Schedule contractors are requested to submit a written technical and price quotation in response to the attached Performance Work Statement (PWS) (Attachment 1) and Quality Assurance Surveillance Plan (QASP) (Attachment 2). Further information on submission of technical and price quotations is provided throughout this document.

IAW with DISA Acquisition Regulation Supplement (DARS) 52.209-9000, each contractor shall specifically identify in its quotation, whether or not any potential or actual Organizational and Consultant Conflicts of Interest (OCCI) as described in Federal Acquisition Regulation (FAR) Subpart 9.5 exists for this this procurement. If the contractor believes that no OCCI exists, the OCCI response shall set forth sufficient details to support such a position. If a contractor believes that an actual or perceived OCCI does exist on this procurement, the contractor shall submit an OCCI plan with the quotation, explaining in detail how the OCCI will be mitigated and/or avoided.

3. Evaluation Factors and Basis for Award Determination:

a. Award will be made to the contractor whose quotation is determined to represent the overall best value to the Government using an LPTA evaluation process. Quotations will be evaluated with regard to how they respond to the requirements of the PWS, and on price submissions. The technical portion of the quotation should include all information the contractor wants the Government to consider and evaluate regarding its company’s ability to perform all required tasks and conform to all required terms and conditions. In order to be eligible for award, the contractor’s quotation must sufficiently address all aspects of the PWS. The Government reserves the right, but is not obligated, to conduct a realism analysis. With regard to the best value analysis for the award decision, the below evaluation factors and subfactors will be used in the determination of technical acceptability. Section 508 Accessibility Standards do not apply to this requirement.

b. After receipt of quotations, the Government will first evaluate the lowest price quotation.

If the lowest price quotation is determined to be technically acceptable and otherwise properly awardable, no further evaluations will be conducted, and award will be made. If, however, the lowest price quotation is determined to be technically unacceptable and/or otherwise not properly awardable, further evaluations will be conducted.

c. The standards for determining technical acceptability are below, as well as information concerning price quotations. Page limitations are identified below under each evaluation factor and will be treated as maximums. If exceeded, excess pages will not be read or considered in evaluation of the quotation. The following items will not be considered when calculating page limits: cover page, table of contents, glossary, executive summary, generic resumes, past performances, OCCI mitigation plans, SCRM plans, and Section 508 compliance. In the event any evaluation notices are issued, page limitations may also be placed on those. The technical portion of the quotation is to be submitted as a Microsoft (MS) Word 2007 or PDF document.

The page format shall have a 1 inch margin using no smaller than an 11 point Ariel, Times New Roman, or Courier New font for all text to include any text contained in a table. The price quotation spreadsheet shall be a separate document from the technical quotation and shall be submitted in MSExcel 2007 format (Attachment 3). The dollar amounts in the price proposal spreadsheet shall be rounded to the nearest dollar. All formulas shall remain present in the spreadsheet for verification. The file size of each document shall not exceed 2MB.

Page limitations shall not be circumvented by including inserted text boxes/pop-ups or internet links to additional information; such inclusions are not acceptable and will not be considered part of a quotation. The Government reserves the right not to consider for award any quotation that does not adhere to the administrative requirements of this RFQ.

(1) Technical/Management Approach Factor: The Technical/Management Subfactors will be evaluated and rated either Acceptable or Unacceptable. In order for a quotation to be rated overall Technically Acceptable, each of the following subfactors must be rated Acceptable.

Technical/management quotations should not exceed 20 pages.

(a) Subfactor 1: Site Survey, PWS Paragraph 6.1.1. In order for this subfactor to be rated Acceptable the vendor must conduct a site survey between 4 - 8 April 2016, 1000 and 1300hrs CEST to review the current room structure, extract measurements, and analyze dimensions to develop a furniture design for the space.

(b) Subfactor 2: Produce Furniture, Fixtures, and Equipment (FF&E) Design, PWS Paragraph 6.1.2. In order for this subfactor to be rated Acceptable the vendor shall produce comprehensive furniture, fixtures, and equipment (FF&E) interior design options, color/finish samples, and dimensional drawings. The furniture, fixtures, and equipment design must complement the existing architecture of the facility.

(c) Subfactor 3: Provide Furniture Part List, PWS Paragraph 6.1.3. In order for this subfactor to be rated Acceptable the vendor shall provide a furniture part list that meets the end user’s requirements.

(d) Subfactor 4: Furniture Delivery and Install, PWS Paragraph 6.3. In order for this subfactor to be rated Acceptable the vendor shall deliver and install the furniture as defined in the PWS.

(2) Price Factor:

(a) Price quotations will be evaluated to determine if they are reasonable and complete.

The Government reserves the right, but is not obligated, to perform a price realism analysis. The total evaluated price will consist of the contractor’s quoted price for all items listed on Attachment 3.

(b) If the Government receives quotes in more than one currency, the Government will evaluate the quote by converting the foreign currency to U.S. dollars (USD) and using rates from www.oanda.com in effect as follows: (1) On the date specified for receipt of quotes, if award is based on initial quotes, otherwise (2) On the date specified for receipt of quote revisions.

4. The Government intends to evaluate quotations and award a contract without exchanges with contractors. However, the Government reserves the right to conduct exchanges or seek clarifications if the contracting officer (KO) determines they are necessary. The Government further reserves the right to reduce the number of quotations being competitively considered to the greatest number that will permit efficient competition among the most highly rated quotations. If a contractor’s quotation is removed from further consideration for award, written notice of the removal will be provided to the contractor. In the event issues pertaining to the proposed contract cannot be resolved to the KO's satisfaction, the Government reserves the right to withdraw and cancel the proposed proposal. In such event, contractors will be notified in writing.

5. Attachment 5 contains provisions that apply to this solicitation and clauses that will be applicable to the resulting award. Some of the provisions require a response from the contractor and will need to be submitted along with your quotation.

6. In response to this RFQ, no terms, conditions, or assumptions will be entertained. If terms, conditions, or assumptions are included in a quotation, the quotation may be excluded on grounds that it failed to comply with the RFQ’s instructions.

7. All quotations received in response to this RFQ shall remain valid for 90 calendar days from the date of receipt of quotations. Further, should the contractor be awarded this contract, the contractor will furnish the items and/or services identified in its quotation at the price quoted to the designated point(s) within the time specified in the schedule.

QUALITY ASSURANCE SURVEILLANCE PLAN (QASP)

1. Contract or Task Order Title: Furniture Purchase design, delivery, and installation.

2. Work Requirements:

• Task 6.1 – Conduct Site Survey, Produce Furniture, Fixtures, and Equipment (FF&E) Design, and Provide

• Task 6.2 – Purchase Furniture

• Task 6.3 – Furniture Delivery and Installation

3. Primary Method of Surveillance: There will be Customer Input, Design Review, and Periodic Inspection on

Task 6.1 and 6.2. There will be 100% inspection on Task 6.3.

100 Percent Inspection. This is usually only the most appropriate method for infrequent tasks or tasks with stringent performance requirements, e.g., where safety or health is a concern. With this method, per-formance is inspected/evaluated at each occurrence. One hundred percent inspection is too expensive to be used in most cases.

Random Sampling. This is usually the most appropriate method for recurring tasks. With random sam-pling, services are sampled to determine if the level of performance is acceptable. Random sampling works best when the number of instances of the services being performed is very large and a statistically valid sample can be obtained. Computer programs may be available to assist in establishing sampling proce-dures.

Periodic Inspection: These services are monitored weekly, monthly, quarterly, semiannually, annually, etc. Periodic types of activities are perfect for periodic inspection because not only are they infrequent, but there is normally a predetermined, specified time frame within which the tasks must be accomplished.

Therefore, you know exactly when to conduct the evaluations. Periodic inspections automatically become 100 percent evaluations or "100 percent checks." Inspections should be divided and scheduled by frequen-cy: annual, semiannual, quarterly, monthly, weekly and as required. Sometimes services are required for which the time or frequency cannot be predicted, such as accident investigations, one-time special tasking by higher headquarters, etc. These would be labeled "as required inspections." Others are known and pre-dictable such as the quarterly status report or the monthly travel report currently included in some DISA service contracts.

Customer Input. Although usually not a primary method, this is a valuable supplement to more systemat-ic methods. For example, in a case where random sampling indicates unsatisfactory service, customer complaints can be used as substantiating evidence. In certain situations where customers can be relied upon to complain consistently when the quality of performance is poor, e.g., dining facilities, building services, customer surveys and customer complaints may be a primary surveillance method, and customer satisfac-tion an appropriate performance standard. In all cases, complaints should be documented, preferably on a standard form.

4. Scope of Performance: This specification establishes the minimum requirements for the purchase and installa-tion of furniture. Contractor shall provide all labor, tools, and new supplies as described in the Performance Work Statement (PWS). The contractor shall conduct a site survey to review the current room structure, extract measure-ments, and analyze dimensions to develop a furniture design for the space. The contractor shall provide a design compatible with PWS Attachment 1 layout. All furniture parts and product information will be disclosed to ensure the proposed furniture meets the requirements of the PWS.

The Contracting Officer's Representative (COR) and the Contracting Officer (KO) will evaluate the initial submission and will conduct ad hoc reviews of the contractor's compliance throughout the remainder of the contract. The contractor will monitor compliance as required by FAR 52.222-50, Combating Trafficking in Persons, and report any issues to the KO.
