QUALITY ASSURANCE SURVEILLANCE PLAN

FOR

GROUNDS MAINTENANCE SERVICES

AT THE

U.S.C.G SECTOR, SAN DIEGO, CALIFORNIA

Introduction: The contractor will provide grounds maintenance services for the Coast Guard Sector, San Diego, California. This is a firm fixed price contract. The services will be performed for one base year (01 Oct 2015 through 30 Sep 2016) and four option years.

Surveillance Program Objectives: The QASP provides a systematic method to evaluate the services the contractor is required to furnish. It is designed to provide an effective surveillance method of monitoring contractor performance for each listed objective on the Performance Requirements Summary (PRS) in the maintenance contract.

This QASP is based on the premise the government desires to maintain a quality standard in maintaining the grounds. The contractor, not the government, is responsible for management and quality control actions to meet the terms of the contract. The role of the government is quality assurance to ensure contract standards are achieved.

Surveillance Strategy and Approach: The primary method of surveillance will be Periodic Inspection as described below. Periodic Inspection will be supplemented with validated customer complaints and Incidental Inspections.

Project Surveillance Sector and Schedule: The Performance Requirements Summary provides a summary listing of all contract requirements and their associated performance. Based on the contractor’s work schedule, the COR will draft a monthly surveillance schedule.

Surveillance Methods: This contract uses three primary methods to evaluate Contractor performance as follows:

PERIODIC INSPECTION. This method, sometimes called “planned sampling” consists of the evaluation of tasks selected on other than a 100 percent or random basis. This method will be used on those items identified in the Performance Requirements Summary Table, Technical Exhibit #6.

a. The COR will perform surveillance of 1/5th of the grounds each weekday covering the entire base each week.

b. The Contractor is required to provide a schedule indicating when each of these services will be performed. Coast Guard will use this schedule and complete its inspections reasonably soon after the work is scheduled.

Page 2 of 3 Technical Exhibit 5

FOR INFORMATION PURPOSES ONLY:

c. If the number of defects observed during a month-long period is equal to or less than the Acceptable Quality Level (AQL), then there will be no deduction in payments to the Contractor for that task. However, if the number of defects observed during a month-long period is greater than the AQL, then there will be a deduction in payments to the Contractor for that task.

EXAMPLE OF PAYMENT COMPUTATION FOR SERVICES NOT PERFORMED

Assume an AQL of 3 complaints per month for Maintain Improved Grounds under CLIN 0001 that 4 defects were found by the Contracting Officer’s Representative (COR).

The payment would be as follows:

(1) Maximum contract payment per month (Total CLIN amount) $10, 000

(2) Maximum payment percentage for the service (Column 3, PWS Table) (Weight) x 20%

(3) Maximum payment for acceptable service $2,000

(4) 4 defectives exceeds AQL

(5) Percentage of lot found unacceptable =20% (4 defects divided by 20 units = 20%)

(6) Percentage of lot found acceptable (100% minus Line 5) 85%

(7) Credit for sample defective corrected = (1 defect corrected) (defects in sample corrected divided by lot size, then multiplied by 100) or (1 divided by 20 x 100) = 5%

(8) Payment for percentage of lot found acceptable service $1,800 after correction (Line 3 times (Line 6 + Line 7))

CUSTOMER COMPLAINTS. Any base employee that observes unacceptable services, whether incomplete or not performed, for any performance objectives should immediately contact the COR. The COR will record the complaint including date and time, customer name, phone number and the nature of the complaint. The COR will respond as soon as possible after receipt of a complaint to conduct a site visit to personally observe the reported defect. If the COR determines the complaint is valid, the COR will complete appropriate documentation to record the complaint and notify the Contractor of the defect. The COR should inform the customer of the approximate time the unacceptable performance will be corrected and advise the customer to contact the Contracting Officer if not corrected. For purposes of calculating deductions, a validated customer complaint will be treated as if it had been identified as part of a periodic inspection.

INCIDENTAL INSPECTION. This unplanned surveillance is used to supplement planned inspection by documenting casually observed performance while in route to or on site at planned sample locations for other contract requirements.

Surveillance Procedures: The government evaluator will inspect all work required by the contract to ensure contractor compliance in accordance with the Performance Work

Page 3 of 3 Technical Exhibit 5

Statement each time service is performed. Record the results of inspection, noting the date and time of inspection. If inspection indicates unacceptable performance, notify the Contracting Officer of the deficiencies and have Quality Correction Inspection (QCI) correct. Contractor shall be given two hours after notification to correct the deficiencies for daily work and 24 hours for all other work following notification.
