QUALITY ASSURANCE SURVEILLANCE PLAN (QASP)
PURPOSE. This Quality Assurance Surveillance Plan (QASP) is a Government developed and applied document used to make sure that systematic quality assurance methods are used in the administration of the Multiple Award Boat Charter Services Agreements. The intent is to ensure that the Contractor(s) perform(s) in accordance with performance metrics set forth in the contract documents, that the Government receives the quality of services called for in the contract and that the Government only pays for the acceptable level of services received.
This QASP is scaled to the risk, complexity and criticality of the requirement and specifies for the contractor, how and when the Government will assess the contractor's performance of the tasks set for in the Statement of Work (SOW). Because this assessment is made against the standards of the PWS, the QASP and the PWS are consistent and coordinated.
1. AUTHORITY. Authority for issuance of this QASP is provided under the Inspections clause of the Agreement which provides for Government inspections and acceptance of services performed, workmanship and material furnished or utilized in the performance of services.
2. SCOPE. The Contractor(s), and not the Government, is responsible for performing or having performed all inspections necessary to substantiate that the services furnished under this contract conform to the contract requirements. The QASP on the other hand identifies Government surveillance of the Contractor's quality control efforts to assure that they are timely, effective and are delivering the results specified in the contract.
The Government may provide the Contractor an information copy of the QASP as an Attachment to the solicitation.
3. RESPONSIBILITIES. The Government resources shall have responsibilities for the implementation of this QASP as follows:
3.1. Contracting Officer - The Contracting Officer ensures performance of all necessary actions for effective contracting, ensures compliance with the terms of the contract and safeguards the interests of the United States in the contractual relationship. It is the Contracting Officer that assures the Contractor receives impartial, fair, and equitable treatment under the contract. The Contracting Officer is ultimately responsible for the final determination of the adequacy of the Contractor's performance. Funding and scope surveillance will be retained by COR.
3.2. Contracting Officer Representative - A COR will be appointed in accordance with the USSOCOM &
NAVSPECWARCOM policy and guidance. The COR is responsible for technical administration of the project and assures proper Government surveillance of the Contractors performance. The COR is not empowered to make any contractual commitments or to authorize any contractual changes on the Government's behalf. Any changes that the Contractor deems may affect contract, price, terms, or conditions shall be referred to the Contracting Officer for action. COR limitations are contained in the written letter of designation.
3.3. Contract Administration will not be delegated beyond NAVSPECWARCOM.
3.4. METHODS OF QA SURVEILLANCE. The below listed methods of surveillance may be used in the administration of this QASP. Surveillance will be performed, as stated in the QASP, in order to apply consistent surveillance to the contractor’s assessment process with the objective to assess the contractor's performance against the established criteria. Adherence to the QASP may include the following identified QA Surveillance Tasks:
3.4.1. Level of documentation:
3.4.1.1. Notification of Unsupportable Request, PWS, paragraph 5.5
3.4.1.2. Pre-action Estimate, PWS, paragraph 5.6
3.4.1.3. After Action Report, PWS, paragraph 5.7, Not applicable.
3.4.1.4. Additional Reports as required, PWS, paragraph 5.8
3.4.2. Procedures for resolving discrepancies (project meetings, report detailing issue and recommend action, weekly telecoms);
3.4.3. How the Quality Assurance Evaluator (QAE) will notify the contractor of deficiencies (weekly telecoms, written reports via email);
3.4.4. How the QAE will determine the contractor’s degree of responsibility (review of work accomplished against SOW).
4. SURVEILLANCE PROCEDURES
a. The COR may develop a schedule of surveillance activities based on the Performance Standards as outlined in the contract and Performance Requirements Summary. The surveillance outlined in the schedule details information to provide the Government with the objective quality evidence required to support acceptance of a monthly contract invoice. The schedule provides periodic surveillance during all required hours of the Contractors operation. The schedule is "FOR OFFICIAL USE ONLY" and is not releasable to anyone other than authorized Government personnel. The schedule will identify the date of inspection, activity to be monitored, and who will conduct the surveillance (dates and times can be altered without a formal change to the schedule as long as all inspections are performed). The monthly schedule will be completed no later than seven calendar days prior to the beginning of the period it covers and a copy forwarded to the KO for information and review.
b. The COR will monitor performance in accordance with the schedule and will ensure all services required by the order/contract are successfully performed. All inspections will be documented and include as a minimum date and time, who performed the inspection, what was inspected, to what standard and the results. Successful or exceptional performance will be documented as well as deficiencies in Contractor performance. Documentation will be maintained for future reference, audit, and proof of inspection. Any uncorrected unacceptable observation will result in an unacceptable rating, a corrective action request to the Contractor and notification to the KO.
c. The COR will notify the Contractor, in person, each time an unacceptable observation has been recorded and ask the Contractor to correct the problem. The COR will record the contract requirement, the specific deficiency to the requirement, the date and time it was discovered, and have the Contractor initial the entry. The Contractor will be given two working days after notification to correct the deficiency in accordance with requirements of the contract.
Deficiencies that cannot be corrected within two working days will be reported to the Contracting Officer.
d. The COR will report more serious (systemic in nature) deficiencies or recurring deficiencies in the same area that could indicate a trend. The COR will forward copies of CDRs to the KO within one working day. The KO will notify the Contractor and request corrective action. The KO may require the Contractor to re-perform any services that do not meet contract requirements at no additional cost to the Government. The Contractor will return a copy of the completed and signed CDR to the KO for inclusion in the contract/order file.
e. The COR will re-inspect services that were found deficient to ensure corrections were made within the timeframe outlined in the contract or other agreed upon time. The COR will not consider the services complete until all deficiencies have been corrected. Contractor's failure to complete corrective actions will be reported immediately to the KO for further action.
f. The COR will accept the services provided and authorize payment, typically on a monthly basis or upon satisfactory completion of the work. This is done by approving the Contractor's invoice in Wide Area Workflow.
5. DOCUMENTATION
The COR will be responsible for surveillance of contract services and receipt and acceptance of all reports.
The COR will ensure subsequently review contractor monthly invoices through Wide Area Workflow.
Contractor invoices will be executed and paid pursuant to DFARs 252.232-7003, entitled, Wide Area Work Flow Instructions and Information.
6. REVISIONS
Revisions to this QASP are the joint responsibility of technical customer and the contracting office. The Contracting Officer must approve all revisions prior to execution.
7. CUSTOMER COMPLAINT RECORD
This record is used to document customer complaints of unsatisfactory performance, the validity of the complaint, time and date the responsible official notified, and the actions taken to correct the problem.
Customer complaints shall be handled in the following manner: When a complaint is received, the COR will complete appropriate documentation to record the complaint on a DA Form 5477. The COR will notify the Contracting Officer and instructions will be provided based upon the nature of the complaint.
8. UNACCEPTABLE PERFORMANCE
If the number of complaints/defects exceeds the performance threshold for any item in the PRS, the cause of unacceptable performance must be determined. Government-caused complaints/defects shall not be counted against the contractor. If the contractors' performance is judged unacceptable on PRS items or any other requirement of the contract, the COR shall notify the Contracting Officer for appropriate action in accordance with the Inspection/Acceptance procedures in the Agreement.
