GENERAL INSTRUCTIONS
1) Reference Section L.5.2 for complete instructions.
2) This attachment provides a section for each of the Relevant Experience Projects submitted for Pool 2. If applying for
Pool 1, the Offeror shall also submit Attachment J.12.1 (Relevant Experience Projects Supplemental Form Pool 1).
3) If one or two of the Relevant Experience Projects is based on a “collection of task orders”, use Attachment J.12.4
(Relevant Experience Projects Supplemental Form Pool 2-Collection of Task Orders) for each “collection of task orders” submitted. Do not fill out Relevant Experience Projects 5 and 6, if submitting two “collection of task orders” in lieu of Relevant Experience Projects. Do not fill out Relevant Experience Project 6, if submitting one “collection of task orders” in lieu of a Relevant Experience Project.
