About this file

This is a questionnaire appendix for an electrical contractor solicitation (GSS27748-ELECTRICIAN) that supplements technical proposals for signs and sign installation work. The document establishes a structured format for contractors to provide detailed responses across three primary question sets without modifying the worksheet structure or using special formatting, formulas, or non-text elements.

The questionnaire addresses three service categories with specific markup and operational requirements. Question Set 1 focuses on supply markup percentages applied to actual invoiced costs of materials, equipment, and supplies, requiring contractors to explain calculation methods, cost component inclusions, and any exceptions to standard markup rates. Question Set 2 addresses rental equipment markup percentages for third-party equipment such as aerial lifts, bucket trucks, trenchers, generators, and compressors, requiring contractors to clarify whether markups apply to actual rental invoices, detail cost components included in the markup, identify any category-specific rate variations, and specify hourly minimums for bucket truck rental. Question Set 3 concerns solar panel services including design, installation, integration, testing, commissioning, and maintenance of photovoltaic systems compliant with the National Electrical Code Article 690 and applicable building codes, requiring contractors to confirm service provision, identify qualified personnel including NABCEP-certified solar technicians, describe their complete solar project process including design and utility interconnection procedures, and detail warranties, performance guarantees, and commissioning verification procedures.

Appendix C - Questionnaire

Signs and Sign Installation
GSS27748-ELECTRICIAN
Question Set 1:1 Additional Services - Supply Mark up Percentage
Supply Mark‑Up Percentage

The Supply Mark‑Up Percentage is the contractor‑applied percentage added to the actual invoiced cost of materials, equipment, and supplies required to perform the electrical work. This percentage represents the contractor’s administrative handling, procurement overhead, and profit associated with acquiring and delivering materials to the project site.

QuestionResponse
What is your supply mark up percentage?
How is your supply mark‑up percentage calculated, and what specific cost components does it include?
Will you provide supplier invoices for all materials, and is the mark‑up applied strictly to the actual invoiced cost?
Are there any materials, equipment, or subcontracted items that use a different mark‑up rate, and under what conditions would those rates apply?
Question Set 2:1 Add'l Services Rental Equipment Mark Up Percentage
Rental Equipment* Mark‑Up Percentage

The Rental Equipment Mark‑Up Percentage is the contractor‑applied percentage added to the actual rental invoice cost of equipment obtained from an external rental company. This percentage represents the contractor’s administrative handling, coordination, overhead, and profit associated with procuring and managing rented equipment.

*Rental Equipment Rental Equipment refers to tools, machinery, vehicles, lifts, and specialized devices that the contractor does not own and must obtain from a third‑party rental provider to perform the required electrical work. Rental Equipment includes, but is not limited to, aerial lifts, bucket trucks, trenchers, generators, compressors, and other project‑specific machinery necessary to complete the scope of work.

QuestionResponse
What is your rental equipment mark up percentage?
Is the rental equipment mark‑up applied to the actual rental invoice cost, and will you provide the original rental invoices for verification?
What specific cost components are included in your rental mark‑up (e.g., delivery, fuel, environmental fees, insurance, administrative handling)?
Do any rental categories (e.g., specialty lifts, trenchers, generators, bucket trucks) use a different mark‑up percentage, and under what conditions would those rates apply?
Do you have an hourly minimum for bucket truck rental? If so, what is the minimum number of hours for rental?
Question Set 3:1 Solar Panel Services
Solar Panel Services

Solar Panel Services refer to the design, installation, integration, testing, commissioning, maintenance, and performance verification of solar photovoltaic (PV) systems. These services must be performed by qualified personnel with demonstrated expertise in PV technology and must comply with the National Electrical Code (NEC), including Article 690, applicable building codes, manufacturer specifications, and utility interconnection requirements.

QuestionResponse
Do you provide Solar Panel Services (installation, integration, or maintenance)?
Do you employ Solar Technicians, NABCEP (National American Board of Certified Energy Practioners);, and engineers specializing in PV (photovaltaic) Systems ?
Can you describe your full solar project process—including system design, structural evaluation, electrical integration, and utility interconnection—and identify which parts you self‑perform versus subcontract?”
What warranties, performance guarantees, and commissioning/testing procedures do you provide for solar installations, and how do you verify system output after installation?
