DEPARTMENT OF HOMELAND SECURITY (DHS)
Statement of Work (SOW) For
Consumable Medical Supplies (CMS) (Replenishment/Emergency Declaration)
1.0 BACKGROUND
Historically, there has been an absence of adequate and appropriate shelter support and care for sheltered disaster survivors who require Consumable Medical Supplies (CMS) resources. The provision of Consumable Medical Supplies (e.g., medications, diapers, and bandages) are medical supplies which may be ingested, injected, or applied and/or are one time usage. The CMS list herein has been developed by the Federal Emergency Management Agency (FEMA) Mass Care Functional Needs Support Services (FNSS) review group.
This document identifies the basic consumable medical supplies necessary to sustain and support a 100-200 person shelter for a 72 hour period, which FEMA may provide as supplemental assistance to shelter providers when requested by a State; pursuant to Section 403 (b) of the Robert T. Stafford Disaster Relief and Emergency Assistance Act (Public Law 93-288) as amended.
FEMA desires to establish one or multiple Indefinite Delivery Indefinite Quantity (IDIQ) contracting vehicles to procure a completely kitted and packaged CMS Kits and ala carte items, as identified in Attachment 1. “CM-000.” CM-000 consists of line items CM-001 through CM-094.
2.0 SCOPE:
The Contractor shall provide CM-000, and/or individual CMS listed on an ala carte basis, as requested by FEMA in both steady state to maintain stock levels, and for rapid deployment under emergency conditions. Specifically, the Contractor shall:
• Deliver complete Consumable Medical Supply kits to Distribution Centers (CONUS & OCONUS). See Attachment 2
• Replenish the perishable/ non-perishable items within Consumable Medical Supply Kits, stored at FEMA Distribution Centers (Attachment 2)
• Deliver complete kits or ala carte orders to affected disaster areas during a response to a presidentially declared disaster.
3.0 STEADY STATE: Requirements, Deliverables and Delivery Schedule:
The Contractor shall deliver complete CMS kits, as described in Attachment 1 below, to each FEMA Distribution Center located in the continental US (CONUS) and outside the continental US (OCONUS) Attachment 2 below, within 30 days of request. Perishable commodities MUST be separated in kits and identified properly with packing slip, currently. No substitutions or partial deliveries will be permitted without the prior approval by the FEMA Contracting Officer (CO), The Contractor shall provide ala carte items described in Attachment 1 in the quantities requested by FEMA, for delivery to locations specified by FEMA, within 30 days of the request. The maximum quantity during Steady State would be two times that particular line item quantity per kit for CONUS DCs & one times that quantity for OCONUS DCs for the 28 life-cycle items.
FEMA requires a minimum shelf life of 15-18 months from date of delivery. Any item not in compliance with the shelf life terms shall be replaced at the contractor’s expense with a new product six (6) months prior to expiration. The vendor shall ship the replacement items and advise FEMA whether to return the expiring material (with the vendor’s prepaid return postage) or to donation to a non-profit organization on the vendor’s behalf.
During the base year of the contract period of performance, and once again in future years when FEMA transitions to a new system, FEMA will require at least two (2) individuals from the Contractor’s staff to be trained in the agency’s inventory system of record: Logistics Supply Chain Management System (LSCMS). This training will allow Contractor to enter individual shipments for FEMA’s visibility. The training will take place in person in Washington, DC, and be for one day.
4.0 EMERGENCY RESPONSE/SURGE: Requirements, Deliverables and Delivery Schedule
The Contractor shall deliver up to 10 complete CMS kits and/or individual ala carte CMS items as described in Attachment 1 for delivery within 48 hours of task order, to any location within the CONUS or OCONUS (U.S. territories) specified by FEMA. The Contractor shall have a tracking system to provide real time tracking of commodities during a disaster response.
5.0 RECORD KEEPING AND ACCEPTANCE:
A FEMA representative will sign for receipt of kits at each location. This signature(s) does not imply inspection & acceptance. The signature only acknowledges the delivery was made.
Inspection will be made at the point of final destination; any hidden or latent discrepancies will be reported to the CO or COR.
The Contractor shall maintain accurate records of all shipments and deliveries and ensure that proper signatures by FEMA representative are obtained for tracking and accountability purposes, in accordance with government regulations.
6.0 PACKAGING AND LABELING
During Disaster and Steady State operations, commodities often experience multiple movements, CMS Kits should be delivered as a complete packaged kit. Each kit shall contain a packing slip that details the components, quantity and shelf life information.
Shelf life data includes: manufacturer, lot number & expiration/use by date.
The CMS kits shall be packaged in tri-wall containers. The tri-walls will be labeled to identify the kit number, contents, quantity of each item and shelf life information. The tri-walls shall be on a 48L X 40W four-way pallet and have a height of no higher than 55”.
Each tri-wall shall be secured to pallet with plastic or metal banding straps. If kits are not kitted in compliance with paragraph 6.0, the vendor will be notified, and at there on expense must go to appropriate Distribution Center and kit properly according to contract.

CMS KIT
1.1– COMPLETE 94-line item CMS kit (CM-000) consists of line items CM-001 through CM-094. To order complete kit use “CM-000”. The complete CMS kit contains 28 perishable line items (see attachment HIGHLIGHTED) in yellow: CM-003, CM-014, CM-015, CM-017, CM-018, CM-023, CM-024,CM-025, CM-026, CM-032, CM-054, CM-055, CM- 058, CM-60, CM-61,CM-062, CM-067, CM-071, CM-075, CM- 076, CM-077, CM-078, CM-079, CM080, CM-082, CM-083, CM-090,CM-091, and CM-094. All items should be brand name or equivalent with minimum 15 months shelf life.
