All offers in response to this solicitation must be emailed directly to Mr. Gerald V. Costa at Gerald.V.Costa.Civ@Mail.Mil. Note: As a reminder all offers must arrive by the time specified in the solicitation.

(2). Offerors may request verification by email to Mr. Gerald V. Costa at Gerald.V.Costa.Civ@Mail.Mil to confirm that their proposal was received by the time specified in the solicitation.

(c) This acquisition is being conducted utilizing the policies and procedures of FAR Parts 8 and 12 in conjunction with appropriate parts of FAR Part 13 for the acquisition of commercial services. The Government intends to award a firm fixed-price personal services contract under the authority of Section 1091 and 1089 of Title 10, U.S.C. Offerors are instructed to ensure that their offered rates DO NOT include costs associated with medical malpractice liability insurance. Offerors must quote a firm-fixed price for each Contract Line Item (CLIN).

(d) The Government intends to award a contract without further communicating with contractors. Consequently, contractors are highly encouraged to quote their best technical and pricing quotes in their initial submissions. However, the Government reserves the right to communicate with any or all contractors submitting a technical and price quote, if it is determined advantageous to the Government to do so. This statement is not to be construed to mean that the Government is obligated to communicate with every offeror (note that FAR Part 15 procedures do not apply to FSS orders, therefore formal discussions are not applicable). A contractor may be eliminated from consideration without further communication if its technical and/or pricing proposals are not among those contractors considered most advantageous to the Government based on a best value determination.

(e) Any questions with respect to the solicitation shall be submitted not later than 28 August 2015 @ 09:00 a.m. EST via EMAIL to Mr. Gerald V. Costa at Gerald.V.Costa.Civ@Mail.Mil, phone calls will not be accepted. Responses to questions will be posted on www.FBO.Gov via RFP amendment.

(f) BASIS FOR AWARD: Award will be made on a Lowest Price Technically Acceptable basis. Award will be made to the Offeror provided its technical proposal conforms to the requirements of the solicitation; it has an acceptable record of past performance; its price is determined to be fair and reasonable by the contracting officer; and the contracting officer determines the offeror is responsible. The Contracting Officer intends to award without discussion, but reserves the right to do so in his or her sole discretion.

(g) Data Universal Numbering System (DUNS) / CAGE Code. The offeror shall obtain these said Codes and submit them to Contracting Center. Registration in the System for Award Management (SAM) https://www.sam.gov/portal/public/SAM/and the Online Representations and Certifications, if you are a U.S. business entity. Award cannot and shall not be made to an offeror without these codes.

(h) Proposal Preparation Costs. The Government will not pay any costs incurred in the submission of an offer, in making any necessary studies for the preparation thereof, or for any visit(s) the Contracting Officer may request for the purpose of clarification of the offer.

(i) The offeror is responsible for ensuring that its proposal is complete. The proposal shall be clear, concise, and shall include sufficient detail for effective evaluation and for substantiating the validity of any stated claims. The proposal shall not simply rephrase or restate the Government's requirements, but rather shall provide convincing rationale to address how the offeror intends to perform the requirement. Offerors shall assume that the government has no prior knowledge of their experience and will base its evaluation solely on the information presented in the offeror's proposal.

(j) Protests received at a level higher than the contracting officer will be referred to the contracting officer. Protestor may request a review by the contracting officer or an independent review at a level higher than the contracting officer. Requests for appeals or independent review will be addressed to the contracting officer for referral to the review authority. Questions on this process will be referred to the cognizant contracting officer identified in the solicitation.

(k) The non-FAR Part 12 discretionary FAR, DFARS, AFARS, and LOCAL provisions included herein are incorporated into this solicitation either by reference or in full text. If incorporated by reference, see provision 52.252-1 herein for locations where full text can be found.

(l) The solicitation should be completed, signed and received at this office not later than the date/time specified in on page one of the solicitation (Standard Form [SF] 1449). Failure to provide all information requested and in the appropriate format as required may cause the offer to be rejected and no longer considered for award. Please note that your offer must include all of the following information. Submit proposal in four (4) separate volumes as follows:

Proposal Copies: Offerors shall provide two separate electronic copies of their proposals. Copy One (1), Not-Sanitized; Copy Two (2), Sanitized. Copy two shall NOT contain the name of or any reference to the Offeror, any Joint Venture Partner, or any Subsidiaries. This copy will be used in the Evaluation of Proposals. Offerors are advised hereby that without a Sanitized copy of their Proposal their submission must be deemed as Non-Responsive, as the proposal cannot be evaluated. FORMAT: submit your proposal in four (4) separate volumes labeled by Factor (one email).

PART I: TECHNICAL INFORMATION: The information provided by the offeror is a significant factor in the Government’s determination of technical acceptability as it pertains to the offeror’s ability to hire qualified personnel as well as to provide continuous service based on a sufficient incentive to retain those qualified personnel. Offerors are responsible for including sufficient details to permit a complete and accurate evaluation of each proposal. Proprietary information shall be clearly marked. Within the technical portion of the offeror’s proposal, the offeror shall provide:

Sub factor 1: Compensation. See attachment entitled “Compensation Matrix”. Offerors are directed to fill-in / populate the “Compensation Matrix” that includes employee base salary amount, employee fringe benefits, as well as company profit, overhead, and G&A for the geographic area of service (Fort Belvoir, VA). If the salary is less than the amount paid in the local area, the offeror must demonstrate that its “Compensation Matrix” is adequate to recruit and retain quality health care providers and ensure uninterrupted performance of services throughout the life of the contract. The escalation factor or other means for price changes in the Option periods must be addressed within the narrative of the “Compensation Matrix”.

Sub factor 2: Recruitment and Retention Plan. The Recruitment and Retention Plan shall describe the offeror’s policies, strategies and procedures to recruit those health care providers with the qualifications as stated in the PWS. Offerors shall describe the methods used and describe the proposed employee benefits provided to retain quality health care providers. Offerors shall describe how they will ensure proper certification, including training and educational plans offered. Offerors shall describe their plans to fill positions that become vacant during performance for the geographic area of service (Fort Belvoir, VA)

The technical information shall not exceed 20 pages and must be submitted in Microsoft Word compatible format. The technical information shall include the company’s name, address, individual names, CAGE Code, DUNS number anywhere on the first page.

PART II: PAST PERFORMANCE

(a) Offerors shall submit the information requested below as part of their proposal for both the offeror and any proposed subcontractors for subcontracts expected to exceed 25% of the requirement. The information may be submitted prior to other parts of the proposal in order to assist the Government in reducing the evaluation time. The document shall be submitted in Microsoft Word compatible format and shall not be more than one page for the prime contractor and one additional page for each major subcontractor.

(b) (1) Offerors shall submit a list of three (3) or more contracts and subcontracts completed in the last three years, including contracts and subcontracts currently in process, which are similar in nature to the requirement.

(2) The contracts and subcontracts listed may include those entered into with Federal, State, local governments and commercial businesses, which are similar in scope, magnitude, relevance and complexity to the requirement described in the RFP. Include the following information for each contract and subcontract listed:

(a) Name of contracting activity.
(b) Contract number.
(c) Contract title.
(d) Contract type.

(e) Brief description of contract or subcontract and relevance to this requirement.

(f) Total contract value.

(g) Period of performance.

(h) Contracting Officer, telephone number and e-mail address, if available.

(i) Program manager/project officer, telephone number and e-mail address, if available.

(j) Administrative Contracting Officer, if different from (h) above, telephone number and e-mail address (if available).

(k) List of subcontracts (if applicable).

(c) Offerors should not provide general information on their performance on the identified contracts and subcontracts. General performance information will be obtained from the references.

(1) Offerors may provide information on problems encountered and corrective actions taken on the identified contracts and subcontracts.

(2) References that may be contacted by the Government include the Contracting Officer, program manager/project officer, or the Administrative Contracting Officer.

(3) If no response is received from a reference, the Government will make an attempt to: contact another reference identified by the offeror, contact a reference not identified by the offeror, or complete the evaluation with those references who responded. The Government shall consider the information obtained from other sources when evaluating an offeror’s past performance (such as PPIRS, FAPIIS, etc.).

(4) Attempts to obtain responses from references will generally not go beyond two telephone messages and/or written requests from the Government, unless otherwise stated in the solicitation. The Government is not obligated to contact any of the references identified by the offeror. It is the offeror’s responsibility to ensure that their references submit the Questionnaire to the Government. The Government is not responsible for the failure of any source to respond to a request for a past performance assessment on a timely basis.

(d) Offerors must send a Past Performance Questionnaire (see Attachment 6 of the Performance Work Statement of the solicitation) to each reference listed in their proposal to assist in the timely processing of the past performance evaluation. Offerors are encouraged to consolidate requests whenever possible (i.e., if the same reference has several contracts, send that reference a single notice citing all applicable contracts). Offerors may send Past Performance Questionnaire to references with copies forwarded to the Contracting Officer.

(1) If an offeror has no relevant past performance history, an offeror must affirmatively state that it possesses no relevant past performance history.

(2) Past Performance Questionnaires should be sent to the individual references on or around the tenth working day after the proposal release date, but no later than 5 working days prior to the proposal closing date. The offeror may forward a copy of the Past Performance Questionnaire to the Contracting Officer simultaneously with mailing to references.

(e) The questionnaire should be emailed directly to Gerald V. Costa at Gerald.V.Costa.Civ@Mail.Mil. ON or BEFORE 09:00AM EST 11 September 2015. The offeror should instruct POCs not to send a copy back to the offeror, but to Gerald.V.Costa.Civ@Mail.Mil as listed on the questionnaire.

(f) Please request that any questionnaire submitted includes the following information in the subject line so the email is not misconstrued as SPAM or otherwise deleted: Past Performance Questionnaire HT0014-15-R-0017.

(g) Past performance information may be used for both responsibility determinations and as an evaluation factor for award. The Past Performance Questionnaire included with the Attachments will be used to collect information on an offeror’s performance under existing and prior contracts/subcontracts for services similar in scope, magnitude, relevance and complexity to this requirement in order to evaluate offerors consistently in accordance with the past performance evaluation factor set forth in the Evaluation Factors for Award clause. References other than those identified by the offeror may be contacted by the Government and used in the evaluation of the offeror’s past performance.

(h) Any information collected concerning an offeror’s past performance will be maintained in the official contract file.

(i) Offerors with no relevant past performance history or for whom information on past performance is not available, will be evaluated neither favorably nor unfavorably on past performance.

PART III: PRICE: Offerors will submit their proposed CLIN prices as presented in the RFP. If the prices are the same for each contract period, provide one schedule and state that the schedule applies to all contract periods. If the prices are not the same for all contract periods, each CLIN price must be separately identified. DO NOT round figures in the pricing schedule.

PART IV: RFP DOCUMENTS: Solicitation, Offer, and Award Documents, Representations and Certifications, and Offeror’s Business Information.

(a) Completed and signed SF 1449, page 1, Blocks 17a., 30 a.- c, of the solicitation. Offeror must also put the total amount of the proposal (to include the summation of all CLINs, base and option periods) into Block 26, page 1 of the SF 1449. The offeror is not required to submit the entire SF 1449. The offeror MUST submit the SF 1449 pages that have been filled out by the offeror.

(b) Signed amendments (if any). Only page 1 of the amendment shall be submitted and must be done so in PDF compatible format. The offeror may combine the signed page 1 of the SF 1449 and Amendments into a single PDF file.

(c) Representations and Certifications found at FAR 52. 212-3 Alt 1 and 52.209-7 - MUST be completed. These must be submitted in Microsoft Word compatible format. The Offeror is encouraged to cut-and-paste these clauses into a separate Microsoft Word document to reduce file size. The Offeror should ensure that the appropriate NAICS Code for this procurement is included in the System for Award Management (https://www.sam.gov/portal/SAM/#1) and in the Representations and Certifications for the company.

(d) A business information document shall be submitted along with the RFP documents. At a minimum, this document must include contact information such as e-mail addresses and telephone numbers for the principals and other information about the business and major subcontractors as applicable. If there are major subcontractors (performing 25% or more of the work), the offeror must state what percentage of the effort the subcontractor(s) will be performing. This document must be in Microsoft Word or Adobe PDF compatible format. At its discretion, the offeror may provide additional information about the company’s history, capabilities, or any other details deemed appropriate. This document must be no more than 3 pages in length.

(e) Each volume shall be submitted in an electronic format either in Microsoft Word or PDF Format. Any revisions will be clearly marked (as a result of negotiations or proposal revisions) and should be of a different color. Each file shall be clearly identified (e.g. Volume I – Technical, Sub factor 2 “Recruitment & Retention Plan”).

(f) A standard, 12-point minimum font size applies. Arial or New Times Roman fonts are required. Tables and illustrations may use a reduced font size no less than 8-point and may be landscape.

(g) Page limitations shall be counted as maximums. If exceeded, the excess pages will not be read or considered in the evaluation of the proposal. Each page shall be counted except for the following:
